Megjegyzések:Introduction: Traditional prognostic indices, while widely used, exhibit limitations in comprehensively delineating the clinical heterogeneity of follicular lymphoma (FL). Positron emission tomography combined with computed tomography (PET/CT) during upfront treatment is investigated as a potential method for refining prognostication and informing thera- peutic decision-making in FL. This study was designed to evaluate the prognostic value of interim PET/CT imaging in newly diagnosed FL patients undergoing front line therapy. Methods: This retrospective, single-center evaluation comprised 114 treatment-naive FL patients who had PET/CT imag- ing, performed after 3-4 cycles of front line therapy between 2009 and mid-2025. Notably, the utilization of interim PET/CT scans was not uniformly adopted as a standard practice during the investigated period, this study was granted approval by the Regional and Institutional Research Ethics Committee at the University of Debrecen. The primary outcome measure was progression-free survival (PFS). The prognostic significance of positive PET/CT results performed after 3-4 cycles of treat- ment, SUVmax values and Deauville scores were assessed in conjunction with other clinical parameters. A Deauville score of 1-3 was considered a negative, while a score of 4-5 was considered a positive examination. Patients received chemo im- munotherapy if treatment initiation was indicated according to GELF criteria, anti-CD20 monoclonal antibody and rituximab, cyclophosphamide, vincristine, prednisolone or bendamustine or cyclophosphamide, doxorubicin, vincristine, prednisolone chemotherapy combination. Results: The median age was 54 years at diagnosis, and median follow up was 45 months. Thirty-two patients (28.1%) pre- sented with positive interim PET/CT results. FL patients who had a Deauville score of 1-3 had significantly superior PFS com- pared to those, with Deauville score 4-5 (median PFS not reached vs. 14 months, p 0.0001) at the interim time point. In particular, those FL patients who demonstrated a SUVmax value of more than 3.315, determined by a receiver operating characteristic (ROC) analysis, at the interim PET/CT had a significantly reduced PFS as compared to those with lower values (median PFS 42 months vs. not reached, p 0.0001). We did not observe a survival correlation with SUVmax change and/ or ratio of the baseline and interim examination.Discussion: In conclusion, our data may strengthen the otherwise unclear prognostic value of interim PET/CT in FL. However, these findings are tempered by the study's retrospective design, single institution data collection, and limited sample size. Nevertheless, the most effective PET/CT measurement remains a topic of discussion. Additional studies are warranted to confirm our observations and to investigate individualized treatment ap- proaches informed by interim PET/CT findings.

Megjegyzések:Objective: Follicular lymphoma (FL) is an indolent, lymphoproliferative disease of B-cell origin that has a heterogeneous disease course with varying outcomes. Certain patients may undergo autologous stem cell transplantation. We investigated the outcome of autologous stem cell transplantation in patients with FL. Methods: Patients who received autologous stem cell transplantation at the University of Debrecen's Department of Hematology between 2004 and 2021 were retrospectively analyzed. The overall survival (OS) and progression-free survival (PFS) after transplantation of patients with FL were examined. Prognostic factors that may influence the course of the disease were chosen. Results: Data were collected from 49 patients. OS was influenced only by age, whereas PFS was affected by age and the lymphocyte/monocyte ratio. The combination of age and lymphocyte/ monocyte ratio defined a patient population with a particularly unfavorable prognostic risk profile: patients over 47 years of age with a pre-transplant lymphocyte/monocyte ratio greater than or equal to 2.675. Conclusion: Age and lymphocyte/monocyte ratio were identified as useful prognostic factors for PFS in patients with FL following autologous stem cell transplantation.

Megjegyzések:Very few studies have determined the prognostic value of interim and restaging PET/CT in patients with Hodgkin lymphoma using current standard of care therapy outside clinical trials. We analyzed the effect of the results of interim and restaging PET/CT on the survival (overall- and relapse-free) in patients who received standard first-line treatment based on the stage of disease and risk factors. We investigated the differences between the relapse and non-relapse groups based on the clinical pathological characteristics of patients who had positive interim PET/CT results. Between January 1, 2007 and December 31, 2011, the staging, interim and restaging PET/CT scans of patients with Hodgkin lymphoma were analyzed. The Deauville criteria were used for the evaluation of interim PET/CT scans. One hundred and thirteen Hodgkin lymphoma patients underwent staging, interim and restaging PET/CT scans. None of the therapy was modified based on the interim PET/CT results. The median follow-up time was 43.5 months. A total of 62 early stage patients and 51 advanced stage patients were identified. The five-year overall survival rates were 93.4% in the interim PET negative group and 58% in the interim PET positive group (p<0.001). The five-year relapse-free survival rates for the negative and positive groups were 92.7% and 40.8%, respectively (p<0.001). The negative predictive value was 100% in the early stage group and 82.35% in the advanced stage group. By comparison, the positive predictive values were 53.8% and 58.8%, respectively, in these two groups. In the interim PET positive group, patients over 40 years of age had a significantly higher probability of relapse (p=0.057). The routine clinical use of interim PET/CT is highly recommended based on our investigation. However, patients with positive interim PET/CT results required frequent additional evaluations.

Megjegyzések:Hodgkin lymphoma shows a well-known geographic pattern, but temporal changes have been found recently as well. Patients and Methods. 439 Hodgkin lymphoma patients' clinicopathological and treatment data were processed in calendar periods of approximately ten years. The patients were treated at our department from 1980 until the end of 2008. Results. The first period (1980?89) contained 177 patients, the second (1990?99) 147, and the third (2000?08) 115 Hodgkin lymphoma patients. The mean age of the patients was 40.1, 35.9, and 36.8 years in order. The male/female ratio: 1.42, 1.45, 1.05 in order. Contrary-wise a unimodal age group pattern could have been seen with an incidence peak between 30 and 39 in the past decades. The incidence of classical mixed cellularity histological subtype is decreasing (43.7%, 58.23%, 42.6%, P=0,00098 (it is only significant in the second period)); classical nodular sclerosis shows an increasing tendency (25%, 27.32%, 34.78%,P=0.1734). The first incidence peak is predominantly created by classical nodular sclerosis, meanwhile the second peak by classical mixed cellularity. The number of early-stage patients (59.12%) is beyond the advanced stage (40%) in the last decade. Meanwhile, the number of second-stage patients was increasing (25.8%, 26.35%, 49.56%, P=0.0001) and of patients in third stage was decreasing (53.4 %, 50.67%, 20%?P<0.0001). The 5- and 10-year overall survival data were progressing: 59.7 %, 77.4 %, and 90.5 % and 44.1 %, 70.6 % and 90.5 % (expected survival) in the last decade. Conclusions. Changes can be explained by the altered nature of Hodgkin lymphoma, the changes in socioeconomic status and the development of diagnostic and therapy methods.
