Megjegyzések:The aim of this study was to describe the effect of systemic alphacalcidol (1 alpha OH vitamin D3) treatment on clinical and immunological parameters in patients with psoriatic arthropathy. Among the 19 patients investigated, 10 were treated with 0.25 mikro g oral alphacalcidol twice daily for 6 months, while 9 other patients served as controls. In the peripheral blood of the treated group but not in the controls, a statistically significant decrease was observed in the percentage of CD3/CD69-positive activated and CD8-positive interferon-?-producing T cells and in the serum level of interferon-? during the first 3 months and also in the clinical activity of the disease during the whole 6-month follow-up period. Our results show that systemic alphacalcidol treatment has an immunomodulatory effect on patients with psoriatic arthropathy. This effect is manifested by a short-term temporary decrease in type 1 immune responses and a continuous decrease in disease activity.

Megjegyzések:Our aim was to assess whether the presence of highly active effector T-cells in atopic dermatitis (AD) is associated with changes in the number and/or function of regulatory T-cells (Tregs). Flow cytometry was utilised to determine the percentage of CD4+CD25brightCD127-/lowFOXP3+ and skin-homing CLA+CD4+CD25brightFOXP3+ Tregs in healthy controls and AD patients. The correlation between disease severity and Treg percentages was estimated. Treg suppressor activity and cell proliferation were measured after T-cell stimulation. Significantly increased percentages of Tregs were found in AD patients compared to healthy individuals, and significant correlation between the frequency of Tregs and disease severity was also detected. The otherwise normal suppressor activity of Tregs decreased in the presence of Staphylococcus enterotoxin B (SEB). In conclusion, the continuous presence of SEB can trigger an acquired functional impairment of Tregs in AD patients and the correlation between the increased frequency of Tregs and disease severity supports their important role in AD pathogenesis.

Megjegyzések:In this study we investigated skin bacterial colonization, allergen-specific IgE and antiphospholipid/antinuclear antibodies in 72 children with atopic eczema/dermatitis syndrome (age 2-17 years). Bacteria were found on the skin in 41 cases and serological allergen-specific IgE positivity in 37. The different forms of antibodies appeared in the ratio 21/72 (33 antibodies in 21 children). The occurrence of antiphospholipid antibodies was significantly higher in the patients than in the controls. There were significantly more allergens in the group with bacterial colonization than in the group without colonization. The SCORAD index showed a significant positive association with the skin colonization. We conclude that there are significant relationships between the occurrence of Staphylococcus aureus colonization and the levels of inhalant allergen-specific IgE in children with atopic eczema/dermatitis syndrome, and between the occurrence of antiphospholipid IgM positivity and atopic eczema/dermatitis syndrome.

Megjegyzések:Pemphigus vulgaris (PV) developing in conjunctionwith D-penicillamine treatment is a rare disorder; todate the number of described cases is about 100 (1). Most reports of D-penicillamine-induced PV are inpatients with rheumatoid arthritis (RA) (2), but thereare no data in the medical literature about this complicationin systemic sclerosis-rheumatoid arthritisSSc-RA overlap syndrome. We here report a case ofD-penicillamine-induced PV in a patient with SSc-RAoverlap syndrome. Discontinuation of the offendingagent and a high dose of methylprednisolone wererequired to terminate the disease flare.
