Leukoaraiosis refers to an age-related, abnormal appearance of the brain white matter on neuroimaging. The association between leukoaraiosis and cerebrovascular disease suggests that ischemia may be an important contributing factor; however, the pathogenesis of the condition remains controversial. We hypothesized that physical abnormalities of blood vessels might be culpable and compared the external and internal measurements of blood vessel walls between brains that demonstrated leukoaraiosis on imaging and normal control brains. Fourteen brains of individuals who had been diagnosed as having severe leukoaraiosis and five non-leukoaraiosis control brains were studied. Arterial cross-sections were evaluated by length measurements with an image analysis device. Arterial wall thickness and the ratio of the outer and inner diameters of the vessel were measured. We measured a total of 108 vessels in the leukoaraiosis group and 95 vessels in the control group. The vessel walls of the leukoaraiosis patients were an average of 5.5 mikrom thicker than the walls of control vessels of the same inside diameter (P = 0.0000, 95% CI 3.01-8.08) and an average of 2.3 mikrom thicker than walls of control vessels of the same outside diameter (P = 0.016, 95% CI 0.48-4.17). Our data provide evidence that leukoaraiosis is associated with vessel wall thickening in an additive fashion and indicate that structural vascular abnormalities are associated with leukoaraiosis.

Dementia is one of the major burdens of our aging society. According to certain predictions, the number of patients will double every 20 years. Although Alzheimer's disease (AD), as the most frequent neurodegenera-tive dementia, has been extensively analysed, less is known about dementia with Lewy bodies (DLB). Neuropa-thological hallmarks of DLB are the deposition of intracellular Lewy bodies (LB) and Lewy neurites (LN). DLB belongs to the ?-synucleinopathies, as the major component of these inclusions is pathologically aggregated ?-synuclein. Depending on the localizationof LBs and LNs in the central nervous system cognitive and motor symptoms can occur. In our work, we will systematically review the possible etiology and epidemiology, patho-logical (both macroscopic and microscopic) features, structural and functional imaging findings, with a special emphasis on the clinico-pathological correlations. Finally, we summarize the latest clinical symptoms-based diagnostic criteria and the novel therapeutic approaches. Since DLB is frequently accompanied with AD pathol-ogy, highlighting possible differential diagnostic approaches is an integral part of our paper. Although our pre-sent knowledge is insufficient, the rapid development of diagnostic and research methodsprovide hope for better diagnosis and more efficient treatment, contributing to a better quality of life.

We describe an atypical neuropathological phenotype ofsporadic Creutzfeldt-Jakob disease in a 76-year-old man.The clinical symptoms were characterized by progressivedementia, gait ataxia, rigidity and urinary incontinence.Thedisease duration was 6 weeks.MRI did not show prominentatrophy or hyperintensities in cortical areas, striatum orthalamus.Biomarker examination of the cerebrospinal fluiddeviated from that seen in pure Alzheimer's disease.Triphasic waves in the EEG were detected only later in thedisease course, while 14-3-3 assay was positive. PRNPgenotyping revealed methionine homozygosity (MM) atcodon 129. Neuropathology showed classical CJD changescorresponding to the MM type 1 cases. However, a strikingfeature was the presence of abundant kuru-type plaques inthe white matter. This rare morphology was associated withneuropathological signs of intranuclear inclusion bodydisease and advanced stage of argyrophilic grain disease.These alterations did not show correlation with each other,thus seemed to develop independently. This case furtherhighlights the complexity of neuropathological alterationsin the ageing brain.

Aging-related tau astrogliopathy (ARTAG) is a recently introduced terminology. To facilitate the consistent identification of ARTAG and to distinguish ARTAG from other main types of astroglial tau pathologies related to primary frontotemporal lobar degeneration-related tauopathies we evaluated how consistently neuropathologists recognize i) different astroglial tau immunoreactivities including those of ARTAG and those associated with primary tauopathies (study 1); ii) ARTAG subtypes (study 2A); and iii) the severity of ARTAG (study 2B). Microphotographs and scanned sections immunostained for AT8 tau were made available for download and preview. Percentage (%) of agreement and kappa values with 95% confidence interval (CI) were calculated for each evaluation. The overall agreement for study 1 was > 60% with a kappa value of 0.54 (95% CI 0.433-0.645). Moderate agreement (> 90%, kappa: 0.48, 95% CI 0.457-0.900) was reached in study 2A for the identification of ARTAG pathology for each ARTAG subtype (kappa: 0.37-0.71), whereas fair agreement (kappa: 0.39, 95% CI 0.341-0.445) was reached for the evaluation of ARTAG severity. The overall assessment of ARTAG showed moderate agreement (kappa: 0.59, 95% CI 0.534-0.653) between raters. Our study supports the application of the current harmonized evaluation strategy of ARTAG with a slight modification of the evaluation of its severity.

Aims: Frontotemporal lobar degeneration with Pick bodies (Pick's disease) is characterized by the presence of tau immunoreactive spherical structures in the cytoplasm of neurons. In view of confusion about the molecular pathology of Pick's disease, we aimed to evaluate the spectrum of tau pathology and concomitant neurodegeneration-associated protein depositions in the characteristically affected hippocampus. Methods: We evaluated immunoreactivity for tau (AT8, 3R, 4R), α-synuclein, TDP43, p62, and ubiquitin in the hippocampus, entorhinal and temporal cortex in 66 archival cases diagnosed neuropathologically as Pick's disease. Results: Mean age at death was 68.2 years (range 49 to 96). Fifty-two (79%) brains showed 3R immunoreactive spherical inclusions in the granule cells of the dentate gyrus. These typical cases presented mainly with the behavioural variant of FTD, followed by progressive aphasia, mixed syndromes or early memory disturbance. α-Synuclein immunoreactivity was seen only in occasional spherical tau-positive inclusions, TDP-43 IR was absent, and 4R IR was present only as neurofibrillary tangles in pyramidal neurons. Aβ immunoreactivity was observed in 16 cases; however, the overall level of Alzheimer's disease-related alterations was mainly low or intermediate (n = 3). Furthermore, we identified six cases with unclassifiable tauopathy. Conclusions: 1) Pick's disease may occur also in elderly patients and is characterized by a relatively uniform pathology with 3R tau inclusions particularly in the granule cells of dentate gyrus; 2) even minor deviation from these morphological criteria suggests a different disorder; and 3) immunohistological revision of archival cases expands the spectrum of tauopathies that require further classification. © 2012 The Authors.
