Megjegyzések:Seminal vesicle cysts can cause sub- or infertility. Minimally invasive techniques have the advantage ofpreserving the vas deferens by the treatment of symptomaticcases. After reviewing the published articles, only a few of them presented data on fertility beforeand after surgery. The authors now report the successfultreatment of two patients with seminal vesicle cysts, inwhich laparoscopic cyst removal resolved the symptoms,preserving fertility and erectile function. Due to the rarityof seminal vesicle cysts, preoperative examinationsand treatment modalities should focus not only on therelief of symptoms but also on the preservation of fertilityand erectile function.

Megjegyzések:There are no universally accepted guidelines for the treatment of recurrent IgA nephropathy. The clinical course of this condition is variable due to the fact that it can be diagnosed in asymptomatic patients on a protocol biopsy, in patients with mild hematuria or proteinuria, or in patients with a rapidly deteriorating kidney function. Our aim was to present the case of a kidney transplant patient with biopsy proven IgA nephropathy. Our findings show that glomerular filtration pressure can be successfully decreased with combined antihypertensive treatment and SGLT2 inhibitor treatment. Refill with personalized immunosuppressive regimen helped us achieve partial remission.

Megjegyzések:Objective To compare simultaneous bilateral percutaneous nephrolithotomy (SBPCNL) and unilateral PCNL in separate sessions in patients with bilateral renal stones for several variables before and after surgery.Patients and methods The results from SBPCNL carried out at two centres in different countries on 198 patients (aged 1.25?70 years) were compared with those from 300 patients undergoing unilateral PCNL in separate sessions. At one centre where extracorporeal shockwave lithotripsy (ESWL) was available SBPCNL was used for stones where at least two sessions of ESWL would have been required on each side. Other associated procedures were also used wherever required.Results The stones were cleared bilaterally in one session of SBPCNL in 190 patients; in eight, fragments of < 3 mm remained but were not clinically significant. A second session was required in six patients on one side only. Variables assessed before and after treatment (e.g. hospital stay, analgesia requirements and complications) were not significantly different between SBPCNL and PCNL. The mean (range) total operative duration for SBPCNL was 46 (20?100) min and the hospital stay 4.3 (3?8) days.Conclusion From this experience, SBPCNL is a cost-effective and beneficial solution for selected patients, with clear advantages over separate unilateral PCNL in patients with bilateral stones.

Megjegyzések:The incidence of appendix testis has been shown to be 76% in descended and 24% in undescended testis in our previous intraoperative survey. To determine the possible role of the appendix testis in the process of testicular migration,we compared the androgen and estrogen receptor status of appendix testis in descended and undescended testes. Methods: Thirty-seven appendix testes were collected intraoperatively and the expression of androgen and estrogen receptors were examined with immunostaining and immunofluorescence labeling. Based on the diagnosis, the specimens were divided into three groups. Group H (groin hernia, n = 11, as a group of descended testis), Group AU (acquired undescended testis, n = 14), and Group CU (congenital undescended testis, n = 12). Results: The testicular appendages were found to express both androgen and estrogen receptors in Group H and Group AU, but specimens in Group CU were only estrogen receptor positive, whereas androgen receptors were not present. Conclusion: The presence of the androgen receptor in the appendix testis of the descended testes and acquired undescended testes and its absence in patients with congenital undescended testis suggests that the appendix testis might play a role in the process of testicular descent.

Megjegyzések:Purpose: We introduce a new modified penile skin reconstruction technique to treat paraffin-induced sclerosing lipogranuloma of the penis. Materials and Methods: From 2017 to 2020, 49 patients underwent the procedure. Complete removal of the lipogranuloma-involved penile skin was performed. A subcutaneous tunnel was then created between a horizontal scrotal incision and a proximal penile circumferential incision. The denuded penis was pulled through the tunnel, and a subcoronal and longitudinal dorsal penile suture line was made. An inverted V-shaped incision was made on the scrotum on the ventral side of the penis, followed by longitudinal closure. Outcomes and complications of the procedure were retrospectively studied. The long-term effect of surgery on sexual function and overall satisfaction was measured using a patient-reported questionnaire, which was completed by 30 patients. Results: The overall complication rate was 26.5%. Clavien-Dindo grade 1, 2, 3a, 3b, 4, and 5 complications occurred in the postoperative period 5, 0, 8, 1, 0, and 0 times, respectively, in 13 patients. Surgery was successful in 27 (90%) patients,according to the patient-reported questionnaire. Erectile dysfunction, pain/tension during erection, premature ejaculation, and penile lymphoedema were observed in 2, 3, 1, and 1 patients, respectively. All patients reported sexual intercourse ability. Conclusions: The type of penile skin reconstruction after the removal of sclerosing lipogranuloma of the penis is controversial. The reconstruction technique presented herein is an effective single-stage treatment option with a high success rate in patients with sclerosing lipogranuloma of the penis with intact scrotal skin.
