Megjegyzések:Background: IL-6 is a pro-inflammatory cytokine which has many well-defined effects. Its synthesis and release from mononuclear cells of drug-sensitized patients was related before to in vitro drug-allergy diagnostics but has not yet been studied in detail. Methods: The specific release of preformed IL-6 from peripheral blood mononuclear cells (PBMC) after 20 minutes incubation with 0.15?0.5 ?M of pure drugs was measured in two groups of drug-allergy suspected donors (159) and respective controls (48). IL-6, TNF-alpha, IL-2, IL-4, IFN-gamma have been measured from cell supernatants by ELISA or by cytometric bead assay. Epicutaneous, intradermal and systemic provocation tests were performed to prove or disprove culprit substances (203 in vivo against 482 in vitro tests). T-test (paired and unpaired); chi2 contingency table; Z statistics and McNemar's test were used to evaluate results. Results: Concanavalin A as positive control released IL-6 from PBMC in linear concentration and exponential time dependent fashion (up to 60 minutes) pointing to the existence of a preformed pool of this cytokine. Preformed IL-6 released at any of 4 standard drug dilutions tested, above 50% over their diluents' levels significantly correlated with the patients' history on drug-induced hypersensitivity symptoms and with in vivo tests. Sensitivity of 85.4% and specificity of 82.4% of the IL-6 release assay were found. The 20· drop in release of TNF-alpha had no diagnostic importance; it has accompanied increased IL-6 release. IL-2, IL-4 and IFN-gamma were undetectable in 20 minutes supernatants. IL-6 release depended on the clinical phenotype but not on the eliciting drug(s) in the molecular mass range of 76?4000 Da. Reactivity of mononuclear cells at the lowest or at multiple drug test concentrations reflected clinical severity per diagnoses and according to area of skin involvement. Conclusion: This rapid test is applicable to detect a wide scale of drug hypersensitivity

Megjegyzések:Autoimmune diseases are a family of chronic systemic inflammatory disorders, characterized by the dysregulation of the immune system which finally results in the break of tolerance to self-antigen. Several studies suggest that Toll-like receptors (TLRs) play an essential role in the pathogenesis of autoimmune diseases. TLRs belong to the family of pattern recognition receptors (PRRs) that recognize a wide range of pathogen-associated molecular patterns (PAMPs). TLRs are type I transmembrane proteins and located on various cellular membranes. Two main groups have been classified based on their location; the extracelluar group referred to the ones located on the plasma membrane while the intracellular group all located in endosomal compartments responsible for the recognition of nucleic acids. They are released by the host cells and trigger various intracellular pathways which results in the production of proinflammatory cytokines, chemokines, as well as the expression of co-stimulatory molecules to protect against invading microorganisms. In particular, TLR pathway-associated proteins, such as IRAK, TRAF, and SOCS, are often dysregulated in this group of diseases. TLR-associated gene expression profile analysis together with single nucleotide polymorphism (SNP) assessment could be important to explain the pathomechanism driving autoimmune diseases. In this review, we summarize recent findings on TLR pathway regulation in various autoimmune diseases, including Sjögren's syndrome (SS), systemic lupus erythematosus (SLE), multiple sclerosis (MS), rheumatoid arthritis (RA), systemic sclerosis (SSc), and psoriasis.

Megjegyzések:Background : Growing evidence shed light on the significant effects of physical activity on immunomodulation. Consequences may largely depend on the type of activity, its intensity and duration. However, little information is available regarding the immunological effects of sporting activities in older ages. The aim of our study was to examine the changes in a wide spectrum of lymphocyte subtypes after a period of regular workout among healthy women of different ages. Method : In a cross- sectional study, we enrolled 34 young adult women (between ages of 19- 23 years), 8 of them were a player of a team in Hungarian Women ' s Volleyball League, while 26 of them were not engaged in regular physical activity. Additionally, we enrolled 16 elderly women (between ages of 60- 75 years) not engaged in regular physical activity. During a follow- up study, 17 from the group of non- athlete young adults completed a 12- week Pilates workout program, while the group of elderly women took part in a 6- week lightweight conditioning gymnastic exercise program once a week. The percentages of peripheral natural killer (NK), NKT cells, T and B lymphocyte subtypes (early- /late- activated T, naive and memory T, cytotoxic T (Tc), T- helper (Th)1, Th2, Th17, T regulatory type 1 (Tr1), CD4 + CD127- CD25 bright Treg, as well as naive and memory B cells) were determined by flow cytometry based on the staining of extracellular markers and intracellular cytokines. Results : The investigated baseline parameters did not show significant differences between athlete and non- athlete young adults. In the elderly, levels of CD3 + 6B11 + NKT cells were lower, while ratios of CD4 + Th/CD8 + Tc cells were higher compared to the values of younger individuals. At the end of exercise programs, changes observed among 60- 75 year- olds were more pronounced compared to alterations developed in younger subjects. In elderly women, percentages of IgD+ naive B cells decreased, while levels of CD27 + switched- memory B cells increased. Furthermore, proportions of CD4 + IL- 4 + Th2 cells increased, while levels of CD8 + IFNgamma+ Tc cells and immunosuppressive CD4 + CD127- CD25 bright Treg cells decreased as the result of regular exercise. Conclusion : Differences observed after lightweight exercise programs reflect a presumably enhanced immunoreactivity and increased ability for immune responses, especially in older ages.
