Pseudoxanthoma elasticum (PXE) is an inherited systemic disorder, characterized by dermal, ocular and cardiovascular lesions. Genetic defects of the ABCC6 (MRP6) transporter are known to cause PXE. OBJECTIVES: The purpose of this study was to identify the genetic background of a PXE patient with a very early onset of the disease and severe systemic involvement. METHODS: Direct sequencing of genomic DNA obtained from peripheral whole blood.RESULTS: Our patient was found to be compound heterozygous with both ABCC6 alleles having genomic deletions. A novel exon 24-25 deletion was identified on one allele, while the frequently observed exon 23-29 deletion was found on the other allele. The novel deletion is 4.68 kb long and was shown to extend from intron 23 to 25. DNA-sequencing of a 2.03 kb fusion fragment revealed the deletion breakpoints within introns 23 and 25 originating in the middle of two Alu-repeats. CONCLUSION: In a patient with severe clinical symptoms, we found two genomic deletions in regions that might be important for function of the ABCC6 transporter. Genomic deletions in ABCC6 may occur more frequently in PXE patients than previously expected and future genetic analysis should focus on these mutations as well.

Background: Atopic eczema (AE) is a chronic inflammatory and pruritic skin disease. There is still anunmet need for topical anti-inflammatory and anti-pruritic substances exhibiting an excellent safetyprofile. The endocannabinoid system is known to regulate various aspects of cutaneous barrier andimmune functions, thus targeting it may be a valid approach for alleviating the symptoms of AE.Objective: To assess the putative efficacy of Echinacea purpurea-derived alkylamides (Ec. extract)activating cannabinoid (CB)-2 receptors in exerting anti-inflammatory effects and alleviating symptomsof AE.Methods: In vitro anti-inflammatory efficiency was investigated by monitoring the effects of Ec. extract onpoly-(I:C)-induced pro-inflammatory cytokine expression (Q-PCR) and release (ELISA) of HaCaTkeratinocytes. Irritancy and sensitization potential (assessed by Human Repeat Insult Patch Test;Clinical trial 1); clinical efficiency in alleviating symptoms of AE (Clinical trial 2) as well as effects onhuman skin structure and lipid content (Clinical trial 3 followed by transmission electron microscopy andHPTLC) were investigated in randomized double blind clinical trials.Results: Ec. extract significantly reduced mRNA expression as well as release of poly-(I:C)-induced proinflammatorycytokines (IL-6 and IL-8) in keratinocytes. Thus, not surprisingly, the well-tolerated(Clinical trial 1) Ec. extract-based cream reduced local SCORAD statistically significantly, not onlycompared to baseline, but also compared to the comparator (Clinical trial 2). Of great importance, besidesthe in vitro anti-inflammatory effects, administration of the Ec. extract-based cream also resulted insignificantly higher levels of overall epidermal lipids, ceramide EOS (v-esterified fatty acid + sphingosinesphingoid base), and cholesterol at Day 15 compared to baseline as well as significantly greater numbersof intercellular lipid lamellae in the intercellular space (Clinical trial 3).Conclusion: The investigated Ec. extract shows great potential in alleviating cutaneous symptoms of AE,and by exerting remarkable anti-inflammatory actions and restoring the epidermal lipid barrier, it will bevery likely a well-tolerated, powerful novel ingredient for the adjuvant therapy of AE.
