Megjegyzések:Bevezetés: A gyermekkori fogszuvasodás prevalenciájában jelentős különbségek figyelhetőek meg a kelet- és nyugat-európai államokban. A gyermekek fogászati szűrésére, szájhigiéniai oktatására sokféle ajánlás létezik az Európai Unióban. Munkánk során a gyermekeknek nyújtott államilag finanszírozott fogászati megelőző szolgáltatások jellemzőit vizsgáltuk a tagállamokban. Anyag és módszer: Online kérdőívünk kitöltésére a gyermekfogászatra szakosodott szakmai testületeket kértük fel. Keresztmetszeti vizsgálatunkhoz az adatok egységesítése céljából alkalmazott indikátorokat az EGOHID és SIGN-138 útmutatása szerint választottuk meg. A kiértékelést a STROBE irányelvei alapján végeztük, leíró statisztikai módszereket alkalmazva. Eredmények: Kérdőívünkre 27 EU-tagállamtól kaptunk választ. A fogászati prevenciós szolgáltatások elérhetősége és finanszírozása tagállamonként jelentős különbségeket mutat. A szűrések gyakorisága és a szűrt korcsoportok eltérőek országonként, nem általánosak az óvodáskorúak ingyenes szűrései sem. A folyamatos monitorozás csak a válaszadó országok 37%-ában működik. A 27 országból 25-ben főként a szájhigiéniai oktatásra irányuló preventív intézkedések érhetőek el. A hosszútávon költséghatékony megelőző fogászati kezelések alulfinanszírozottak vagy nem támogatottak. Következtetések: Átfogó tanulmányok szükségesek a nemzetközi felmérések egységesítését elősegítő további kulcsindikátorok meghatározásához. Az egységes adatnyilvántartás elengedhetetlen a népegészségügyi intézkedések tervezéséhez, megvalósításához, értékeléséhez és a finanszírozási rendszer optimalizálásához.

Megjegyzések:The associations among early childhood caries (ECC), socioeconomic status, and sugar consumption are of the utmost importance, due to their potential policy implications. The purpose of this study was to identify trends in ECC burden in children under 5 years old among European Union (EU) member states over time and to evaluate the relationship with its risk factors. Global Burden of Disease 2019 data were analyzed to estimate the burden of ECC over time, specifically incidence, prevalence, and years lived with disability (YLDs) for children under 5 years old. Four ecological variables with a potential effect on YLDs for ECC were used to investigate the association between 2014 and 2017. The YLDs rate was consistently higher among Eastern EU countries over time. Univariate models showed a positive significant association between at-risk-of-poverty rate and YLDs rate, while GDP per capita and urbanization were inversely associated with YLDs rate. In the multivariate analysis, sugar consumption, GDP per capita and urbanization showed significant association with YLDs rate. After stratification by region, association remained significant only in the Eastern EU countries between GDP, urbanization, and YLDs rate, while sugar consumption and at-risk-of-poverty rate had no significant impact on YLDs rates. This study found increasing ECC burden in the EU. The complexity of the problem indicates the need for innovative and personalized policy approaches to tackle the disease.

Megjegyzések:Background/Objectives: Type 2 diabetes mellitus (T2DM) represents a growing public health burden in the European Union (EU), largely driven by modifiable lifestyle factors such as diet. This systematic review aimed to synthesize observational evidence on the associations between nutritional exposures and incident T2DM across the EU-28, with a focus on regional heterogeneity and implications for EU-level nutrition policy. Meth- ods: The review followed PRISMA 2020 guidelines and was registered in PROSPERO (CRD42020219994). Searches were conducted in different databases (PubMed, Embase, Scopus, Web of Science) identifying 23,437 records, from which 104 observational studies were included. Eligible studies involved adults (?18 years) without T2DM at baseline and assessed dietary exposures in relation to T2DM incidence. Data extraction and methodolog- ical quality assessment were performed in duplicate using the NHLBI tool in Covidence. Results: Most included studies were cohort studies (77.9%), primarily conducted in West- ern, Northern, and Southern Europe. Diets characterized by high consumption of whole and minimally processed foods, such as fruits, vegetables, legumes, whole grains, and fermented dairy, consistently showed associations with lower T2DM risk. In contrast, high intake of red and processed meats, sugar-sweetened beverages, and ultra-processed foods was linked to higher risk. Adherence to Mediterranean or plant-based dietary patterns was associated with lower risk, whereas high animal-protein dietary patterns were detri- mental. Conclusions: Nutritional determinants play a decisive role in shaping T2DM risk in the EU. Evidence supports prioritizing dietary patterns rich in plant-based and minimally processed foods while reducing ultra-processed and meat-based products. Tai- lored region-specific policies are needed to address the T2DM epidemic and guide effective prevention strategies.

Megjegyzések:Multiple chronic conditions reduce the quality of life and increase healthcare needs for people with diabetes mellitus (DM). This study aims to describe the prevalence of comorbidities associated with DM in the European Union (EU) at national and sub-national levels and to assess the utility of a comorbidity burden index. The study was carried out using microdata from European Health Interview Survey 2019 including adults aged 25 and older with DM from 26 EU member states (n?=?20,042). The comorbidity index was calculated for 9 chronic conditions using the self-rated general health of individuals and disability weights obtained from the Global Burden of Disease 2019. Beta regression analysis was performed to evaluate the association between the comorbidity index and several determinants. A higher comorbidity index was found in sub-populations exhibiting lower education, unemployment or other labour status, lower income, rural residence, and poor health behaviours including obesity, physical inactivity, and poor diet. A higher comorbidity burden was observed in Eastern and Southern European countries and specific subregions within each country. The comorbidity index has the potential to identify regions and subpopulations with the highest disability burden and to help develop interventions to improve the quality of life of people with DM.

Megjegyzések:Population shift towards healthier lifestyles can help reduce the burden of type 2 diabetes mellitus (DM), therefore understanding and monitoring the lifestyle-related risk factors are crucial for setting up effective preventive strategies and disease management. The present study aimed to explore the changes in prevalence of DM and major risk factors including smoking, physical activity, fruit and vegetable consumption, as well as body mass index (BMI) over three waves of European Health Interview Survey, and to investigate the association between risk factors and presence of DM across 11 European Union member states. Poisson regression models were used to evaluate the association between risk factors and DM, adjusted for demographic and socioeconomic variables. The estimated age-standardized prevalence of DM increased from 7.01% in 2009 to 7.96% in 2019, with substantial increase in subgroups with higher BMI and unhealthy lifestyle including physically inactive people, or current smokers. Obesity and overweight and physical inactivity were significantly associated with DM in all survey waves. Our findings underline that obesity prevention and weight loss promotion along with physical activity promotion are the subject of lifestyle interventions to reduce the burden of DM in EU member states.

Megjegyzések:Background The use of dental care among older people is low compared to other forms of health care, with significant health consequences. However, the evidence on the extent to which countries' welfare systems and socio-economic factors influence the uptake of dental care by older people is limited. This study aimed to describe trends of dental care utilisation, and to compare use of dental care with other types of healthcare services among the elderly, considering different socio-economic factors and welfare systems in European countries. Methods Multilevel logistic regression analysis was performed using longitudinal data from four waves (between Wave 5 and 8) of the Survey of Health, Ageing and Retirement in Europe database, with a follow-up period of 7 years. The study sample included 20,803 respondents aged 50 years or older from 14 European countries. Results The annual dental care attendance was the highest in Scandinavian countries (85.7%), however, improving trends of dental attendance was recognized in Southern and Bismarckian countries (p<0.001). The difference in use of dental care services between socio-economic groups was expanding over time regarding low- and high-income level and residential area. A more marked difference was observed between social groups in dental care utilisation compared to other forms of care. Income level and unemployed status had significant effect on forgoing dental care due to cost and unavailability. Conclusion The observed differences between socioeconomic groups may highlight the health consequences of the different organization and financing of dental care. The elderly population could benefit from adopting policies aiming to reduce the financial barriers to dental care usage, especially in Southern and Eastern European countries.

Megjegyzések:Introduction: Surveillance of the European Union's (EU) legislations on the prevention of diabetes mellitus (DM) is needed, to more effectively tackle the rising prevalence of DM. Methods: This legal surveillance was carried out through a systematic search and screening, using EUR-Lex database to identify treaties, acts, and other legal documents for prevention of DM, non-communicable diseases (NCDs) and obesity, followed by their content analysis and assessment according to DM specific guidelines, target population and functional categories. Results and discussion: We found 22 legislations aimed at preventing DM, NCDs and obesity, but only 5 out of them specifically addressed preventing DM. The aims of legislations covered a broad spectrum of themes indicated by DM specific guidelines, mostly initiatives of life-course approach in preventing DM, NCDs and obesity from the area of energy intake. The target group of most legislations was the general population; high-risk subgroups such as pregnant women were hardly ever the primary target group. Our results prove that the EU has made cross-sectoral legislative efforts to reduce the disease burden and prevent DM but does not exhaust all possibilities. However, given its persistently rising DM prevalence, it is imperative to make sure that DM is a top health priority for various EU authorities and is incorporated into new initiatives, policies and laws.
