Megjegyzések:Endometrial carcinosarcoma is a rare and aggressive high-grade endometrial carcinoma with secondary sarcomatous trans-differentiation (conversion theory). The clinical presentation and diagnostic work-up roughly align with those of the more common endometrioid counterpart, although endometrial carcinosarcoma is more frequently diagnosed at an advanced stage. Endometrial carcinosarcoma is not a single entity but encompasses different histological subtypes, depending on the type of carcinomatous and sarcomatous elements. The majority of endometrial carcinosarcomas are characterized by p53 abnormalities. The proportion of POLE and microsatellite instablity-high (MSI-H) is directly related to the epithelial component, being approximately 25% and 3% in endometrioid and non-endometrioid components.The management of non-metastatic disease is based on a multimodal approach with optimal surgery followed by (concomitant or sequential) chemotherapy and radiotherapy, even for early stages. Palliative chemotherapy is recommended in the metastatic or recurrent setting, with carboplatin/paclitaxel doublet being the first-line regimen. Although the introduction of immunotherapy plus/minus a tyrosine kinase inhibitor shifted the paradigm of treatment of patients with recurrent endometrial cancer, patients with endometrial carcinosarcoma were excluded from most studies evaluating single-agent immunotherapy or the combination. However, the US Food and Drug Administration (FDA) and the European Medicines Agency (EMA) approved the use of pembrolizumab and lenvatinib in endometrial cancer (all histotypes) after progression on chemotherapy and single-agent immunotherapy in MSI-H cancers. In the era of precision medicine, emerging knowledge on molecular endometrial carcinosarcoma is opening new promising therapeutic options for more personalized treatment. The present review outlines state-of-the-art knowledge and future directions for patients with endometrial carcinosarcoma.

Megjegyzések:Enzalutamide (ENZA) is a frequently used therapy in metastatic castration-resistant prostate cancer (mCRPC). Baseline or acquired resistance to ENZA have been observed, but the molecular mechanisms of resistance are poorly understood. We aimed to identify proteins involved in ENZA resistance and to find therapy-predictive serum markers. We performed comparative proteome analyses on ENZA-sensitive parental (LAPC4, DuCaP) and -resistant prostate cancer cell lines (LAPC4-ENZA, DuCaP-ENZA) using liquid chromatography tandem mass spectrometry (LC-MS/MS). The top 4 most promising candidate markers were selected using bioinformatic approaches. Serum concentrations of selected markers (ALCAM, AGR2, NDRG1, IDH1) were measured in pre-treatment samples of 72 ENZA-treated mCRPC patients using ELISA. In addition, ALCAM serum levels were measured in 101 Abiraterone (ABI) and 100 Docetaxel (DOC)-treated mCRPC patients' baseline samples. Results were correlated with clinical and follow-up data. The functional role of ALCAM in ENZA resistance was assessed in vitro using siRNA. Our proteome analyses revealed 731 significantly differentially abundant proteins between ENZA-sensitive and -resistant cells and our filtering methods identified 4 biomarker candidates. Serum analyses of these proteins revealed only ALCAM to be associated with poor patient survival. Furthermore, higher baseline ALCAM levels were associated with poor survival in ABI- but not in DOC-treated patients. In LAPC4-ENZA resistant cells, ALCAM silencing by siRNA knockdown resulted in significantly enhanced ENZA sensitivity. Our analyses revealed that ALCAM serum levels may help to identify ENZA- and ABI-resistant patients and may thereby help to optimize future clinical decision-making. Our functional analyses suggest the possible involvement of ALCAM in ENZA resistance.

Megjegyzések:Ca(2+)-signaling of human melanoma is in the focus of intensive research since the identification of the role of WNT-signaling in melanomagenesis. Genomic and functional studies pointed to the important role of various Ca(2+) channels in melanoma, but these data were contradictory. In the present study we clearly demonstrate, in a number of different ways including microarray analysis, DNA sequencing and immunocytochemistry, that various human melanoma cell lines and melanoma tissues overexpress ryanodine receptor type 2 (RyR2) and express P2X(7) channel proteins as compared to melanocytes. These channels, although retain some of their usual characteristics and pharmacological properties, display unique features in melanoma cells, including a functional interaction between the two molecules. Unlike P2X(7), RyR2 does not function as a calcium channel. On the other hand, the P2X(7) receptor has an antiapoptotic function in melanoma cells, since ATP-activation suppresses induced apoptosis, while knock down of the gene expression significantly enhances that.

Megjegyzések:AimsLaparoscopic radical hysterectomy is a widely accepted operative treatment of early stage cervical cancer. Our aim was toanalyze retrospectively our initial results and the feasibility of this technique.MethodBetween July 2016 and April 2017 we performed laparoscopic type-C radical hysterectomy and pelvic lymphadenectomy in tenpatients with early stage (IA2-IIB) cervical cancer.ResultsThe mean operation time was 165 min (range, 135-198). During surgery, the average decrease in hemoglobin and hematocritlevels was 20.3 g/L and 0.07, respectively. The average width of parametrial resection was 3.7 cm and the mean lymph nodecount was 16.3. There was no conversion to laparotomy. One intraoperative urinary bladder injury occurred and treatedlaparoscopically at the same time. No postoperative complications higher than grade 2 occurred according to the Clavien-Dindoclassification. The mean hospital stay was 4.7 days.ConclusionAccording to our initial results, introduction of laparoscopic radical hysterectomy is a feasible approach to change practice insurgical treatment of early stage cervical cancer. It is associated with low blood loss and short hospitalization. In addition, it is asafe and effective technique for staging and treating cervical cancer. In comparison to open surgery, pathological evaluationsuggests non-inferior prognosis, but further analysis is required to assess long term outcome measures.

Megjegyzések:Background ESGO (European Society of Gynaecological Oncology) and partners are continually improving the developmental opportunities for gynaecological oncology fellows. The objectives of this survey were to evaluate the progress in the infrastructure of the training systems in Europe over the past decade. We also evaluated training and assessment techniques, the perceived relevance of ENYGO (European Network of Young Gynaecological Oncologists) initiatives, and unmet needs of trainees. Methodology National representatives of ENYGO from 39 countries were contacted with an electronic survey. A graduation in well/moderately/loosely-structured training systems was performed. Descriptive statistical analysis and frequency tables, as well as two-sided Fisher's exact test, were used. Results National representatives from 33 countries answered our survey questionnaire, yielding a response rate of 85%. A national fellowship is offered in 22 countries (66.7%). A logbook to document progress during training is mandatory in 24 (72.7%) countries. A logbook of experience is only utilized in a minority of nations (18%) for assessment purposes. In 42.4% of countries, objective assessments are recognized. Trainees in most countries (22 (66.7%)) requested additional training in advanced laparoscopic surgery. 13 (39.4%) countries have a loosely-structured training system, 11 (33.3%) a moderately-structured training system, and 9 (27.3%) a well-structured training system.Conclusion Since the last publication in 2011, ENYGO was able to implement new activities, workshops, and online education to support training of gynaecological oncology fellows, which were all rated by the respondents as highly useful. This survey also reveals the limitations in establishing more accredited centers, centralized cancer care, and the lack of laparoscopic training.

Megjegyzések:Introduction/Background* Ovarian cancer is the 7th most common malignancy among women and the leading cause of gynecologic cancer death. The most important prognostic factor of the disease is optimal debulking surgery (R0) with no macroscopic residual disease. Achieving optimal result is a challenging duty in advanced stage (FIGO IIIC-IV). Based on previous studies neoadjuvant chemotherapy (NAC) can help to improve the optimally debulked ratio of this popu- lation with non-inferior survival outcome. The aim of our study was to evaluate the effectiveness of NAC among pri- marily inoperable patients. The focus was not only on sur- vival outcome but on cost effectiveness (need for transfusion, hospitalization, ICU admission, medication demand, etc.). Methodology Between 2015-2018 112 debulking surgeries were performed on stage FIGO IIIC-IV ovarian cancer patients. The cases were divided into potentially operable and inoperable group based on preoperative imaging, tumor marker levels according to our institutional protocol. In spe- cial situation where operability was not obvious, diagnostic laparoscopy was done to categorize patients. The peri-, intra-, postoperative reports and survival data was collected. Result(s)* Complete tumor reduction was performed in 63 cases while in 49 cases only partial tumor reduction was achieved. Median progression-free survival did not differ sig- nificantly between patients who underwent primary or interval debulking surgery (PDS = 12 months, IDS = 11.2 months, p = 0.264). The rate of R0 resection was higher after NAC, but not significantly (37.9% vs. 54.2%, p = 0.179). There was no significant difference in survival of patients who suc- cessfully underwent complete tumor reduction, despite the fact that the "inoperable" group treated with NAC had a worse prognosis (p = 0.264). The cost-effectiveness was comparable between groups, the hospital stay and transfusion demand was favourable in neoadjuvant group. Conclusion* The rate of optimal tumor reduction can be improved not only by increasing radicality but also by apply- ing appropriate patient selection criteria. Neoadjuvant treat- ment according to the current recommendations is applicable in the inoperable group, in accordance with the protocol we use. The survival data of patients who have undergone com- plete tumor reduction after NAC was not inferior to those who went through primary debulking.
