Megjegyzések:Purpose: To contribute to the basic understanding of Congenital Hereditary Endothelial Dystrophy (CHED) by clinical, functional, and histopathological examinations of three cases. Methods: Prior to grafting, corneas were evaluated by slit lamp examination and by assessment of endothelial permeability to fluorescein. Following penetrating keratoplasty, corneal buttons were evaluated by light- and electron microscopy and by assessment of stromal swelling pressure. Results: Patients with CHED had a markedly increased corneal thickness (0.93-0.98 mm) with epithelial oedema and a stromal swelling pressure close to zero; suggesting that the stroma was maximally swollen in vivo. Corneal endothelium showed an increased permeability to fluorescein; suggesting a functional barrier defect. Histopathological evaluation revealed: 1) a normal endothelial cell density; 2) an abnormal endothelial morphology with irregular and multinucleated cells containing abnormal cell organelles; and 3) a profound thickening of Descemet's membrane, 16-18 mm, with multiple focal areas of abnormal fibrillar deposits in the posterior half. Conclusions: This study suggests that the primary defect in patients with CHED is a degenerated and dysfunctional corneal endothelium, characterized by an increased permeability and an abnormal and accelerated Descemet's membrane secretion. The underlying pathophysiological mechanism(s) may be related to an abnormal endothelial barrier function, leading to secondary swelling of the stroma and epithelium. Further studies are needed to identify the specific functional defect(s) and the embryological origin of the abnormal corneal endothelium in CHED.

Megjegyzések:To test the hypothesis that pH value of human tears is affected by flow rate. METHODS: After stimulation with a drop of 70% alcohol into the ipsilateral nasal cavity tears from the left eyes of 30 healthy volunteers were collected with glass capillaries while flow rate of tear secretion was registered. The pCO(2), actual and standard pH were determined in the collected samples with the Astrup method: Standard pH is defined as the pH of the sample that has been equilibrated at 37 degrees C with a gas mixture having pCO(2)=40 mmHg. RESULTS: The flow rate correlated positively and significantly with both the standard pH value (r=0.503, p<0.01) and the pCO(2) (r=0.517, p<0.01) while there was no correlation between the actual pH and the flow rate. CONCLUSION: Loss of CO(2) from the sample is a cause of serious uncertainty in tear pH determination. A positive flow rate-pH relationship was found for human tears when the uncertainty was removed by determination of the standard pH.

Megjegyzések:Protein C is a major component of the natural anticoagulant pathway. Resistance of coagulation factor V (FV) to activated protein C (APC), mostly due to FV Leiden mutation, is the most common cause of inherited thrombophilia. The aim of this retrospective study was to determine the prevalence of APC resistance and Leiden mutation in patients with non-arteritic anterior ischaemic optic neuropathy (NAION). METHODS: A total of 25 patients with NAION were examined between 1997 and 2002. The patients were screened for APC resistance and FV Leiden mutation as well as for acquired risk factors of vascular disease such as diabetes mellitus, hypercholesterolaemia, hypertonia and ischaemic heart disease. A control group of subjects without ocular vascular disease and with homogenous distribution of the same risk factors was used for comparison. RESULTS: Six of the 25 patients (24%) with NAION had APC resistance due to the heterozygous Leiden mutation of FV. The frequency of the same genetic mutation in the control group was only 5.9%. Odds ratio calculations showed that patients with the Leiden mutation were at a significantly higher risk of NAION than control patients (p < or = 0.021). CONCLUSION: The high frequency of Leiden mutation in NAION suggests a pathogenic role of the mutation in the disease.

Megjegyzések:Purpose: The clinical use and comparison of two recently developed A-scan ultrasound devices. Methods: The same investigator determined central corneal thickness (CCT), anterior chamber depth (ACD) and axial length (AL) with an OcuScan RxP (Alcon, Forth Worth, Texas, USA) and an AL-2000 (Tomey, Erlangen, Germany) ultrasound device in 80 eyes of 40 patients. The mean patient age was 63.72 years [standard deviation (SD) 18.92]. The patients did not suffer from any systemic or eye disease affecting the anterior and posterior segments of the eye, and their refractive error (spherical and astigmatic) did not exceed ± 3.0 dioptres. Results: The value of the CCT was 541.55 (SD 34.97) lm with the OcuScan RxP, and 547.46 (SD 35.70) lm with the AL-2000 device (P < 0.001). With respect to the ACD and AL, significantly lower values were obtained with the AL-2000 instrument (P < 0.001). The ACD was 2.92 (SD 0.48) mm with the AL-2000 and 3.07 (SD 0.47) mm with the OcuScan RxP device. The AL was 22.67 (SD 0.84) mm with the AL-2000 and 22.81 (SD 0.87) mm with the OcuScan RxP instrument. However, a positive and significant correlation could be demonstrated between the devices (r 1/4 0.88 CCT, r 1/4 0.86 ACD and r 1/4 0.91 AL; P < 0.001). Conclusion: The instruments are reliable in clinical use; however, statistically significant differences were found between the devices. During patient followup, the devices cannot simply be used interchangeably.

Megjegyzések:We compared the accuracy and the reproducibility of the measurements using a Haag-Streit, a Shin-Nippon, and a Carl Zeiss keratometer and an EyeSys videokeratoscope. Two investigators performed 20 measurements on 3 calibrated steel balls and 5 measurements on 22 normal corneas with all instruments. Calculating the accuracy of the measurements the limits of the 95% confidence intervals of the bias were determined. The accuracy was better than +/-0.1D for the keratometers, whereas it was better than +/-0.25D for the EyeSys videokeratoscope. The coefficient of reproducibility (1.96 x standard deviation) on test balls and corneas was smaller than 0.25D in the case of the Shin-Nippon and Carl Zeiss keratometers and the EyeSys videokeratoscope whereas it was between 0.25-0.5D in the case of the Haag-Streit keratometer. The Carl Zeiss keratometer was the best with respect to the accuracy and reproducibility of the measurements. Paired t-test was used to find possible significant differences between the results of the two investigators, but only clinically insignificant differences were found.
