Megjegyzések:A non-invasive nuclear medicine method was developed for screening patients (PTS) with painful hands, in order to separate PTS with normal from those with abnormal microcirculation (MIC) of the hands in different clinical conditions. Such a method would be of great importance as the other methods available are either importance as the other methods available are either subjective or rather complicated. The study comprised ten healthy individuals, twenty-three PTS with Raynaud syndrome (RS), and fifteen PTS with mixed connective tissue disease (MCTD) - 6 active and 9 inactive. After the bolus injection of 99mTc-DTPA through a dorsal foot vein, dynamic study was performed during 1 min. (1 sec/game images). Regions of interest (ROIs) were drawn on the summed images around the fingers, and the palmar region. The fingers-to-palm ratio (FPR) is then calculated from the total counts inside these ROIs separately for each hand. Mean FPR was 0.94+-0.18 (0.71-1.25) for the healthy group, 0.57+-0.22 (0.21-1.11) for the MCTD group, and 0.40+-0.14 (0.18-0.77) for the Raynaud PTS. Variance analysis showed these differences to be highly significant (P<0.001). There were also significant differences between 6 MTCD PTS in active (mean 0.48) and 9 in inactive (mean 0.66) clinical state (2 sample t-test: P<0.05). However, there were no significant differences between the FPR values of left and right hands of the same patient (1 sample t-test). From among the 23 primary Raynaud PTS, capillary microscopic data were pathological in only 8 cases (34 per cent). We conclude that our method is capable for differentiating PTS with normal from those with abnormal MIC of the hands. Though the FPR measurement is not a specific method, it is useful both for staging and in the follow-up of PTS.
