Megjegyzések:Transient receptor potential canonical-6 (TRPC6) ion channels, expressed at high levels in podocytes of the filtration barrier, are recently implicated in the pathogenesis of various forms of proteinuric kidney diseases. Indeed, inherited or acquired up-regulation of TRPC6 activities are suggested to play a role in podocytopathies. Yet, we possess limited information about the regulation of TRPC6 in human podocytes. Therefore, in this study, we aimed at defining how the protein kinase C (PKC) system, one of the key intracellular signalling pathways, regulates TRPC6 function and expression. On human differentiated podocytes, we identified the molecular expressions of both TRPC6 and several PKC isoforms. We also showed that TRPC6 channels are functional since the TRPC6 activator 1-oleoyl-2-acetyl-sn-glycerol (OAG) induced Ca2+-influx to the cells. By assessing the regulatory roles of the PKCs, we found that inhibitors of the endogenous activities of classical and novel PKC isoforms markedly augmented TRPC6 activities. In contrast, activation of the PKC system by phorbol 12-myristate 13-acetate (PMA) exerted inhibitory actions on TRPC6 and suppressed its expression. Importantly, PMA treatment markedly down-regulated the expression levels of PKCa, PKCb, and PKCg reflecting their activation. Taken together, these results indicate that the PKC system exhibits a 'tonic' inhibition on TRPC6 activity in human podocytes suggesting that pathological conditions altering the expression and/or activation patterns of podocyte-expressed PKCs may influence TRPC6 activity and hence podocyte functions. Therefore, it is proposed that targeted manipulation of certain PKC isoforms might be beneficial in certain proteinuric kidney diseases with altered TRPC6 functions.

Megjegyzések:Lesion detectability in PET investigations is limited by a number of physical and scanning constraints. The aim of this work is to characterize the minimum detectable activity level for the Gemini TF PET scanner varying the parameters such as scan duration, injected activity, target-to-background ratio and object size. Materials & methods The Gemini TF PET scanner has no septa allowing 3D only acquisition and based on the LYSO scintillator. The system timing resolution is about 600 ps. We used the International Electrotechnical Commission (IEC) body phantom set with the six fillable spheres with inner diameters of 10,13,17,22,28 and 37 mm. The four smaller spheres were filled with F18 activity that the target to background (T/B) ratio were 16, 8, 4 and 2, respectively. The cavity of the IEC phantom was filled with F18 water of 12, 4.9 and 2.1 kBq/ml activity concentration for three different acquisition set. The duration of the PET scans were 10 min at all case. In the Gemini TF the data are acquired in list-mode enabling to the user to define any frame length for the image reconstruction. We set the frame length to 1, 3, 6 and 9 min. The images were reconstructed with the manufacturer supplied 3D listmode iterative algorithm with and without TOF kernel. ROIs were drawn on the spheres of the central slices and the contrast recovery coefficients (CRC) and the contrast to noise ratio (CNR) parameters were calculated for any reconstructed images. Results With T/B ratios of 4, 8, 16 the 13, 17, 22 mm spheres were detectable for any frame length and background concentration activity level. When we set the T/B ratio to 2 the number of detectable spheres were depend on the scan duration and background activity, respectively. At the lowest background activity (2.1 kBq/ml) only the 22 mm sphere could be realized for scan length of at least 3 min. The 13 and17 mm spheres were identified at the used highest background activity (12 kBq/ml) and at least 3 min scan duration. In this background level the 22 mm sphere was easily detectable for any scan duration. We also found that the CNR less depends on the total injected activity than scan duration and the weight between them is sensitive to the applied reconstruction kernel (TOF or non-TOF). Conclusion It is possible to visualize sources with T/B ratio of 2 in the case of Gemini TF PET scanner.

Megjegyzések:Aim/Introduction: Graves' ophtalmopathy (GO) in immunologically active or incative state requires different treatments. We have examined more than 1000 patients' retrobulbar uptake with [Tc-99m]DTPA in the last 15 years, evaluated by a custom software using manual ROIs on a standardized thick slice containing the retrobulbar space. The clinicians mainly used this method for differentiating active from non-active ophtalmopathy, with 10x10-6 ID/mL as threshold. We investigated the potential of quantitative SPECT/ CT by more available software and more reproducible VOI positioning to simplify the method. Materials and Methods: SPECT projection images of twenty-six retrobulbar regions of 13 patients with suspected active GO were acquired 20 min after administering 400 MBq [Tc-99m]DTPA (128 projections x 30 sec, 128x128 matrix with ... mm pixel size); followed by a CT (120 kV, 40 mAs, 1.5 pitch, 90/min rotation with abdomen filter). OSEM iterative reconstruction included CT-based attenuation correction, Monte Carlo-based scatter correction, and resolution recovery. We drew three sets of VOIs, utilizing both SPECT and CT information. Method A: 2 cm diameter sphere to the retrobulbar area showing the highest uptake; method B: a cone covering the whole retrobulbar territory; and Method C: an 1 mL standard sphere to the highest activity in the retrobulbar area. The uptake was expressed as (a) activity concentration, (b) total VOI activity, or (c) maximal concentration; each normalized to the injected dose. The same parameters were tested when normalized to body weight as well. For each quantitative measure Pearson's r, and Kendall's tau-b correlation coefficients were calculated with the former method as reference. Results: Total and maximal VOI uptake values as well as measures corrected for body weight showed weaker correlation with the reference method than mean activity concentrations. We obtained better correlation with the larger sphere (2 cm vs. 1.2 cm diameter), and even slightly better with the conical VOI. Conclusion: We found that defining the retrobulbar area is much easier when utilizing the CT image as well. For the sake of simplicity and reproducibility, we select the sphere with 2 cm diameter as the VOI of choice, since drawing the conical region is more subjective. As the present analysis aimed at the best correlation with the reference method that did not take body size into account, we plan to consider activity concentration normalized to body weight as well during the forthcoming clinical evaluation based on a larger patient group. Megjegyzések:Vascular calcification is a frequent complication of atherosclerosis, diabetes and chronic kidney disease. In the latter group of patients, calcification is commonly seen in tunica media where smooth muscle cells (SMC) undergo osteoblastic transformation. Risk factors such as elevated phosphorus levels and vitamin D3 analogues have been identified. In the light of earlier observations by our group and others, we sought to inhibit SMC calcification via induction of ferritin. Human aortic SMC were cultured using b-glycerophosphate with activated vitamin D3, or inorganic phosphate with calcium, and induction of alkaline phosphatase (ALP) and osteocalcin as well as accumulation of calcium were used to monitor osteoblastic transformation. In addition, to examine the role of vitamin D3 analogues, plasma samples from patients on haemodialysis who had received calcitriol or paricalcitol were tested for their tendency to induce calcification of SMC. Addition of exogenous ferritin mitigates the transformation of SMC into osteoblast-like cells. Importantly, pharmacological induction of heavy chain ferritin by 3H-1,2-Dithiole-3-thione was able to inhibit the SMC transition into osteoblast-like cells and calcification of extracellular matrix. Plasma samples collected from patients after the administration of activated vitamin D3 caused significantly increased ALP activity in SMC compared to the samples drawn prior to activated vitamin D3 and here, again induction of ferritin diminished the osteoblastic transformation. Our data suggests that pharmacological induction of ferritin prevents osteoblastic transformation of SMC. Hence, utilization of such agents that will cause enhanced ferritin synthesis may have important clinical applications in prevention of vascular calcification.

Megjegyzések:Angiogenesis, the formation of new vessels, is important in the pathogenesis of rheumatoid arthritis (RA) and other inflammatory diseases. Chemotactic cytokines termed chemokines mediate the ingress of leukocytes, including neutrophils and monocytes into the inflamed synovium. In this review, authors discuss the role of the most important angiogenic factors and angiogenesis inhibitors, as well as relevant chemokines and chemokine receptors involved in chronic inflammatory rheumatic diseases. RA was chosen as a prototype to discuss these issues, as the majority of studies on the role of angiogenesis and chemokines in inflammatory diseases were carried out in arthritis. However, other systemic inflammatory (autoimmune) diseases including systemic lupus erythematosus (SLE), systemic sclerosis (SSc), Sjogren's syndrome (SS), mixed connective tissue disease (MCTD), polymyositis/dermatomyositis (PM/DM) and systemic vasculites are also discussed in this context. As a number of chemokines may also play a role in neovascularization, this issue is also described here. Apart from discussing the pathogenic role of angiogenesis and chemokines, authors also review the regulation of angiogenesis and chemokine production by other inflammatory mediators, as well as the important relevance of neovascularization and chemokines for antirheumatic intervention.
