Megjegyzések:Objective: Saphenous venous grafts (SVGs) are established choices for coronary artery bypass grafting (CABG); however, their lumen patencyis limited. Our goal was to investigate the risk factors of SVG degeneration.Methods: Seventy-five patients (mean age, 57.5?10.4 years) with 133 SVG conduits who had cardiac catheterization ?1 year after CABG wereselected; follow-up period was 67.6?36.8 months. Patients were divided into 3 groups according to angiographic status at follow up [intact: <20%(n=23); narrowed: 20?99% (n=24); and occluded (n=28)]. Baseline clinical conditions were evaluated in relation to follow-up angiography. Asonset date of chronic total occlusions is usually uncertain, they arise typically from thrombotic lesions; thus, their value in evaluation is limited.Results: There were no significant differences between the 3 groups in clinical parameters. Linear correlation analysis found significant (p<0.01)positive connection of SVG disease (luminal diameter reduction 20?99%) with C-reactive protein (CRP) and homocysteine (Hcy), as well asbetween CRP and Hcy. Multiple regression analysis showed plasma Hcy level to be significantly related to graft diameter reduction normalizedto time elapsed until angiography in narrowed grafts: 1 ?mol/L increase of Hcy was associated with 0.053%/month decrease in lumen diameter(p<0.01; R2=0.428); extrapolating: +10 ?mol/L higher Hcy level during 5 years is associated with 32.1% lumen reduction.Conclusion: Medium- to long-term SVG degeneration is related to elevated plasma total Hcy in patients with sub-occlusive graft stenosis, whilein cases with intact SVGs, the beneficial local flow conditions may protect the grafts from degeneration. Megjegyzések:Background: Measurements of fractional flow reserve (FFR) and/or coronary flow reserve (CFR) are widely used for hemodynamic characterization of coronary lesions. The frequent combination of the epicardial and microvascular disease may indicate a need for complex hemodynamic evaluation of coronary lesions. This study aims at validating the calculation of CFR based on a simple hemodynamic model to detailed computational fluid dynamics (CFD) analysis. Methods: Three-dimensional (3D) morphological data and pressure values from FFR measurements were used to calculate the target vessel. Nine patients with one intermediate stenosis each, measured by pressure wire, were included in this study. Results: A correlation was found between the determined CFR from simple equations and from a steady flow simulation (r = 0.984, p < 10-5). There was a significant correlation between the CFR values calculated by transient and steady flow simulations (r = 0.94, p < 10(-3)). Conclusions: Feasibility was demonstrated of a simple hemodynamic calculation of CFR based on 3D-angiography and intracoronary pressure measurements. A simultaneous determination of both the FFR and CFR values provides the capability to diagnose microvascular dysfunction: the CFR/FFR ratio characterizes the microvascular reserve.

Megjegyzések:Számos experimentális, klinikai és epidemiológiai vizsgálat igazolta, hogy a vér emelkedett homocystein szintje a thromboemboliás kardiovaszkuláris megbetegedések egyik független rizikófaktorának tekinthető. 1010 kelet-magyarországi, kardiovaszkuláris betegségre utaló panasszal (stabil angina) rendelkező beteg adatainak retrospektív feldolgozásával kerestük a homocystein-szint, a kardiovaszkuláris betegség, bizonyos laborparaméterek, és a genetikai eltérések összefüggéseit. Saját vizsgálataink szerint a kardiovaszkuláris betegek homocystein-szintje emelkedett, melynek prognosztikus értéke lehet, illetve az atherosclerosisban ismert szerepe alapján a homocystein-szint csökkentése kedvező hatással bírhat ezen betegekben.

Megjegyzések:Mechanical and biochemical alterations were investigated in permeabilized cardiomyocytes along with the progression of dilated cardiomyopathy (DCM) in a transgenic mouse line overexpressing the activated Galphaq protein (Tgalphaq*44). The isometric force, its Ca(2+) sensitivity (pCa(50)) and the turnover rate of the actin-myosin cycle (k(tr)) were determined at sarcomere lengths (SLs) of 1.9 mum and 2.3 mum before (at 4 and 10 months of age) and after hemodynamic decompensation (at 14 and 18 months of age) in Tgalphaq*44 cardiomyocytes and in age-matched control cardiomyocytes. The SL-dependence of pCa(50) was not different in Tgalphaq*44 and control hearts. In contrast, a significant increase in pCa(50) was observed in the Tgalphaq*44 cardiomyocytes (DeltapCa(50): 0.10-0.15 vs. the controls) after 10 months of age that could be diminished by exposures to the catalytic subunit of protein kinase A (PKA). Accordingly, a decline in endogenous PKA activity and decreased troponin I phosphorylation were detected after 10 months in the Tgalphaq*44 hearts. Finally, the maximal Ca(2+)-activated force (F(o)) and k(tr) were lower and the passive force (F(passive)) was higher at 18 months in the Tgalphaq*44 cardiomyocytes compared to the control. These mechanical alterations were paralleled by a robust increase in beta-myosin heavy chain expression in the Tgalphaq*44 hearts. In conclusion, our data suggested that an initial decrease of PKA signaling and subsequent changes in myofilament protein expression may contribute to the development of dilated cardiomyopathy in Tgalphaq*44 hearts.

Megjegyzések:Background: Clinically silent cerebral ischemia (SCI) detected by diffusion-weighted MRI hasbeen reported in 5-40% of patients undergoing pulmonary vein isolation (PVI). Although initial reportssuggested a high rate of SCI with phased radiofrequency (RF) ablation on use of the pulmonary veinablation catheter (PVAC), the incidence was subsequently markedly reduced in consequence ofprocedural modifications in recent studies.We analyzed cerebral microembolization as assessed with transcranial Doppler during phasedRF ablation and with two other single-shot AF ablation technologies: the cryoballoon (CB) and thenMARQTM multipolar irrigated RF ablation system.Methods and results: A total of 89 patients (mean age: 57, SD: 12 years; 62 male) withparoxysmal or persistent AF, underwent PVI. Phased RF was used according to the initial protocol in 7patients (PVAC Group I), with procedural modifications and a newer (14.4) version of the RF generatorin 37 patients (PVAC Group II) and with the most recent (version 15.0) generator in 18 patients (PVACGroup III). Ablation was performed with the CB in 13 and with the nMARQ system in 14 patients.
