Megjegyzések:	We have recently shown that fluoxetine (FX) suppressed polyinosinic-polycytidylic acid-induced inflammatory response and endothelin release in human epidermal keratinocytes, via the indirect inhibition of the phosphoinositide 3-kinase (PI3K)-pathway. Because PI3K-signaling is a positive regulator of the proliferation, in the current, highly focused followup study, we assessed the effects of FX (14 ?M) on the proliferation and differentiation of human epidermal keratinocytes. We found that FX exerted anti-proliferative actions in 2D cultures (HaCaT and primary human epidermal keratinocytes [NHEKs]; 48- and 72-h; CyQUANT-assay) as well as in 3D reconstructed epidermal equivalents (48-h; Ki-67 immunohistochemistry). Importantly, FX did not influence epidermal thickness (hematoxylin-eosin staining), and it did not have a major impact on the differentiation-associated alteration of the gene expression pattern (24-h treatments; RNA-Seq). Moreover, neither keratin (K)-1, nor K10 expression was altered by FX in NHEKs (RT-qPCR) or in 3D epidermal equivalents (semi-quantitative immunohistomorphometry). FX did not influence differentiation-induced up-regulation of occludin (RT-qPCR; NHEKs), and did not alter differentiation-associated barrier forming capacity of epidermal keratinocytes (electrical impedance; Lucifer Yellow penetration assay). Our data indicate that, besides the previously reported combined anti-inflammatory and putative anti-pruritic effects, FX may also suppress proliferation of human epidermal keratinocytes without impairing their differentiation and barrier-forming capacity.

Megjegyzések:	Introduction and importance: Ganglioneuromas are extremely rare, slow-growing, benign tumors that arising from Schwann cells, ganglion cells, and neuronal or fibrous tissue. Their malignant degeneration occurs very rarely, complete surgical removal is recommended to eliminate possible symptoms or to prevent possible malignant transformation. Reviewing the literature, there is currently insufficient data available on laparoscopic resection of retroperitoneal ganglioneuromas. Case presentation: 20-year-old young woman with no previous medical history or regular medication use complaints of abdominal pain. Abdominal CT scan found a cystic mass measuring up to 50 mm in diameter with a thick fluid density and no contrast accumulation, was identified in the porta hepatis region extrahepatically. Ultrasound-guided biopsy was performed, histopathological finding revealed mature benign neurogenic tumor tissue consisting of mature ganglion cells, mature Schwann cells, and branching stroma. Clinical discussion: A laparoscopic surgery was performed, the 5 cm large tumor was excised from the hepatoduodenal ligament. The tumor was removed from the region of the inferior caval vein, portal vein, and the common and proper hepatic arteries. Final histological diagnosis is ganglioneuroma of the hepatoduodenal ligament. After uneventful postoperative period, the patient was discharged home on the 6th day. Conclusions: Retroperitoneal tumors were previously excised during laparotomy. However, in recent decades, with the development of laparoscopic surgical techniques and tools, laparoscopic removal of some retroperitoneal tumors seems to be the ideal approach. The use of laparoscopy improves visibility of the relationship of the tumor to the surrounding, often vital, structures. Based on a review of the international literature and our own experience, laparoscopic ganglioneuroma resection is the recommended procedure with careful patient selection, as well as appropriate preoperative imaging and diagnostics, and with adequate expertise.

Megjegyzések:	Background Stoma creation is an essential part of general surgery, used in procedures such as protective loop ileostomy formed during low rectal resection or coloanal anastomosis, and in Hartmann's procedures, often performed in emergency settings. Despite its clinical importance, the optimal wound closure technique following stoma reversal remains controversial. This study aims to identify a more effective wound closure method following stoma takedown. Methods Between June 1, 2017, and January 31, 2020, we prospectively observed 75 patients at the University of Debrecen, Hungary, who underwent stoma closure (29 colostomies and 46 ileostomies). Among these patients, 37 underwent purse-string "Volcano" suture closure (VSC), while 38 underwent primary suture closure (PSC). We recorded demographic data, operative times, and postoperative outcomes, including wound infections. The primary endpoint was the rate of surgical site infection (SSI) in the short-term and the occurrence of postoperative hernia in the long-term period. Secondary endpoints included operative time, duration of wound dressing, and surgical complications according to the Clavien?Dindo classification. The planned follow-up period for long-term complications was 5 years. In this study, Surgical Site Infection (SSI) was defined and classified strictly according to the Centers for Disease Control and Prevention (CDC) Guideline for the Prevention of Surgical Site Infection (2017). Results No significant differences were observed between the VSC and PSC groups in age, sex, BMI, comorbidities, ASA classification, time from primary surgery to stoma closure, or length of hospital stay. However, the operative time was significantly shorter in the VSC group (59 min) compared to the PSC group (87 min; p?=?0.034). Notably, the incidence of postoperative wound infection was 0% in the VSC group and 18.4% in the PSC group respectively (p?=?0.012). Conclusion Our prospective observational study demonstrates that the "Volcano" (purse-string) wound closure technique provides significant advantages over conventional primary closure, notably lowering surgical site infection rates and operative time. Based on these results, we recommend the Volcano technique for stoma reversal following both ileostomy and colostomy.
