Megjegyzések:	Secondary osteoporosis is present in most patients with rheumatoid arthritis. A number of factors including inflammatory mediators, the sex, age and hormonal status of the patients, immobility, decreased levels of estrogens, androgens and vitamin D3, as well as glucocorticoid therapy play an important role in the pathogenesis of arthritis-related osteopenia. Among rheumatoid patients continuously followed at the Third Department of Medicine, more than 75 per cent have osteopenia. Most osteoporotic patients are postmenopausal women. The active vitamin D metabolite alfacalcidol is a well-tolerated antiosteoporotic agent. Its side-effects are usually mild and reversible. Authors found that alfacalcidol prevented vertebral and femoral bone loss during one-year treatment. Active vitamin D may be useful in the treatment of senile osteoporosis or in that of patients taking glucocorticoids. Calcitonin treatment also resulted in a significant bone-preserving effect. In this study the authors obtained additional data on the pathogenesis, diagnostics and treatment of osteopenia associated with rheumatoid arthritis.
