Megjegyzések:Influenza is one of the most common respiratory diseases in humans. The response to vaccination is frequently poor in immunosuppressed individuals. The aim of the present study was to develop an enzyme-linked immunospot (ELISPOT) assay for measuring of the specific T-cell response to influenza vaccination. In all, 18 healthy subjects and six stem cell transplantation (SCT) patients tested before and 4 weeks after influenza vaccination were included in the present study. Peripheral blood lymphocytes were stimulated with four influenza peptides; three based on sequences from the hemagglutinin and one from the M1 protein. The ELISPOT assay and the measurement of intracellular IFN-gamma production were used to determine the cell-mediated responses after stimulation with the peptides. Influenza vaccination elicited strong cell-mediated immune responses in the healthy controls to all four peptides with 3.2-6.9-fold increases in the number of IFN-gamma producing spots/10(6) cells. By intracellular staining, it was suggested that CD4+ cells mediated the responses to the hemagglutinin peptides. In contrast, there was no increase in the number of IFN-gamma producing cells response after vaccination in the six SCT patients. In conclusion, our results suggest that the ELISPOT assay might be used as a complement to serology for monitoring of future influenza vaccine studies in SCT patients.

Megjegyzések:The aim of this study is to evaluate the relationshipof serum sclerostin levels with age, cystatin C,bone mineral density (BMD) and biochemical markers ofbone turnover in healthy Hungarian men[50 years of age.We determined serum levels of sclerostin and examinedits relationship to age, cystatin C, osteocalcin, C-terminaltelopeptides of type-I collagen, procollagen type 1 aminoterminalpropeptide, 25-hydroxyvitamin D, parathyroidhormone, and L1?L4 (LS) and femur neck (FN) BMD dataavailable from 194 randomly selected ambulatory menbelonging to the HunMen cohort. In the study population asa whole [n = 194; age (median, range) 59 (51?81) years],statistically significant correlation was found betweensclerostin and age (r = 0.211; p = 0.003), cystatin C(r = 0.246; p = 0.001), FN BMD (r = 0.147; p = 0.041)and LS BMD (r = 0.169; p = 0.019). Compared tomiddle-aged men (age B59 years, n = 98), elderly men(age [59 years, n = 96) had significantly higher serumsclerostin levels (67.8 ? 15.9 vs 63.5 ? 14 pmol/L; p =0.047). Among men with normal (T score [-1.0) FNBMD, the elderly had significantly higher serum sclerostinlevels compared to the middle-aged men (70.4 ? 17 vs63.9 ? 11.5 pmol/L; p = 0.019). Furthermore, among theelderly men cystatin C was the only significant predictor ofserum sclerostin levels (standardized regression coefficient(b) = 0.487; p\0.001). In the studied healthy elderlycohort, this study reports a significant increase in sclerostinlevels with increasing age and deteriorating kidney functionas determined by plasma cystatin C levels.

Megjegyzések:INTRODUCTION: Citrullination as well as anti-citrullinated protein/peptide antibodies (ACPA) have been implicated in the pathogenesis of rheumatoid arthritis (RA). While ACPAs are specific and sensitive markers for RA, there have been hardly any reports regarding ACPAs in ankylosing spondylitis (AS). The possible role of antibodies to Mycobacterial 65kDa heat shock protein (hsp65) has not been characterized in AS. As new laboratory biomarkers of AS are needed, we investigated the prevalence of anti-mutated citrullinated vimentin (MCV) and anti-hsp65 antibodies in AS. METHODS: Altogether 43 AS and 44 healthy controls were included in the study. Anti-MCV and anti-hsp65 were determined in sera by commercial and in-house ELISA, respectively. Serum autoantibody levels were correlated with ESR, CRP, HLA-B27 status, smoking habits, pain intensity, BASDAI, BASFI and BASMI indices. RESULTS: Patients with AS had significantly higher serum anti-MCV levels (17.3U/mL, range: 8.3-31.5U/mL) in comparison to healthy subjects (8.9U/mL, range: 5.4-13.3U/mL) (p<0.01). Sixteen of the 43 AS patients (37%) and none of the 44 healthy controls (0%) were anti-MCV positive using the cut-off value recommended by the manufacturer (>20U/mL). The mean anti-hsp65 concentration in AS sera was 124.8AU/mL (range: 27.2-1000AU/mL), while controls exerted significantly lower anti-hsp65 levels (mean: 51.8 AU/mL; range: 22.5-88.5AU/mL) (p<0.001). Correlation analysis revealed that both anti-MCV positivity (r=0.613; p=0.012) and absolute serum anti-MCV levels (r=0.553; p=0.021) correlated with anti-hsp65 levels. Anti-MCV positivity also correlated with ESR (r=0.437; p=0.03). CONCLUSIONS: Anti-MCV and anti-hsp65 may be novel biomarkers in AS.

Megjegyzések:A biosimilar is a biopharmaceutical product intended to be comparable to a previously licensed biopharmaceutical agent. The goal of such products is to increase the accessibility of biopharmaceutical therapy for rheumatoid arthritis by reducing costs. They are not like generic drugs, in that they may differ from the reference products in manufacturing, composition, and formulation. Regulatory authorities strive to ensure the absence of clinically meaningful differences between biosimilars and their reference drugs. However, small molecular differences may potentially affect pharmacodynamics (including affinity), pharmacokinetics, and immunogenicity. Intended copies are non-innovator biopharmaceutical products that, unlike biosimilars, do not have enough clinical evidence to demonstrate biosimilarity. For approval of a biosimilar, most countries require preclinical and clinical studies demonstrating comparability with the reference drug. The margin for determining equivalence or non-inferiority is determined on a case-by-case basis in each country, as there are no general criteria. The European Medicines Agency and US Food and Drug Administration have stringent regulatory processes to ensure comparability of biosimilars with their reference drugs. There are also post-marketing surveillance requirements to monitor safety. Only one biosimilar, CT-P13,has beenapproved for rheumatoid arthritis. However, incountries with less stringent regulation, intended copies are being commercialized and safety problems have been documented. Consequently, in such countries, there is an urgent need for appropriate regulatory processes to be established. Attempts to close the affordability gap of biopharmaceuticals should not open another gap between patients treated with an innovator drug and an intended copy.
