Megjegyzések:True umbilical cord knot often remains undiscovered prenatally due to a lack of characteristic clinical or ultrasound signs. We present a pregnancy with favorable outcome in which a non-stress test (NST) found non-reassuring fetal status. Abnormal Doppler blood flow patterns in the descending aorta and in the middle cerebral artery revealed fetal arterial redistribution with normal circulation in the umbilical artery despite a true cord knot. The benefit of fetal Doppler assessment is discussed. Increased aortic-cerebral ratio may reflect acute hypoxic compromise caused by the transitory constriction of the true umbilical cord knot with unrecognized morphologic and circulatory signs.

Megjegyzések:To investigate the association of pre-eclampsia (PE) or PE with superimposed chronic hypertension (PE+SCH) in pregnant women with the risk of various structural birth defects (i.e. congenital abnormalities) in their offspring. STUDY DESIGN: A population-based case-control study using the Hungarian Case-Control Surveillance of Congenital Abnormalities data set (1980-1996), including 22,843 cases with congenital abnormalities and 38,151 matched controls without any congenital abnormalities. The incidence of PE and PE+SCH was compared in women who had offspring with congenital abnormalities (cases) and women who had offspring without any congenital abnormalities (controls). RESULTS: The incidence of PE was examined in 585 cases and 1017 controls, and the incidence of PE+SCH was examined in 154 cases and 269 controls. None of the 25 studied types of congenital abnormality was found to be more likely among the offspring of women with PE. However, the risks of renal dysgenesis [odds ratio (OR) 4.7, 95% confidence interval (CI) 1.7-12.8], esophageal atresia/stenosis (OR 4.6, 95% CI 1.8-12.2) and rectal/anal stenosis (OR 3.7, 95% CI 1.6-8.5) were higher in the offspring of pregnant women with PE+SCH. CONCLUSIONS: PE in pregnant women was not associated with a higher risk of any congenital abnormalities in their offspring, but PE+SCH was associated with a higher risk of renal dysgenesis, esophageal atresia/stenosis and rectal/anal stenosis. These findings need confirmation in other studies.

Megjegyzések:STUDY OBJECTIVE: To measure the efficacy of transcervical hysteroscopic fibroid resection (TCRM) among the authors' patients. STUDY DESIGN: In one department, 73 transcervical hysteroscopic fibroid resections were performed on 68 patients over a 6-year period. Using a computerized database and mailed questionnaires, 58 patients were followed up. RESULTS: Thirty-six of the 58 patients showed no symptoms after the procedure; in 15 cases subsequent fibroid was detected, while in seven cases other types of complaints occurred, not related to the myoma. During the procedures complications happened in four cases; one perforation and three bleeding episodes occurred. CONCLUSION: Our data indicate that TCRM is a safe and successful method of treating submucosal fibroids.

Megjegyzések:OBJECTIVE: To summarize the characteristics of female child sexual abuse and toexplore common features that may be utilized as targets for possible methods ofprevention. DESIGN: Prospective, longitudinal study. SETTING: A Hungarian county,University of Debrecen. PARTICIPANTS: Between 1986 and 2001, 209 girls under theage of 18 who had been exposed to sexual abuse visited the Department ofAdolescent Gynecology. METHODS: We prospectively collected data illustrating thecharacteristics of all cases. Events in which victim and perpetrator were membersof the same family were recorded as intrafamilial. Subsequent legal procedureswere also evaluated. MAIN OUTCOME MEASURES: We describe the medical and legalapproaches to handling child neglect. RESULTS: Seventy-five percent of thevictims were students, and 47% of them were between 11 and 14 years of age. Theperpetrator was familiar to the victim in 66% of the cases, and a stranger in34%. Fifty-two (25%) perpetrators were members of the victims' families. In 11%of cases, the perpetrator was the victim's father and in 10%, her stepfather. Theabuse had occurred on multiple occasions in 21%. The occurrence rate of assaultwas highest in the summer season (59%). Thirty-nine percent of victims wereaccompanied by their mothers when they attended the clinic and 43%, by police.officers. Vaginal penetration was the type of abuse in 80%, and sexual perversionin 20%. Sixty-six victims were physically injured, and in 38 cases the presenceof sperm was confirmed in vulvo-vaginal smears. One pregnancy occurred as theresult of the abuse. In all, 127 cases were reported to the police; 56 of theperpetrators were sentenced as a result of legal proceedings. CONCLUSION: A highproportion of female child sexual abuse takes place within the family and isrevealed only after multiple episodes. The low reported prevalence of sexualassault is the consequence of the lack of cooperation between the emergencyservices in Hungary and the Hungarian criminal law. Prevention requires vigilancein out-of-school times, child education, early involvement of healthcareprofessionals, and adjustment of the administration of justice.

Megjegyzések:Objective: To compare ectopic pregnancy (EP) rates among users of non-medicated and copper-containing IUDs. Study design: The number of women with non-medicated and copper devices were 3491 and 11,682, respectively. Cumulative woman-months of use (CWMU), Pearl Index (PI) and gross cumulative lifetable EP rates for 1000 women were calculated. Results: The numbers of users at the end of the 10th year were 550 and 569 for non-medicated and copper-containing IUDs, respectively. CWMU for plastic and copper devices was 191,678 and 473,533, respectively, at the 169th month of maximum follow-up. The PIs were 0.8 and 1.0 calculated for 1000 users per year. The gross cumulative lifetable EP rates for 1000 women were 1.4 and 0.9 at 12 months, and 6.8 and 8. 9 at the end of the 10th year, respectively, for the plastic and the copper IUDs. Conclusion: Although the rates were lower for the plastic IUD in the majority of the observations, there were no statistically significant differences between the two groups.
