Megjegyzések:	Principles and changes int he surgical treatment of acute pancreatitisAlthought the treatment of acute pancreatitis is conservative, the severe form of the disease frequently requires surgery.Surgical treatment for septic pancreas necrosis shoulg be prolonged to the 4-6th weeks from the onset of the disease. In this period of the patient's general condition is stablizied and the walled off necrosis developed. At that time surgical interentions have big rola besides the minimal invasive approaches. Open necrosectomy with postoperative lavage or transgastric open necrosectomy is recomended. Urgent decopmressive laparotomy is indicated in cases of abdominal compartment syndrome if the conservative treatment has failed. The most effective method of decompressive laparotomy and open abdomen treatment is the negative pressure wound therapy. In case of severe bleeding , gastrointestinal perforation or peritonitis immediate operation is needed. The elective or attached cholecystectomy is absolute indication in biliary pancreatitis.
