Megjegyzések:The protein composition of normal and pathological tears was studied by polyacrylamide-gel (disc) electrophoresis. Polyacrylamide-gel electrophoresis was shown to detect at least 14 fractions in 2-10 microliters of native tears. A comparison was made between the protein composition of normal tears and serum. The most characteristic bands in the tear-protein pattern were identified by parallel electrophoresis of tears, serum, human milk and purified egg-white lysozyme. The identified fractions were specific tear prealbumin, serum albumin, transferrin, lactoferrin and lysozyme. An unidentified major tear-protein component was also described. The tear-protein pattern was divided into six zones: (1) prealbumin zone; (2) post-albumin zone; (3) post-transferrin zone; (4) macroglobulin zone; (5) basic globulin zone; (6) prelactoferrin zone. A significant rise in the level of serum albumin and transferrin was demonstrated in tears from cases of acute catarrhal conjunctivitis. The optimal circumstances were discussed under which major and minor tear components and basic and acidic tear proteins can be determined simultaneously. Polyacrylamide-gel electrophoresis is recommended as a useful method to study the various diseases of the anterior segment of the eye.

Megjegyzések:Purpose We measured vascular endothelial growth factor(VEGF) levels in tear fluid and serum in patients with retinalvein occlusion (RVO).Patients and methods Eight patients with RVO due to secondarymacular oedema were examined. VEGF levels were measuredby enzyme-linked immunosorbent assay. All patientshad a full ophthalmic examination (visual acuity, slit lampbiomicroscopy, perimetry, and fluorescein angiography). Centralretinal thickness (CRT) was examined using optical coherencetomography (OCT). Tear and serum samples werecollected and examinations were performed at diagnosis and1 and 4 weeks later.Results VEGF levels in the tears of RVO eyes were significantlyhigher than in fellow eyes at diagnosis and after both 1and 4 weeks (paired t test, p1=0.01, p2=0.02, p3=0.006). Wefound a weak but significant positive correlation betweenVEGF levels in tear fluid and serum of patients with RVO(r=0.21), while this correlation tended to be stronger betweenthe fellow eyes and serum levels (r=0.33).Conclusion To the best of our knowledge, we are the first toreport an increased level of VEGF in the tear fluid of patientswith RVO. Alterations of VEGF levels in tears may be usefulfor determining stages of RVO. This non-invasive and objectivemethod may also be helpful for estimating the severity ofmacular oedema and efficacy of treatment.

Megjegyzések:To identify changing trends in indications for corneal transplantation in Debrecen, Hungary over the past 64 years. DESIGN: Retrospective study, at the Department of Ophthalmology, University of Debrecen, Hungary. PARTICIPANTS: Four thousand seven hundred and seventy-eight patients who underwent keratoplasty. METHODS: The analysis was based on medical charts, surgical descriptions and eye bank records. MAIN OUTCOME MEASURES: Keratoplasty indications between January 1946 and December 2009. For an easier overview, the 64-year interval was divided into seven time periods (1946-1955, I; 1956-1965, II; 1966-1975, III; 1976-1985, IV; 1986-1995, V; 1996-2005, VI; 2006-2009, VII). RESULTS: Over the 64 years, clinical indications for keratoplasty were corneal scarring (24.9%), regraft (18.6%), keratoconus (18.6%), pseudophakic/aphakic corneal oedema (12%), stromal corneal dystrophies (6%), non-infectious keratitis (4.7%), chemical injuries (3.3%), corneal degenerations (3%), mechanical trauma (1.7%), infectious keratitis (1.4%), endothelial corneal dystrophies (1.3%) and others (4.5%). During periods I-IV, corneal scarring was the most common indication for corneal transplantation. In period V, corneal ectasia became the leading clinical indication. Regraft was the most frequent indication in period VI. In the most recent years, an emerging tendency in pseudophakic/aphakic corneal oedema as the indication for keratoplasty was observed. CONCLUSIONS: In Hungary, the number of grafts has increased greatly in the past 64 years, the transplantation rates are similar to those of industrial countries. Indications for corneal transplantation have changed considerably over the last half decade from corneal scarring to corneal ectasia, regraft and pseudophakic/aphakic corneal oedema.

Megjegyzések:P-selectin receptor is expressed in platelets and endothelial cells in a cell-activation-dependent manner. Platelet P-selectin (CD62) levels may become elevated in a number of vasoocclusive diseases, including arteriosclerosis, atherothrombosis, and diabetes mellitus (DM). Nonarteritic anterior ischemic optic neuropathy (NAION) is associated with a sudden loss of vision due to the vascular insufficiency of ciliary arteries supplying the optic nerve. In this study, our aim was to investigate the presence of increased platelet reactivity in the development of NAION. METHODS: Twenty-one NAION patients, 39 healthy control subjects, and 44 patients suffering from diabetes mellitus (DM) were examined in our case-control, pilot study. Platelet activation was investigated by flow cytometric analysis of the mean fluorescence intensity (MFI) of CD62 on platelets. These results were compared among the different study groups. RESULTS: NAION patients showed considerably although not significantly (p = 0.2017) higher P-selectin MFI values (71.98 ± 40.30) versus healthy subjects (55.48 ± 20.95), insulin-dependent DM patients (50.02 ± 13.08), and non-insulin-dependent DM subjects (54.72 ± 24.74). However, logistic regression analysis resulted in a statistically significant adjusted effect on the odds of NAION when CD62 MFI values were logarithmically transformed (OR: 3.86, 95 % CI: 1.10 to 13.53, p = 0.0346). CONCLUSION: Elevated platelet CD62 positivity may be related to NAION, suggesting a possible role of enlarged platelet activity in the generation of this type of ischemic optic neuropathy.

Megjegyzések:Purpose Comparison of postoperative refraction results using ultrasound biometry with closed immersion shell and optical biometry. Patients and method Three hundred and sixty-four eyes of 306 patients (age: 70.6±12.8 years) underwent cataract surgery where intraocular lenses calculated by SRK/T formula were implanted. In 159 cases immersion ultrasonic biometry, in 205 eyes optical biometry was used. Differences between predicted and actual postoperative refractions were calculated both prior to and after optimization with the SRK/ T formula, after which we analysed the similar data in the case of Holladay, Haigis, and Hoffer-Q formulas. Mean absolute error (MAE) and the percentage rate of patients within ±0.5 and ±1.0 D difference in the predicted error were calculated with these four formulas. Results MAE was 0.5-0.7 D in cases of both methods with SRK/T, Holladay, and Hoffer-Q formula, but higher with Haigis formula. With no optimization, 60-65 % of the patients were under 0.5 D error in the immersion group (except for Haigis formula). Using the optical method, this value was slightly higher (62-67 %), however, in this case, Haigis formula also did not perform so well (45 %). Refraction results significantly improved with Holladay, Hoffer-Q, and Haigis formulas in both groups. The rate of patients under 0.5 D error increased to 65 % by the immersion technique, and up to 80 % by the optical one. Conclusions According to our results, optical biometry offers only slightly better outcomes compared to those of immersion shell with no optimized formulas. However, in case of new generation formulas with both methods, the optimization of IOL-constants give significantly better results.

Megjegyzések:Background: The presentation of microcornea associated with axial myopia by means of modern examination techniques. Methods: Clinical investigations. Results: The horizontal diameter of the cornea was 8.00 mm on both sides. The average refractive power of the cornea was 39.27D/38.48D on the right/left sides by corneal topography. The central corneal thickness was 568 um/ 559 um on the right and left sides, respectively. The depth of the anterior chamber was 1.18 mm/1.14 mm and the origin of the irises was steep, as demonstrated by ultrasound biomicroscopy (UBM). The axial length was 26.42 mm/25.63 mm on the right/left sides. Endothelial morphology disclosed degeneration on both sides.
