Megjegyzések:Chronic urticaria (CU) is characterised by the occurrence of widespread, short-lived weals, appearing daily or almost daily for at least 6 weeks. (1) There has been strong evidence supporting an autoimmune basis of the disease in approximately 27-50% of CU patients, who have functional autoantibodies against the high affinity IgE receptor and/or IgE. (1) A correct diagnosis of autoimmune CU (ACU) is difficult but important because of the presence of more severe clinical symptoms. Currently, autologous serum skin test (ASST) serves as a screening method, after which more adequate confirmation is gained through functional assays, to identify the ACU group.

Megjegyzések:In a recently published research letter Burney et al. demonstrated that isotretinoin (13-cis-retinoic acid, 13CRA) induced acnegenic changes in SEB-1 cultured human sebocytes1 . However, Melnik et al., suggested that the observed changes may largely be influenced by the Simian virus (SV)-40 immortalization of the used cell line, which may inhibit the p53 pathway and together with 13CRA lead to changes, such as the down-regulation of the forkhead box O (FoxO) pathway. In contrast, they found that the FoxO pathway was upregulated in samples from patients receiving 13CRA treatment for 6 weeks.

Megjegyzések:Background Antibodies directed to the a subunit of the high affinity IgE receptorand the IgE molecule are proposed to be of pathogenetic relevance in a group ofpatients with chronic urticaria (CU). The diagnosis of autoimmune chronic urticaria(ACU) is difficult; the autologous serum skin test (ASST) seems to be a usefulscreening test, but reliable, additional confirmatory methods are needed.Objectives To assess the diagnostic value of a modified serum-induced basophilactivation test, the CD63 expression assay, in the diagnosis of ACU by comparingthe results of the CD63 assay with the results of the histamine release (HR) test,the ASST and serum levels of soluble CD40 ligand (sCD40L).Methods Using basophils from an atopic (DA) and a nonatopic (DNA) donor theactivity of sera of 72 patients with CU were measured in HR assay by enzymelinkedimmunosorbent assay and in CD63 expression assay by flow cytometry.An ASST was carried out in all patients; in 30 of the 72 patients sCD40L wasdetected and correlations were derived between the different assays. Sera of 20normal controls and 26 patients with systemic autoimmune diseases were alsotested in the HR assay and in the CD63 expression assay.Results Histamine-releasing activity was detected in the sera of 51% (DA) and 32%(DNA) of CU patients and 57% (DA) and 28% (DNA) of sera upregulated CD63expression on the surface of basophils from the different donors. There was asignificant correlation between the HR and the CD63 assays carried out on bothdonors, but the ASST showed a strong correlation with the HR assay only for basophilsfrom the DA. The serum level of sCD40L was significantly higher inpatients with CU compared with controls, but the difference between the autoimmuneand the nonautoimmune groups was not significant.Conclusions The CD63 expression assay seems to be a reliable functional test in thediagnosis of ACU, particularly if highly sensitive donor basophils are used, butthe determination of the sCD40L serum level was not sufficient to differentiate

Megjegyzések:BACKGROUND: No studies to date have employed the EuroQoL EQ-5D questionnaire to assess health-related quality of life (HRQoL) in patients with pemphigus. OBJECTIVES: To evaluate the HRQoL of patients with pemphigus by the EQ-5D and to analyse the convergent and known-groups validity of the EQ-5D in this patient population. METHODS: Between 2014 and 2017, a multicentre cross-sectional study was carried out. Outcome measures included the five-level EQ-5D (EQ-5D-5L), Dermatology Life Quality Index (DLQI), Autoimmune Bullous Skin Disorder Intensity Score (ABSIS) and an average pain intensity visual analogue scale (VAS) for the past 3 months. RESULTS: In total, 109 consecutive patients with pemphigus participated in the study (mean age 57 years; 64% women). Among the EQ-5D dimensions, the most problems were reported regarding pain/discomfort (50%), mobility (43%) and anxiety/depression (43%). No significant difference was found in mean EQ-5D index scores between patients with pemphigus vulgaris and those with pemphigus foliaceus (0?81 vs. 0?86, P = 0?14). The mean EQ-5D index scores of patients with limited, moderate, significant and extreme pemphigus were 0?88, 0?82, 0?72 and 0?67, respectively (P = 0?001). The number of comorbidities was associated with greater impairment in EQ-5D index scores (P < 0?001). DLQI (rs = -0?62, P < 0?001) and the average pain intensity VAS (rs = -0?59, P < 0?001) more strongly correlated with the EQ-5D index scores than did ABSIS (rs = -0?40, P < 0?001). CONCLUSIONS: This is the first study employing the EQ-5D questionnaire in pemphigus. The EQ-5D is a valid measure of HRQoL in patients with pemphigus that can be useful both in clinical practice and in economic evaluations to assess the health gains associated with new effective treatments.
