Background: Managing patients with fatigue is a clinical challenge. Because fatigue is often reported in hypothyroidism, thyroid hormone (TH) therapy may sometimes be incorrectly considered for biochemically euthyroid individuals. This study aimed to evaluate the prevalence and determinants of this practice in different European countries. Methods: We analyzed aggregate data from the THESIS (Treatment of Hypothyroidism in Europe by Specialists: an International Survey) online survey. We analyzed responses from 5,695 members of 28 national endocrine/thyroid societies' specialists to the statement:"Thyroid hormones may be indicated in biochemically euthyroid patients with unexplained fatigue" . Results: Overall, 7.5% (426/5695) of respondents indicated that TH therapy might be considered for euthyroid patients with unexplained fatigue. The proportion of positive responses varied widely across different countries (between 1.1% in Switzerland and 29.3% in Serbia; p=2 ?10-16) and regions (between 4.7% in Western Europe and 8.7% in Western Asia or 8.8% in Eastern Europe; p=0.004). TH were more frequently prescribed for unexplained fatigue by male respondents (Odds Ratio, OR 1.45, 95% CI 1.18-1.78) and physicians practicing in private practice (OR 1.27, 95% CI 1.02-1.58), and less frequently by endocrinologists (OR 0.62, 95% CI 0.46-0.83). Conclusion: A small, yet not negligible percentage of European thyroid-focused physicians consider using TH for euthyroid patients with unexplained fatigue, with signi cant variations based on geographic, demographic, and practice- related factors. Using levothyroxine and/or liothyronine in such cases lacks evidence and may partially contribute to the concerning overuse of TH therapy.

Arterial hypertension represents a major global health concern; more than one fourth of the population is affected by high blood pressure. Albeit the underlying cause of the disease remains unclear in the vast majority of the cases, approximately ten percent are of secondary origin. Endocrine disorders are common illnesses and some of them may lead to elevated blood pressure, among which thyroid diseases are of high prevalence and often overlooked, especially in mild cases. Overt and subclinical hyper- and hypothyroidism can both lead to (mostly mild) hypertension; however, the underlying mechanisms are only partially understood. The results of clinical studies are often controversial. During the past decades, some genetic mutations in the hypothalamus-pituitary-thyroid axis with cardiovascular consequences were revealed. Atherosclerotic changes resulting from lipid abnormalities due to thyroid dysfunction also affect the vasculature and can cause elevated blood pressure. The review gives a synopsis of our knowledge how thyroid hormone metabolism and functional thyroid diseases affect the cardiovascular system, their negative impact and causative role in the development of hypertension.

Background: Insulinomas are rare insulin-secreting neuroendocrine neoplasms of the pancreas. First-line treatment is the surgical removal of the tumor, however, the localization with standard imaging techniques is often challenging. With the help of selective intraarterial calcium stimulation the insulinoma's localization can be narrowed down to one third of the pancreas which the selected artery supplies. Objective: We aimed to prove the usefulness of the calcium stimulation test in case of 9 patients treated between 2006 and 2021 diagnosed with endogenous hyperinsulinemic hypoglycemia confirmed by fasting test, where conventional imaging methods, like transabdominal ultrasound, CT or MRI failed to detect the source of hyperinsulinemia. Methods: We performed selective intraarterial calcium stimulation with angiography with calcium gluconate injected to the main supporting arteries of the pancreas (splenic, superior mesenteric and gastroduodenal arteries); blood samples were obtained from the right hepatic vein before, and 30, 60 and 120 seconds after calcium administration. Results: With selective angiography we found a significant elevation of insulin levels taken from the right hepatic vein in five of the nine cases. On histopathology, the lesions were between 1-2 cm, in one case malignancy was also confirmed. In four patients we found a significant rise of insulin levels obtained from all catheterized sites, which confirmed the diagnosis of nesidioblastosis. In three cases no surgery was performed, and the symptoms relieved with medical treatment. Conclusions: Selective intraarterial calcium stimulation remains an important tool in localization of the source of insulin excess, especially in cases where other diagnostic modalities fail.

Purpose: Treatment-resistant depression (TRD) is most commonly de ned as depression that has not responded to two different pharmacological agents used for an adequate period of time. We explored the views of European specialists via survey, regarding the use of thyroid hormone (TH) in euthyroid patients with TRD as part of♭Treatment of Hypothyroidism in Europe by Specialists: An International Survey' (THESIS). Methods: The question"Thyroid hormones may be indicated in biochemically euthyroid patients with treatment resistant depression" was posed to specialists from 28 countries. Results: 5695 valid responses were received following 17,232 invitations (33.0% response rate; 65% female, 90% endocrinologists). 348 (6.1%) stated that TH may be indicated in biochemically euthyroid patients with TRD. This view was more common in males (p<0.01), respondents who saw ?100 patients with hypothyroidism per year (p<0.01), respondents who worked in private practice (p=0.05) and respondents who were not members of international specialist associations (p=0.05). Geographical variation existed with respondents in Eastern Europe signi cantly more likely to use TH in TRD (p<0.01). Linear regression showed a statistically signi cant reduction in the use of TH for TRD with increasing gross national income (F-statistic=7.35, CI -0.15 - -0.02, p=0.01). T H i n T R D w a s r e c o m m e n d e d i n p s y c h i a t r y g u i d e l i n e s b u t n o t endocrinology guidelines. Conclusion: While there is limited evidence for their use, over 6% of respondents stated that TH may be indicated in TRD. Due to the risk of iatrogenic thyrotoxicosis and increased morbidity the use of TH should be addressed in relevant endocrinology guidelines and consensus should be reached between specialties.

Introduction: Thyroid eye disease (TED) is an autoimmune disease of the orbit. In moderate to severe active disease, orbital radiotherapy (ORT) is a treatment option. Diabetes is considered a relative contraindication because of potential induction of retinopathy. The purpose of this study was to decide if ORT remains a risk towards retinopathy in the era of linear accelerators with 3D computer aided design. Follow up examinations were performed to detect radiogenic retinopathy. Methods: Of the 151 patients who received ORT for TED between 2004?2020 at the University of Debrecen, 97 patients were available for follow up. In each case, 3D-based LINAC radiotherapy with a total dose of 20 Gy in 2 Gy daily fractions was administered. Eighteen patients had type 2 and one patient had type 1 diabetes. Median duration of diabetes was 16 (IQR:11.75-17.0) years; most recent HbA1c before irradiation was 6.55 (IQR:6.22-6.95) %. Ophthalmological follow up included slit-lamp and fundus examination, visual acuity, exophthalmometry and clinical activity score (CAS) calculation. Results: Median time between TED start and ORT was 2 (IQR:1-8) years. At the time of ORT, median TSH was 0.84 (IQR:0-1.41) mU/L, fT4 18 (IQR:12.15-25.40) pmol/L and fT3 6.5 (IQR:4.4-11.0) pmol/L, CAS was 5 (IQR:4-6). No diabetic retinopathy was present before ORT. The median follow-up period after completion of ORT was 60 (IQR:38-100) months. No patient developed retinopathy. Discussion: The risk of radiation-induced retinopathy after orbital irradiation, including well controlled diabetic patients, appears to be negligible. In the absence of pre-existing diabetic retinopathy and with good glycemic control ORT might be safe if linear accelerator with 3D planning is used. However, ophthalmic surveillance is still recommended.

Introduction: Over-diagnosis and over-treatment of hypothyroidism is a growing concern. The role of patient knowledge has not been previously investigated. The aim was to explore patient knowledge in relation to diagnosis and treatment of hypothyroidism. Methods: Cross-sectional, international online survey. Participants were people with treated hypothyroidism amounting to 3421 valid respondents from 68 countries. A questionnaire was used, which included knowledge statements about hypothyroidism relating to recommendations by international guidelines. The principal knowledge statement was "A patient with a normal thyroid blood test does not need to be treated with thyroid hormones (even if they have positive thyroid antibodies and symptoms)", and participants were asked to classify it as "false", "true", or "don't know". Responses were divided into corresponding groups: "Incorrect", "Correct", and "Unsure". Associations of groups with respondent characteristics and patient reported outcomes were investigated. Responses to a further seven knowledge statements explored ampliative knowledge about hypothyroidism. Results: With regards to the principal knowledge statement, "Correct", "Incorrect" and "Unsure" comprised 15.3%, 50.7% and 34.0% of responses to the respectively. "Incorrect" respondents were more likely than expected to live in the United Kingdom, have Hashimoto's thyroiditis, have a recent low self-reported serum thyrotropin, be treated with liothyronine-containing medication, and use social media and the internet for hypothyroidism-related information daily. "Incorrect" responses were associated with dissatisfaction, poor perceived control of symptoms and negative impact of hypothyroidism on everyday activities. The proportion of "Incorrect" responses for seven other knowledge statements ranged between 1.8-34.9%. Discussion: Incorrect responses to the principal knowledge statement were common in this sample of people with hypothyroidism, and associated with several demographic variables and adverse patient outcomes. Our findings suggest that knowledge gaps about the significance of symptoms in relation to the diagnosis and treatment of hypothyroidism may be important in driving over-diagnosis and over-treatment. The high number of "Unsure" respondents suggests that patient education may be an effective intervention.

Introduction: Dyslipidemia is a common complication of adult growth hormone de ciency (AGHD) and considered an important contributor to increased mortality. Previous studies mainly focused on quantitative assessment of lipoproteins, but lipoprotein subfractions and their relationship with insulin-like growth factor 1 (IGF-1) have not been explored. Purpose: To perform a comprehensive evaluation of lipoprotein subfractions and measuring apolipoprotein L1 (apoL1), sphingosine 1-phosphate (S1P) and apolipoprotein M (apoM) in AGHD. Materials and methods: 11 GH-substituted (GHS) patients, 9 GH-unsubstituted (GHU) patients and 37 controls were included in the study. Lipoprotein subfractions were separated by the Lipoprint system. ApoL1, apoM and S1P were determined by ELISA. In the GHS patients GH-replacement was discontinued for 2 months. Measurements were performed before GH- discontinuation, at the end of the 2-month GH-withdrawal, and 1 month after reinstituting GH-replacement. Results: Standard lipid parameters, apoM and apoL levels were not different between the groups. GHU patients demonstrated lower apolipoprotein A1 compared to controls (p=0.02) and higher apolipoprotein B100 compared to GHS (p=0.02). GHU and GHS showed higher S1P levels compared to controls (p=0.04 and p=0.01, respectively). Both GHU and GHS patients also presented higher percentage of intermediate-density lipoprotein (IDL) compared to controls (p=0.03 and p=0.01, respectively). Mean LDL size was lower in GHU compared to GHS (p=0.04). Percentage of intermediate HDL was lower in GHU and GHS compared to controls (p<0.001 and p<0.01, respectively). GHU demonstrated higher percentage of small HDL than controls (p<0.001). Overall, log10IGF-1 correlated positively with the percentage of large HDL (r=0.27; p=0.04) and intermediate HDL (r=0.38; p<0.01) and negatively with the percentage of small HDL (r=-0.46; p<0.01). Log10IGF-1 was the best predictor of small HDL (standardized b=-0.46; p<0.001) in overall subjects. In the GH-withdrawal study, the amount of HDL-6 increased with GH-withdrawal (p=0.03) and the percentage of IDL increased with reinstitution (p=0.05). Conclusion: Despite no changes in standard lipid parameters, considerable alterations of lipoprotein subfractions were revealed in GH-de cient adults indicating that lipoprotein subfraction analysis may allow for a more precise cardiovascular risk assessment in AGHD. Associations between HDL subfractions and Log10IGF-1 demonstrate a novel insight into the role of GH in lipid metabolism.

Introduction: Adult growth hormone deficiency (AGHD) is associated with a high prevalence of metabolic syndrome (MS), which contributes to the unfavorable cardiovascular risk profile in these patients. Insulin like growth factor-1 (IGF-1) is a widely used biomarker, however it does not always reflect the cardiometabolic risk and has a poor relationship with clinical efficacy endpoints. Consequently, there is an unmet need for biomarkers to monitor responses to GH-replacement. Afamin is a hormone-like glycoprotein, expressed in the liver. Higher afamin levels are strongly associated with MS and insulin resistance (IR). Although both MS and IR are very common in AGHD, afamin has not been investigated in these patients. Purpose: To investigate afamin as a potential biomarker in patients with AGHD. Materials and methods: Participants included 20 AGHD patients (11 GH-substituted and 9 GH-unsubstituted) and 37 healthy controls. Subjects underwent routine laboratory examinations, anthropometric measurements, body composition analysis using multi-frequency bioelectrical impedance analysis (InBody720) and measurement of serum afamin concentrations. In GH-substituted subjects, GH-substitution was withdrawn for 2 months. Measurements were carried out right before GH-withdrawal, at the end of the 2-month withdrawal period, and 1 month after reinstituting GH-replacement therapy (GHRT). Results: GH-unsubstituted patients demonstrated higher afamin levels compared to controls (p=0.03). Afamin positively correlated with skeletal muscle mass, bone mineral content, total body water, extracellular- and intracellular water content, insulin (all, p<0.01), HOMA-IR (p=0.01) and C-peptide (p=0.03) levels in AGHD but not in healthy controls. In GH-substituted patients 2-month of GH-withdrawal caused significant changes in body composition, including decreased fat-free mass, skeletal muscle mass, total body water, and intracellular water content (all, p<0.01); but these changes almost fully recovered 1 month after reinstituting GHRT. Unexpectedly, afamin levels decreased after GH-withdrawal (p=0.03) and increased with reinstitution (p<0.01). Changes of afamin levels during GH-withdrawal positively correlated with changes of HOMA-IR (r=0.80; p<0.01) and changes of insulin (r=0.71; p=0.02). Conclusion: Higher afamin levels in unsubstituted AGHD patients might indicate severe metabolic dysregulation. Significant changes accompanying GH-withdrawal and reinstitution, along with strong correlations with measures of IR, suggest that afamin could be a promising biomarker to monitor GHRT-associated changes of insulin sensitivity.
