Megjegyzések:	Background: The aim of this study was to quantify epithelial, stromal and endothelial celldensity and subbasal nerve morphology in young patients with type 1 diabetes mellitus(T1DM) with and without diabetic retinopathy.Methods: 28 young patients (mean age: 22.86?9.05 years) with T1DM, with (n=18) andwithout (n=10) retinopathy and 17 age-matched healthy control subjects (mean age:26.53?2.43 years) underwent corneal confocal microscopy (CCM).Results: Epithelial (P<0.0001), stromal (P=0.024) and endothelial (P=0.001) cell densitieswere significantly reduced in patients with T1DM compared to controls. There was asignificant reduction in corneal nerve fibre density (P=0.004), nerve branch density(P=0.004), total nerve branch density (P=0.04) and nerve fibre length (P=0.001) with anincrease in nerve fibre width (P=0.04) in patients with T1DM compared to healthy controls.There was a significant reduction in epithelial (P<0.001), keratocyte (P=0.02) and endothelial(P=0.02) cell densities and a reduction in nerve branch density (P=0.02) and nerve fibrelength (P=0.04) in T1DM patients without retinopathy compared to control subjects.Conclusions: Corneal confocal microscopy identifies corneal cellular and small nerve fibrepathology in young patients with T1DM without retinopathy which increases in severity inthose with retinopathy. CCM appears to have considerable utility as an imaging biomarker forearly subclinical pathology in young patients with T1DM.
