Megjegyzések:Although the quantification of health outcomes in a health impact assessment (HIA) is scarce in practice, it is preferred by policymakers, as it assists various aspects of the decision-making process. This article provides an example of integrating a quantitative risk appraisal in an HIA performed for the recently adopted Hungarian anti-smoking policy which introduced a smoking ban in closed public places, workplaces and public transport vehicles, and is one of the most effective measures to decrease smoking-related ill health. METHODS: A comprehensive, prospective HIA was conducted to map the full impact chain of the proposal. Causal pathways were prioritized in a transparent process with special attention given to those pathways for which measures of disease burden could be calculated for the baseline and predicted future scenarios. RESULTS: The proposal was found to decrease the prevalence of active and passive smoking and result in a considerably positive effect on several diseases, among which lung cancer, chronic pulmonary diseases, coronary heart diseases and stroke have the greatest importance. The health gain calculated for the quantifiable health outcomes is close to 1700 deaths postponed and 16000 life years saved annually in Hungary. CONCLUSION: The provision of smoke-free public places has an unambiguously positive impact on the health of the public, especially in a country with a high burden of smoking-related diseases. The study described offers a practical example of applying quantification in an HIA, thereby promoting its incorporation into political decision making.

Megjegyzések:Background The positive association between physical activity (PA) and heart rate (HR) related cardiovascular (CV) fitness and CV risk is well established, but it is less well known how the type and intensity of PA have influence on the efficacy of the risk reduction. The aim of the present study was to investigate the association between CV fitness and three intensity categories of PA in four domains in the Hungarian population. Methods Data for our study were obtained from a cross-sectional survey (n?=?665) conducted in Hungary in 2018. Individual PA was assessed using the IPAQ long form and expressed as the metabolic equivalent of task minutes per week for light, moderate and vigorous activities for total and each domain (work, transport, domestic and gardening - dom. & gard., leisure-time - LTPA). CV fitness was assessed using the YMCA 3-minute step test, with HR measured at rest (HRrest) and immediately after exercise (HRaft) and their difference (?HR) was calculated. Multiple regression analysis was used for the statistical analyses. Results Total PA (B=-0.001; p?<?0.001), its vigorous (B=-0.001; p?=?0.001) and moderate (B=-0.001; p?<?0.001) intensity forms, and LTPA (B=-0.002; p?<?0.001) showed a significant association with both HRaft and ?HR, whereas dom. & gard. domain (B=-0.001; p?=?0.010) is only significantly associated with ?HR. Vigorous intensity form of dom. & gard. (B=-0.002; p?=?0.006) and moderate intensity form of LTPA (B=-0.004; p?<?0.001) showed individually significant associations with HRaft but not with ?HR. Transport by bicycle was the only form of PA that showed a significant association with both HRaft (B=-0.004; p?<?0.001) and ?HR (B=-0.003; p?=?<0.001). Conclusions Based on our results, among the domains of PA, dom. & gard. and LTPA, both moderate and vigorous intensity forms, as well as the use of bicycles for transport showed the strongest association with favourable CV fitness, and their promotion in public health interventions is recommended. Key messages ??Recommendations should consider that not all forms of physical activity significantly improve individual cardiovascular fitness. ??Interventions targeting the improvement of cardiovascular fitness should improve physical activity in leisure time and the use of bicycles for transport.

Megjegyzések:Background Mental disorders constitute a public health problem in the general population and in the medical profession as well. Medical doctors as professionals are not only helpers in matters of health and disease but also role-models for patients. Therefore the mental health and health behaviour of future doctors is an important research issue. This was the focus of a health survey conducted among medical students at one university in Hungary. Methods A representative sample of all 1?5-year students filled a standardized self-administered questionnaire. The question naire included items on demographic data, self-perceived health, the 12-item version of the general health questionnaire (GHQ-12), a domain on social support, the abbreviated (13-item long) version of the sense of coherence scale, ques tions on body weight & height, physical activity, diet, smoking, alcohol & drug use and sexual behaviour. Results Eighty-one percent of invited students filled the questionnaire. Self-perceived health was good or very good according to more than three-quarters of the students. Mental health was mea sured by the 12-item general health questionnaire (GHQ) on which 19% of the students scored above the strict threshold indicating mental health problems. The prevalence of mental health problems was higher among medical students than in the same age group of the general population. The mean score of sense of coherence (SOC) measured by the abbreviated Antonovsky questionnaire was 62.5 (95% CI 60.3?64.7). SOC showed strong correlation with self-perceived health and mental health but not with health behaviour, specifically physical activity, diet and substance use. Conclusion The relatively high prevalence of mental health problems draws attention to the needs of future medical doctors to learn more about determinants of their mental health and to improve their stress-coping skills.
