Megjegyzések:Background: The social representation of restricted health care use during the COVID-19 pandemic has not been evaluated properly yet in Hungary. Objective: Our study aimed to quantify the effect of COVID-19 pandemic measures on general practitioner (GP) visits, specialist care, hospitalization, and cost-related prescription nonredemption (CRPNR) among adults, and to identify the social strata susceptible to the pandemic effect. Methods: This cross-sectional study was based on nationally representative data of 6611 (Nprepandemic = 5603 and Npandemic = 1008) adults. Multivariable logistic regression models were applied to determine the sociodemographic and clinical factors influencing health care use by odds ratios (ORs) along with the corresponding 95% confidence intervals (CI). To identify the social strata susceptible to the pandemic effect, the interaction of the time of data collection with the level of education, marital status, and Roma ethnicity, was tested and described by iORs. Results: While the CRPNR did not change, the frequency of GP visits, specialist care, and hospitalization rates was remarkably reduced by 22.2%, 26.4%, and 6.7%, respectively, during the pandemic. Roma proved to be not specifically affected by the pandemic in any studied aspect, and the pandemic restructuring of health care impacted the social subgroups evenly with respect to hospital care. However, the pandemic effect was weaker among primary educated adults (iORGP visits, high-school vs. primary-education = 0.434; 95% CI 0.243?0.776, ORspecialist visit, high-school vs. primary-education = 0.598; 95% CI 0.364?0.985), and stronger among married adults (iORGP visit, widowed vs. married = 2.284; 95% CI 1.043?4.998, iORspecialist visit, widowed vs. married = 1.915; 95% CI 1.157?3.168), on the frequency of GP visits and specialist visits. The prepandemic CRPNR inequality by the level of education was increased (iORhigh-school vs. primary-education = 0.236; 95% CI 0.075?0.743). Conclusion: Primary educated and widowed adults did not follow the general trend, and their prepandemic health care use was not reduced during the pandemic. This shows that although the management of pandemic health care use restrictions was implemented by not increasing social inequity, the drug availability for primary educated individuals could require more support.

Megjegyzések:The COVID-19 pandemic had a considerable impact on the whole health sector, particularly on emergency services. Our aim was to examine the performance of the Hungarian National Ambulance Service during the first four waves of the pandemic. We defined the 2019 performance of the service as the baseline and compared it with the activity during the pandemic years of 2020 and 2021. The data contained deliveries related to acute myocardial infarction, hemorrhagic stroke, ischemic stroke, overall non-COVID-related ambulance deliveries, COVID screenings performed by the ambulance service, and COVID-related ambulance deliveries. The data were aggregated for each week of the investigated time period and stratified by gender and age. Compared with the pre-pandemic era, we found a significant increase in all three medical conditions and overall deliveries (p < 0.001 in all cases). As a result of the increased burden, it is important for emergency services to prepare for the next global epidemic and to improve organizational performance and rescue activities. The Hungarian example highlights that in a pandemic, it can be beneficial to organize the emergency care of a country or a larger region under a single provider with a single decision maker supported by business intelligence.

Megjegyzések:Communication is one of the most important aspects not only in the healthcare services but in every field of hu man interactions. In all societies, in order to get an efficient and effective healthcare delivery, good communica tion plays an essential role especially between patients and physicians. This collaborative interaction not only im proves health outcomes, but also the patients' and health professionals' satisfaction. Given the importance of com municative health literacy, the field has become more important for the public health perspective especially in health promotion. Our aim is to introduce the communicative health literacy questionnaire based on the results of the Hungarian survey. Regarding the data collection a total of 1206 adults aged 18 years and older completed the telephone interview below the umbrella of the Health Literacy Survey 2019. The questionnaire included items among others on sociodemographic data and communicative health literacy. Statistical weights were applied to ensure that estimates reflect the general adult Hungarian population by gender, age group, educational level, settle ment type. The mean score of the communicative health literacy scale was 86.8, nearly one eighth of the respon dents had difficulties during communication in health care services. The value of Cronbach's alpha was 0.87. When calculating this measure for the different sociodemographic layers of the population, the Cronbach's alpha was at least 0.85 in all groups. The easiest task was to explain health concerns, while getting enough time in the consul tation was marked as the most difficult one. The questionnaire can be characterized with good internal consisten cy. Perhaps getting enough time in the consultation is only one among many difficulties faced by people in the health care services. The instrument has proven to map communicative health literacy in research on a population level. In consultations with patients physicians should set aside more time to be able to provide information.

Megjegyzések:Orbital connective tissue expansion is a hallmark of Graves' orbitopathy (GO). In moderateto-severe active GO, glucocorticoids (GC) are the first line of treatment. Here we show that hydrocortisone (HC), prednisolone (P), methylprednisolone (MP), and dexamethasone (DEX) inhibit the hyaluronan (HA) production of orbital (OF) and dermal (DF) fibroblasts. HA production of GO OFs (n = 4), NON-GO OFs (n = 4) and DFs (n = 4) was measured by ELISA. mRNA expression of enzymes of HA metabolism and fibroblast proliferation was examined by RT-PCR and BrdU incorporation, respectively. After 24 h of GC treatment (1?M) HA production decreased by an average of 67.9 ? 3.11% (p < 0.0001) in all cell cultures. HAS2, HAS3 and HYAL1 expression in OFs also decreased (p = 0.009, p = 0.0005 and p = 0.015, respectively). Ten ng/mL PDGF-BB increased HA production and fibroblast proliferation in all cell lines (p < 0.0001); GC treatment remained effective and reduced HA production under PDGF-BB-stimulated conditions (p < 0.0001). MP and DEX reduced (p < 0.001, p = 0.002, respectively) PDGF-BB-induced HAS2 expression in OFs. MP and DEX treatment decreased PDGF-BB stimulated HAS3 expression (p = 0.035 and p = 0.029, respectively). None of the GCs tested reduced the PDGF-BB stimulated proliferation rate. Our results confirm that GCs directly reduce the HA production of OFs, which may contribute to the beneficial effect of GCs in GO.
