Megjegyzések:Venous thrombosis is a typical common complex disease as acquired and genetic causes play a role in its development. The different "loss of function" mutations of the natural anticoagulant system lead to antithrombin (AT), protein C (PC) and protein S (PS) deficiencies. Since thrombophilia testing has high cost and it has several methodological issues (analytical, pre-analytical), which makes the interpretation of results difficult, considerations should be made on the indications of testing, on the parameters that are measured and on the best available method to use. The latest guideline on clinical and laboratory management of thrombophilia kept the relatively old laboratory recommendations unchanged. This is partly because of the existence of unresolved problems with the laboratory tests used for diagnosis. Based on the literature and our previous research here we discuss the unresolved problems, the recently raised questions and issues concerning AT, PC and PS laboratory diagnosis and summarize the recent findings in molecular genetic investigations.

Megjegyzések:With an aging population, there is a marked increase in prevalence of metabolic bone diseases, especially osteoporosis. Perhaps the most dreaded complication of metabolic bone disease, fractures typically impose a huge burden on the ailing body and are associated with high co-morbidity and mortality. The consequent public health and socioeconomic burden warrant timely diagnosis, treatment and follow-up of these disorders. Knowing the limitations of radiological techniques, biochemical markers of bone turnover measurements come handy since the changes in their levels readily reflect bone physiology. Bone biomarkers typically analyzed in high throughput automated routine laboratories are collagen degradation products, reflecting osteoclast activity, and the collagenous or non-collagenous proteins produced by the osteoblasts. Since bone biomarker levels vary considerably due to quite a few endogenous and exogenous pre-analytical factors, knowledge of these limitations is mandatory prior to clinical utilization since these variabilities complicate test result interpretation. Standardization to harmonize different assay methodologies is desired, and the primary aims of the IFCC/IOF bone marker standards working group are also presented. Current literature data advocate bone markers as best used in monitoring anti-osteoporosis therapy efficacy and compliance, nonetheless, there is abundant data supporting their role in predicting bone loss and fracture risk. Furthermore, they have widespread clinical utility in osteoporosis, renal osteodystrophy, and certain oncological conditions and rheumatic diseases.

Megjegyzések:Introduction Serum total lactate dehydrogenase (LDH) activity was elevated and showed a positive correlation with disease severity and outcome in severe COVID-19 disease. However, it is still unknown whether the relative abundance or calculated activity of any LDH isoenzyme is predominately increased in COVID-19 subjects. Methods Twenty-two consecutive patients suffered from moderate or severe COVID-19 pneumonia were recruited into this study who showed enhanced total LDH activity. The ratio of LDH isoenzyme activities was further investigated using gel electrophoresis (Hydragel, Sebia) with densitometric evaluation. Calculated act ivity values of these isoenzymes were correlated with routine laboratory parameters, the degree of lungparenchymal affection based on chest CT and clinical outcome. Results Total LDH activity was raised in the range of 272-2141 U/L and significantly correlated with calculated LDH-3 and LDH-4 activities (r=0.765, P=0.0001; and r=0.783, P=0.0001, respectively). In contrast, the relative abundance of neither LDH isoenzyme was exclusively abnormal in COVID-19 patients. Calculated activity of LDH-3 and LDH-4 demonstrated a modest but statistically significant association with serum ferritin (r=0.437, P=0.042; r=0.505, P=0.016, respectively). When the relationship between the severity of pulmonary affection by SARS-CoV-2 infection and relative abundance of LDH isoenzymes was studied, a larger ratio of mid-zone fractions was observed in the presence of ? 50% lung parenchymal involvement. Finally, regardless of LDH isoenzyme pattern, abnormal relat ive ratio of LDH-4 and higher calculated LDH-3 and LDH-4 activity values were detected in subjects with unfavorable outcome. Conclusion No characteristic profile of LDH isoenzymes can be detected in COVID-19 pneumonia, however, elevated activities of LDH-3 and LDH-4 are associated with worse clinical outcomes.

Megjegyzések:Since a large proportion of medical decisions arebased on laboratory results, clinical laboratoriesshould meet the increasing demand of clinicians andtheir patients. Huge central laboratories may processover 10 million tests annually; they act as productionfactories, measuring emergency and routinetests with sufficient speed and accuracy. At the sametime, they also serve as specialized diagnostic centerswhere well-trained experts analyze and interpretspecial test results. It is essential to improve andconstantly monitor this complex laboratory service,by several methods. Sample transport by pneumatictube system, use of an advanced laboratory informationsystem and point-of-care testing may result indecreased total turnaround time. The optimizationof test ordering may result in a faster and more costeffectivelaboratory service. Autovalidation can savetime for laboratory specialists, when the analysis ofmore complex results requires their attention. Smallteams of experts responsible for special diagnosticwork, and their interpretative reporting according topredetermined principles, may help to minimize subjectivityof these special reports. Although laboratoryinvestigations have become so diversely developed in the past decades, it is essential that the laboratorycan provide accurate results relativelyquickly, and that laboratory specialists can supportthe diagnosis and monitoring of patientsby adequate interpretation of esoteric laboratorymethods.

Megjegyzések:Multidrug resistance (MDR) is an unwanted phenomenon, that may cause therapy failure in several neoplasms includinghematological malignancies. The purpose of any type of laboratory MDR assay is to reliably identify such patients and to provideuseful data to clinicians with a relatively short turnaround time. MDR can be multicausal and several previous data identified agroup of transmembrane proteins - the ATP-binding casette (ABC) proteins - that may be involved in MDR in varioushematological malignancies. The prototype of these proteins is the P-glycoprotein (Pgp, MDR1, ABCB1) that is a seven-membranespanning transmembrane protein capable of extruding several cytotoxic drugs that are of key importance in the treatmentof hematological disorders. Similarly other ABC proteins ? Multidrug resistance associated protein 1 (ABCC1) and breastcancer resistance protein (ABCG2) are both capable of pumping out cytotoxic drugs. Here, we present flow cytometric methodsto identify MDR proteins by antigen and activity assays. The advantage of flow technology is the short turnaround time and itsrelative easiness compared to nucleic acid based technologies. However, for the activity assays, it should be noted, that thesefunctional tests require live cells, thus adequate results can only be provided if the specimen transport can be completed within6 hours of sample collection. Identification of MDR proteins provides prognostic information and may modulate therapy, thussignifies a clinically useful information in the evaluation of patients with leukemias.

Megjegyzések:Myelodysplastic syndromes are clonal hematopoietic stem cell disorders. Their exact etiology is unknown. Myelodysplasticsyndromes cause progressive bone marrow failure resulting in pancytopenia and refractory, transfusion-dependent anemia.One can observe typical morphological alterations in the erythroid, myeloid and/or megakaryocytic cell lineage. Blast countsmay also be increased. The pathologic cells are genetically unstable, and a myelodysplastic syndrome might transform into acutemyeloid leukemia. The overall survival of these diseases range between few months to around ten years. Correct diagnosis andaccurate prognostic classification is essential. In the past decades several scoring systems were established beginning with theFrench-American-British classification to the most recent Revised International Prognostic Scoring System. In all of theseclassifications bone marrow morphology is still the most important factor, though nowadays the genetic aberrations and flowcytometry findings are also included. The diagnosis and prognostic classification of myelodysplastic syndromes remain a greatchallenge for hematologists.

Megjegyzések:The role of vitamin D in calcium-phosphorus metabolism regulation is the most highlighted, nonetheless there is enormous literature on the extra-skeletal effects of vitamin D, and lately new insight into the role of vitamin D in endocrine disease mechanisms has seen light of day. The present narrative review gives an overview of the proposed roles of vitamin D in the etiology of Hashimoto's thyroiditis, Grave's disease, Addison's disease and primary hyperthyroidism. The implications as pertaining to the routine laboratory practice are readily applicable to this patient group as well, and do not pose any additional challenge.

Megjegyzések:There is a great effort to find out the biological role of cell-free nucleic acids (cfNAs). They are considered very promising targets in the diagnosis of genetic diseases. Non-invasive sampling (liquid biopsy) has recently become a very popular method, and new molecular biological techniques have been developed for these types of samples. Application of next-generation sequencing (NGS) and massively parallel sequencing (MPS) is spreading fast. These are the part of the arsenal of the modern prenatal genetic diagnostic laboratories by now. Cell-free DNA based noninvasive prenatal testing accounts for more than half of the prenatal genetic tests performed, it is gradually replacing the invasive amniocentesis or chorionic villus sample-based diagnostics. Besides that, new non-coding RNAs are taking more attention: microRNAs (miRNAs), long non-coding RNAs (lncRNAs), circular RNAs (circRNAs) are in the focus of the clinical research to detect the most common pregnancyassociated diseases, like preeclampsia, fetal growth restriction, congenital heart diseases and gestational diabetes.
