Megjegyzések:Background: Intravenous thrombolysis (IVT) improves acute ischemic stroke (AIS) outcomes, but with limited success. In addition, ethanol potentiates the effect of r-tPA in ischemia models. Methods: The effect of acute alcohol consumption on IVT outcomes was investigated in a retrospective cohort study. AIS patients with detectable blood alcohol concentration (BAC) during IVT were included (alcohol group; n = 60). For each case, 3 control subjects who underwent IVT but denied alcohol consumption were matched in terms of age, sex, affected brain area, and stroke severity. Outcomes were determined using the NIHSS at 7 days and the modified Rankin scale (mRS) at 90 days. Results: Patients were younger and had a less severe stroke than in a standard stroke study. Favorable long-term outcomes (mRS 0?2) occurred significantly more frequently in the alcohol group compared to controls (90% vs. 63%, p < 0.001). However, the rates of hemorrhagic transformation were similar. Multiple logistic regression models identified elevated BAC as a significant protective factor against unfavorable short-term (OR: 0.091, 95% CI: 0.036?0.227, p < 0.001) and long-term outcomes (OR: 0.182, 95% CI: 0.062?0.535, p = 0.002). In patients with BAC > 0.2%, significantly lower NIHSS was observed at 3 and 7 days after IVT vs. in those with 0.01?0.2% ethanol levels. Conclusion: Elevated BAC is associated with improved outcomes in IVT-treated AIS without affecting safety.

Megjegyzések:Background. Cerebral thromboembolism is a rare but feared complication of transcatheter ablation in patients with atrial fibrillation (AF). Here, we aimed to test which pre-procedural anticoagulation strategy results in less intracardiac activation of hemostasis during ablation. Patients and methods. In this observational study, 54 paroxysmal / persistent AF patients undergoing cryoballoon ablation were grouped according to their periprocedural anticoagulation strategy: no anticoagulation (oral anticoagulation (OAC) free; n = 24), uninterrupted vitamin K antagonists (VKA) ( n = 11), uninterrupted dabigatran ( n = 17). Blood was drawn from the left atrium before and immediately after the ablation procedure. Cryoablations were performed according to standard protocols, during which heparin was administered. Heparin-insensitive markers of hemostasis and endothelial damage were tested from intracardiac samples: D-dimer, quantitative fibrin monomer (FM), plasmin-antiplasmin complex (PAP), von Willebrand factor (VWF) antigen, chromogenic factor VIII (FVIII) activity. Results. D-dimer increased significantly in all groups post-ablation, with lowest levels in the dabigatran group (median [interquartile range]: 0.27 [0.36] vs. 1.09 [1.30] and 0.74 [0.26] mg / L in OAC free and uninterrupted VKA groups, respectively, p < 0.001 ). PAP levels were parallel to this observation. Post-ablation FM levels were elevated in OAC free ( 26.34 [30.04] mg / L ) and VKA groups (10.12 [16.01] mg / L), but remained below cut-o in all patients on dabigatran ( 3.98 [2.0] mg / L; p < 0.001). VWF antigen and FVIII activity increased similarly post-ablation in all groups, suggesting comparable procedure-related endothelial damage. Conclusion. Dabigatran provides greater inhibition against intracardiac activation of hemostasis as compared to VKAs during cryoballoon catheter ablation.

Megjegyzések:The importance of optimal blood pressure control for preventing or reducing the impairment of vascular and cognitive functions is well known. However, the reversibility of early alterations in vascular and cognitive functions through antihypertensive agents is under-investigated. In this study, we evaluated the influence of 3 months of angiotensin-converting enzyme (ACE) inhibition treatment on the morphological and functional arterial wall and cognitive performance changes in 30 newly diagnosed primary hypertensive patients. Common carotid intima-media thickness (IMT) and brachial artery flow-mediated dilatation (FMD) were detected by ultrasonography. Arterial stiffness indicated by augmentation index (AIx) and pulse wave velocity (PWV) was assessed by arteriography. Cognitive functions were assessed by neuropsychological examination. The executive function overall score was significantly higher at 3-month follow-up than at baseline (median, 0.233 (IQR, 0.447) vs ?0.038 (0.936); P?=?.001). Three-month ACE inhibition did not produce significant improvement in IMT, FMD, AIx and PWV values. Significant negative associations were revealed between IMT and complex attention (r?=??0.598, P?=?.0008), executive function (r?=??0.617, P?=?.0005), and immediate memory (r?=??0.420, P?=?.026) overall scores at follow-up. AIx had significant negative correlations with complex attention (r?=??0.568, P?=?.001), executive function (r?=??0.374, P?=?.046), and immediate memory (r?=??0.507, P?=?.005). PWV correlated significantly and negatively with complex attention (r?=??0.490, P?=?.007). Timely and effective antihypertensive therapy with ACE inhibitors has significant beneficial effects on cognitive performance in as few as 3 months. Early ACE inhibition may have an important role in the reversal of initial impairments of cognitive function associated with hypertension-induced vascular alterations.

Megjegyzések:Registered images produced by MRI based multi-modality image processing are essential tools of diagnostic and surgical planning of neurological and neurosurgical diseases. The aim of this study was to investigate how fused images produced by various MRI (T1, T2, fMRI, DTI, MRS) and PET (FDG, Metionine) data can be used in clinical routine. Material and methods: In our study the complex multi-modality tomographic data sets of 6 patients (3 glioblastoma, 2 oligoastrocitóma és 1 pilocitás astrocitoma) were used. For the image registration as well as the production of 2D- and 3D fused images free, commercial and in-house developed software were utilized. Result: We found that our images significantly assisted clinicians in preparing therapeutic and surgical plans. Optimizing the established software and automatizing image registration tasks needs further development.

Megjegyzések:Dynamic imaging of the inflow of technetium-99m hexamethylpropylene amine oxime (HMPAO) to the brain has been proved to allow estimation of the hemispherical cerebral blood flow (CBF) using the Patlak plot. In this study, we compared the hemispherical CBF (in ml/min/100 g) of different patient groups. A total of 25 patients (comprising 13 with migraine and 12 scheduled for endarterectomy owing to angiographically confirmed severe stenosis of the internal carotid artery on at least one side) underwent baseline and acetazolamide 99mTc-HMPAO brain perfusion studies. In addition, acetazolamide 99mTc-HMPAO studies were performed in 12 healthy subjects (no baseline study was performed for ethical reasons.) Dynamic studies were acquired by means of a dual-detector gamma camera with a large field of view (HELIX, Elscint). Special difference images were created to make definition of the aortic arch and hemispherical brain regions easier and more reproducible. A semi-automatic method was developed to determine the transit time from the aorta to the brain, making the generation of the Patlak plot even more robust. The baseline CBF values did not significantly depend on the disease (P>0.1), whereas the CBF values obtained after acetazolamide provocation did do so (ANOVA, P<0.001). Patients suffering from migraine showed a significant increase in global CBF values after acetazolamide provocation (paired t test, P<0.05), but we could not find any effect of the provocation in patients awaiting carotid endarterectomy, indicating a lack of cerebrovascular reserve capacity. Comparison of the results of the acetazolamide study in patients and the control group revealed the CBF values to be significantly lower in patients with carotid stenosis (two-sample t-test, P<0.001), but not in those with migraine (P>0.1). In summary, using quantitative analysis of 99mTc-HMPAO brain studies we could objectively compare the CBF of patients suffering from different diseases. Especially the CBF values obtained after acetazolamide provocation permitted effective differentiation of disease states. The quantitative results may be of assistance in therapy planning, e.g. in selection of the correct operative technique.

Megjegyzések:Quantitative imaging contrast is evaluated which allows the selective measurement of the blood oxygenation state during cerebral ischemia within a multiparametric imaging study on rats. In a first step, the ambiguities arising in T*(2)-weighted images due to T*(2) heterogeneity are eliminated by calculating T*(2) maps. Then, 1/T'(2) maps are calculated according to 1/T'(2) = 1/T*(2) - 1/T(2) to eliminate nonsusceptibility-induced changes of 1/T*(2) after the induction of stroke. This is of particular importance in the presence of vasogenic edema. The changes Delta(1/T'(2)) after the onset of ischemia selectively quantify the variations of the deoxyhemoglobin content during the development of the infarct. The presented results are not available from conventionally recorded parameters of a stroke study and, together with perfusion-weighted images, form a powerful combination to analyze the oxygen consumption and the metabolism of the tissue.

Megjegyzések:Neuroinflammation is an emerging focus of translational stroke research. Preclinical studies have demonstrated a critical role for brain-invading lymphocytes in post-stroke pathophysiology. Reducing cerebral lymphocyte invasion by anti-CD49d antibodies consistently improves outcome in the acute phase after experimental stroke models. However, clinical trials testing this approach failed to show efficacy in stroke patients for the chronic outcome 3 mo after stroke. Here, we identify a potential mechanistic reason for this phenomenon by detecting chronic T cell accumulation?evading the systemic therapy?in the post-ischemic brain. We observed a persistent accumulation of T cells in mice and human autopsy samples for more than 1 mo after stroke. Cerebral T cell accumulation in the post-ischemic brain was driven by increased local T cell proliferation rather than by T cell invasion. This observation urges re-evaluation of current immunotherapeutic approaches, which target circulating lymphocytes for promoting recovery after stroke.

Megjegyzések:Objectives Transcranial color-coded duplex sonography (TCCS) enables to measure blood flow characteristics in cerebral vessels, including vascular resistance and pulsatility. The study aims to identify factors influencing pulsatility (PI) and resistance (RI) indices measured using TCCS in patients with carotid atherosclerosis. Methods Self-sufficient patients with atherosclerotic plaque causing 20-70% carotid stenosis were consecutively enrolled to the study. All patients underwent duplex sonography of cervical arteries and TCCS with measurement of PI and RI in the middle cerebral artery, neurological, and physical examinations. Following data were recorded: age, gender, height, weight, body mass index, systolic and diastolic blood pressure, occurrence of current and previous diseases, surgery, medication, smoking, and daily dose of alcohol. Univariant and multivariant logistic regression analysis were used for identification of the factors influencing RI and PI. Results Totally 1863 subjects were enrolled to the study: 139 healthy controls (54 males, age 55.52 +/- 7.05 years) in derivation cohort and 1724 patients (777 males, age 68.73 +/- 9.39 years) in validation cohort. The cut off value for RI was 0.63 and for PI 1.21. Independent factors for increased RI/PI were age (odds ratio [OR] = 1.108/1.105 per 1 year), occurrence of diabetes mellitus (OR = 1.767/2.170), arterial hypertension (OR = 1.700 for RI only), width of the carotid plaque (OR = 1.260 per 10% stenosis for RI only), and male gender (OR = 1.530 for PI only; P <.01 in all cases). Conclusions The independent predictors of increased cerebral arterial resistance and/or pulsatility in patients with carotid atherosclerosis were age, arterial hypertension, diabetes mellitus, carotid plaque width, and male gender.

Megjegyzések:Extracranial ultrasonography is recommended to use as a baseline non-invasive method in the initial evaluation of either asymptomatic or symptomatic patients to define the possible stenosis on carotid artery. The latest 2011 guidelines specify the sequence of examinations with certain classification of recommendations and level of evidence. Carotid duplex ultrasonography plays an important role both in primary and secondary prevention of stroke and the results found determine the use of further investigations and management of patients with extracranial carotid and vertebral artery disease. In case of diagnostic uncertainty other brain imaging methods, like computed tomography angiography, magnetic resonance angiography and catheter-based angiography can be chosen to assess vascular lesions. Carotid duplex ultrasound serves not only diagnostic purposes but can also be useful in the follow up processes. It is widely used for control examinations after revascularization procedures of the carotid or vertebrobasilar arteries. By the establishment of indications of revascularization procedures degree of carotid stenosis is a major factor which therefore requires accuracy of the assessment. Carotid duplex ultrasound has some difficulties in this question. This diagnostic uncertainty is tried to be solved by improving the criteria system of stenosis grading in internal carotid artery. The aim of this article is to give an overview about the importance and role of extracranial duplex ultrasonography in stroke prevention based on the latest guidelines.

Megjegyzések:Aim The aim of our investigations was to detect the acute and chronic effects of cigarette smoking on arterial wall thickness and stiffness in young, healthy volunteers. We also performed a one-year follow-up to define the possible changes. Subjects We recruited 25 non-smoking and 25 smoking university students aged 19-33. Exclusion criteria were any known diseases, abnormally high cholesterol levels and body mass index (BMI) above 30 kg/m2. Methods We defined the intima-media thickness on both common carotid arteries by using B-mode ultrasonography and we measured the hemodynamic and stiffness parameters with the help of arteriograph. In case of smokers we also investigated the acute effects after smoking one cigarette. In the follow-up study we measured 15 non-smokers and 13 smokers again. Results In the smoking group morphological, stiffness and hemodynamic parameters showed significantly higher values compared to non-smokers. Concerning the acute effects we detected a significant increase in stiffness and hemodynamic parameters after smoking one cigarette. Gender differences were also found in the smoking group. Unadjusted to age, gender and smoking status there was a significant correlation between intima-media thickness and pulse wave velocity. We could not find any progression in smokers after one year, while there was an improvement in intima-media thickness and augmentation index in non-smokers. Conclusion Early atherosclerotic processes due to smoking can be detected even at a young age, in healthy university students. One year regular smoking does not result in detectable changes.
