Megjegyzések:In the first part of the publication the rheological properties of the vehicles used for the production of suppositories were studied in order to determine the ideal parameters for formulation. In this part a detailed methodology and the results of the in vitro membrane diffusion and in vivo bioavailability studies, are presented. The results confirm a general correlation between the in vitro and the in vivo findings. It seems that hydrophilic macrogol-mixture with great molecular mass can be recommended as the optimal vehicle for formulation of diazepam suppositories

Megjegyzések:Status epilepticus is one of the most common neurologic emergencies in children, adolescents, and young adults. The advantage of diazepam suppository in medicinal therapy appears mainly in treatment of childhood epilepsy. A hydrophilic suppository base was investigated in solution by dynamic light scattering and results were compared to those of the membrane diffusion experiments measuring diazepam solubilization. According to the dynamic laser light scattering photometric measurements, the good solubilization effect of the macrogol mixture (5% Polysorbatum 20, 10% Macrogolum 400, 85% Macrogolum 1540) is explained by the formation of small, tight micellas. As the Polysorbatum 20 tenside resulted in the significant decrease (p < 0.05) of the formation of great micellas, its use led to the formation of small, tight, almost monodisperse micellas of 40-50 nm in the aqueous solution of the macrogol mixture.

Megjegyzések:Fomivirsent was approved by the FDA in 1998 and by the EMA in 1999 as the very first antisense drug used to treat CMV retinitis in patients with AIDS. To date, it has been the only first generation antisense oligonucleotide used in therapy. Fomivirsen has been a pioneer in this field and has demonstrated the usefulness of the antisense tehcnology for medicinal science. However, after three years of use, fomivirsen has been withdrawn from the market (in the US in 2001 and in the EU in 2002), and nowadays, gene silencing drugs with a more advanced chemical structure and more complex mechanism of action are used in medicine. On the occasion of the 20th anniversary of its European withdrawal, we briefly overview the history of fomivirsen.

Megjegyzések:The rheological properties of four lipophilic and three hydrophilic suppository bases were studied. The flow and viscosity curves, the initial, equilibrium and plastic viscosity as well as the Bingham's yield value of the systems were determined. It was found that the viscosity curves had two different parts in the examined shearing rate range: the part of greater rise described a definitive destruction of the system, while the part of smaller rise represented an equilibrium state. The values of the rheological parameters of the lipophilic and hydrophilic systems showed a definite difference, which was explained by the greater association of the hydrophilic bases.

Megjegyzések:Background Scientific and clinical evidences documented that vitamin D is essential to maintain calcium absorption and consequently the normal funtion of the musculoskeletal system in human. The deficiency is rather endemic due to significant Vitamin D deficiency detected among most of population. Clinical trial data over the past decades clearly support the skeletal benefits of vitamin D supplementation in patients with circulating 25OHD levels of less than 30 nmol/L. The role of vitammin D supplementation has been establashed for the indication also in prevention to minimize the risk of falls and fractures in general. However, the use of bolus or high-dose vitamin D supplementation in clinical trials among elderly people shown unexpeced adverse drug reactions and the need for further surveillance in dosing specifications of vitamin D therapies. Aims An overall goal is to establish the safety specificiaction of vitamin D supplementation for prevention and also in various medical conditions in order to minimise the risk of falls and fractures in elderly population. Additionally, to identify the risks associated with the most relevant life-style factors and nutritional conditions that may interfere with the success of vitamin D supplementation applied for prevenetion in general. Methods The results of clinical trials based on observational studies at clinical facilities and supported by survey in general public focused on alimentary, solar exposition, the use of dietary- and food supplementation but also assessed the edherence to given medications and other life-style factors. Results VItamin D supplementation is indicated below 75 nmol/L, which is almost 90% of population among elderly and approx. 65-70% in general for the mass of population. The risk of fractures are significantly higher among patients with low 25OHD levels (OR 2.83; CL 1.51-5.31, p=0.007). But over 25% of population is not adherent to prescribed OP therapies. Alimenentary and life-style conditions are both needed optimal skeletal health: two-thirds of the subjects are underestimated their daily calcium intake, although only less then 30% of them are aware of the importance of calcium intake. Moreover, the physical activity level is being adequate in case of 28% even among the younger population. Conclusion The option of loading plus maintenance dose schedule proven to be efficient in cases of significant Vitamin D deficiency, however with proper nutritional and life-style conditions only in reduction of risk of falls and fractures.
