Megjegyzések:AbstractSimultaneous pancreas-kidney transplant is currentlystandard therapy to achieve long-term insulin-freeeuglycemia in patients with type 1 diabetes mellitusand concomitant end-stage kidney failure. A patientwith symptoms of encephalopathy caused byhyperammonemia and with new-onset iron deficiencyanemia was admitted to our institution 20 months aftera simultaneous pancreas-kidney transplant. Detailedscreening did not reveal any specific cause for thehyperammonemia, and despite standard treatment,hyperammonemia did not resolve. An abdominalcomputed tomographic scan was performed, whichshowed a distended duodenal segment of thepancreas graft. This was confirmed during exploratorylaparotomy when the anastomosis betweenduodenum and ileum was dismantled and found notto be stenotic. The excessively long stumps of theduodenum were then dissected and shortened, and anew anastomosis between graft-duodenum andrecipient-ileum was created. The operation wasfollowed by an uncomplicated postoperative course inwhich the serum ammonia normalized on the firstpostoperative day and remained normal afterwards.An excessively long segment of the duodenum of thepancreatic graft may lead to encephalopathy withhyperammonemia after a simultaneous pancreaskidneytransplant. This emphasizes the need formeticulous preparation of the graft to avoid thiscomplication.
