Megjegyzések:Candidiasis is common in children with cancer, particularly during periods of severe immunosuppression and neutropenia. Our aim was to study the microbiological changes in the oral cavity of children with newly diagnosed cancer. The study group consisted of 30 consecutive children and adolescents, 16 with acute lymphoblastic leukemia and 14 with solid tumors. Oral cultures to detect fungi and bacteria were conducted for all patients before treatment, during and after neutropenic episodes. In 23 patients developing fever simultaneous throat, urine and blood sampling was carried out. No pathogens were found in the cultures taken before the outset (30 cultures) or after recovery from (30 cultures) the neutropenic episodes. In the 45 oral cultures taken during the neutropenic episodes 38 (84.4%) proved positive. Fungi were the most frequently isolated oral pathogens: 33/38 yeast and 6/38 bacterial infections were identified. There was no association between the underlying malignancy and the occurrence of the positive cultures. Of the 30 patients, all 23 (76.7%) who have developed moderate-to-severe neutropenia, developed oral fungal colonization or clinically obvious fungal infection at least on one occasion during the study. In addition to oral samples, fungi were identified in 9/23 pharyngeal swabs, 6/23 urine and 1/23 blood cultures. The initial fungal pathogen was exclusively (33/33) Candida albicans. In extended severe neutropenic states, C. albicans was replaced by non-albicans species (C. kefyr, C. lusitaniae, C. sake, C. tropicalis) in 5 patients between 4 to 6 days of the neutropenic episodes. Four of the nonalbicans Candida strains were resistant to azole-type antifungal agents. Neutropenic episodes of children with cancer are associated with an increased risk of developing oral and even systemic infections with C. albicans that can be replaced by azole-resistant nonalbicans strains in prolonged neutropenia contributing to morbidity of these patients.

Megjegyzések:Oral squamous cell carcinoma (OSCC) is the 7th most common malignancy and the 9th most frequent site of cancer death in Europe. Hungary occupies top places of statistics regarding OSCC incidence and mortality. Therefore, there is a high need to improve early detection. Saliva, as a readily available body fluid, became an increasingly important substance for the detection of biomarkers. Different research groups identified salivary biomarkers specific for different countries. In this study saliva samples of Hungarian OSCC patients were studiedin order to discover disease- and perhaps region-specific biomarkers. LC/MS/MS analysis on linear ion trap-Orbitrap mass spectrometer was used for qualitative and quantitative salivary protein profiling. More than 500 proteins were identified from saliva by shotgun proteomics. The up- and downregulated proteins in the saliva of patients with OSCC highlighted the importance of protein-protein interaction networks involving the immune system and proteolysis in disease development. Two potential biomarkers from our shotgun analysis and a third candidate reported earlier by a Taiwanese group were further examined by ELISA on a larger reference set of samples. Resistin reported as a biomarker in Taiwan but not validated in our study highlights the necessity of application of standardized analysis methods in different ethnic or geographical populations in order to identify biomarkers with sufficient specificity and sensitivity.

Megjegyzések:Oral squamous cell carcinoma (OSCC) accounting for about 90% of malignant oral lesionsis the 6th most common malignancy worldwide. Diagnostic delay may contribute to dismalsurvival rate therefore, there is a need for developing specific and sensitive biomarkers toimprove early detection. Hungarian population occupies the top places of statistics regardingOSCC incidence and mortality figures therefore, we aimed at finding potential salivaryprotein biomarkers suitable for the Hungarian population. In this study we investigated 14proteins which were previously reported as significantly elevated in saliva of patients withOSCC. In case of IL-1?, IL-1?, IL-6, IL-8, TNF-? and VEGF a Luminex-based multiplex kitwas utilized and the salivary concentrations were determined. In case of catalase, profilin-1,S100A9, CD59, galectin-3-bindig protein, CD44, thioredoxin and keratin-19, SRM-basedtargeted proteomic method was developed and the relative amount of the proteins wasdetermined in the saliva of patients with OSCC and controls. After several rounds of optimizationand using stable isotope-containing peptides, we developed an SRM-based methodfor rapid salivary protein detection. The validation of the selected potential biomarkers byELISA revealed salivary protein S100A9 and IL-6 as useful protein biomarkers for OSCCdetection improving the diagnostic accuracy for OSCC in the Hungarian population.A noninvasivediagnostic method to detect biomarkers useful for the early diagnosis of OSCC wasdeveloped. This can be an attractive strategy in screening saliva samples collected in anation-wide multi-centric study in order to decrease morbidity, mortality, to enhance survivalrate and to improve quality of life. The heterogeneity of protein biomarkers found in differentethnic groups presented in the literature highlights the importance of identification of population-tailored protein biomarkers.

Megjegyzések:Oral squamous cell carcinoma (OSCC) has a dismal 50% five-year survival rate, emphasizing the need to develop reliable and sensitive tools for early diagnosis. In this study we evaluated the performance of 7 previously identified, potential mRNA biomarkers of OSCC in saliva samples of Hungarian patients. Expression of the putative OSCC biomarkers (DUSP1, OAZ1, H3F3A, IL1B, IL8, SAT and S100P), 2 biomarkers of inflammation (IL6 and TNF) and 8 putative normalizing genes was quantified from each sample using real-time quantitative PCR. In contrast with previous studies, the expression pattern of the 7 mRNA biomarkers was similar between OSCC patients and age-matched control patients in the Hungarian patient population. On the other hand, 5 of the 7 mRNA biomarkers were present at significantly higher levels in saliva samples of OSCC patients when compared to young control patients. The best biomarker combination could distinguish only the OSCC vs. young control patients, but not the OSCC vs. age-matched control patients. In conclusion, the significant differences between our results and previous studies, and the clinical characteristics of the patients suggest that inflammatory processes in the oral cavity may affect the performance of the 7 putative salivary mRNA biomarkers. Lastly, since IL6 mRNA was quantifiable in the majority of OSCC cases, but only in a few control samples, salivary IL6 mRNA may be utilized as part of a biomarker combination to detect OSCC.

Megjegyzések:Bevezetés, célkitűzésA szájüregi rosszindulatú daganatok 90%-át képező szájüregi laphámrák a hatodik leggyakoribb rákféleség, igen alacsony, alig 50%-ot elérő 5 éves túléléssel. A magas halálozási arány oka legtöbbször a késői felismerés, amely szükségessé teszi a korai diagnózist lehetővé tevő módszerek, köztük nyál biomarker kimutatások alkalmazását. A szájüregi nyálkahártyával folyamatosan érintkezik a nyál, ami egy jó indikátorközeget eredményez, hiszen reprezentálja az egész szájüreget és nem csak egy specifikus régiót. Korábbi kutatások alapján 7 mRNS biomarkert azonosítottak, melyek a szájüregi laphámrákban szenvedő, illetve kontroll páciensek között szignifikáns kvantitatív különbséget mutattak. A projekt során először a korábban azonosított mRNS-alapú biomarkerek expressziós szintjét mértük le tumoros és kontroll páciensekben, nyálmintákból izolált RNS-ből, valósidejű kvantitatív PCR-ral. A projekt második fázisában OSCC specifikus biomarkerek azonosítását fogjuk elvégezni nyálból izolált miRNS-en szintén valósidejű kvantitatív PCR technikával a QuantStudio? 12K Flex platformon. 2. Anyag és módszertan2.1. Biobank felállításaA nyálminták gyűjtésében 4 klinika vesz részt (Budapest, Debrecen, Pécs, Szeged). Klinikánként 30-30 lichenes és leukoplákiás pácienst vontunk be a mintagyűjtésbe - ezek tumor-megelőző állapotoknak számítanak -, ezen kívül 30-30 kontroll ill. szájüregi laphámrákos pácienst is bevontunk. Összesen közel 500 páciens mintáit archiváljuk és dolgozzuk fel. A nyálmintákból elsősorban szabad (sejten kívüli) RNS-t izolálunk, illetve a minták egy részén proteomikai analízist is végzünk. A projektben eddig közel 300 nyálmintát gyűjtöttünk, melyek egy részéből már mRNS-t és miRNS-t is izoláltunk.2.2. mRNS biomarkerek analízise és azonosítása nyálmintákbanKorábbi publikációs adatok szerint az alábbi 7 mRNS szignifikánsan magasabb mennyiségben találhatók meg szájüregi laphámrákos páciensek nyálmintáiban, a kontroll páciensekhez képest: IL8, DUSP1, IL1B, OAZ1, H3F3A, SAT1, S100P. Kísérleteink során először azt vizsgáltuk, hogy a magyar betegpopuláció esetében hasonló eredményeket kapunk-e, mint a korábban vizsgált amerikai illetve szerb betegpopuláció esetében. Először 33 szájüregi laphámrákos és 30 hasonló korú kontroll páciens nyálmintájából RNS-t izoláltunk, majd elvégeztük a qPCR méréseket. A minták mind a Debreceni Egyetem Fogászati Klinikájának pácienseitől származtak. Eredményeink nem igazolták a korábbi megfigyeléseket, vagyis egyik mRNS esetében sem detektáltunk a tumoros páciensek mintáiban magasabb expressziót. Mivel a mRNS biomarkerek közül több is a szájnyálkahártya gyulladásos folyamataival hozható összefüggésbe, felmerül annak a lehetősége, hogy a hasonlóan rossz szájhigiénés állapotú tumoros és kontroll páciensek között ezek az mRNS biomarkerek nem tudnak különbséget tenni. Ezért azt is megvizsgáltuk, hogy fiatalabb, jó szájhigiénés állapotú kontroll páciensekkel összehasonlítva milyen a mRNS biomarkerek relatív expressziója. 29 jó szájhigiéniás állapotú páciens nyálmintájából RNS-t izoláltunk, majd a qPCR-n való validálásukat is elvégeztük a korábban leírt mRNS biomarkerekre nézve. 2.3. miRNS expressziós profil, és potenciális miRNS biomarkerek azonosítása nyálmintákbanEbben a fázisban OSCC specifikus biomarkerek azonosítását fogjuk elvégezni nyálból izolált miRNS-en szintén valósidejű kvantitatív PCR technikával a QuantStudio? 12K Flex platformon. Ehhez OSCC specifikusan megváltozott (tumorspeficikus), főként túlszabályozott miRNS-ek keresése szükséges a szakirodalomból. Ezen miRNS megtalálása még jelenleg is folyamatban van. 3. Eredmények, összegzésA tumoros páciensek mintáit a fiatal, szájüregi kórformáktól mentes személyektől nyert nyálmintákkal összehasonlítva, a hét közül 6 mRNS expressziója szignifikánsan magasabb volt a tumoros páciensek mintáiban. Mivel az idős kontroll páciensek két éves nyomonkövetés után is tumormentesek voltak, eredményeink azt jelentik, hogy a 7 génes biomarker panel magyar páciensekben nem alkalmas a tumoros állapot azonosítására.A biomarker panel felhasználhatóságát valószínűleg a magyar lakosság rossz szájhigiénés viszonyai és az ezekből kialakuló gyulladásos folyamatok korlátozzák.

Megjegyzések:Background: Aspects of oral health related quality of life (OHRQoL) attracted an increased attention recently.Objective: The aim of the study was to assess self-reported oral health related quality of life (OHRQoL) amongpatients requiring prosthetic rehabilitation and to determine the rate of improvement 1 month and 6?12 monthsafter therapy. In addition, effect of age, gender, oral health indicators and denture types before treatment wereassessed on OHRQoL as evaluated and reported by the patients.Methods: Hungarian version of OHIP-49 (OHIP-49-H) questionnaire was completed before oral rehabilitation (T0-phase)by 389 patients undergoing prosthetic replacement. After 1 month (T1-phase) and 6?12 months (T2-phase) recallperiods 235 and 92 patients completed the questionnaire. The median interquartile range (IQR) values of the totalOHIP-49-H score were calculated for T0-, T1- and T2-phases. Reliability of the questionnaire was checked by Cronbach'sstatistics. Age, gender, oral health indicators and denture types of patients before and after treatment were recordedand treatment-associated changes in OHRQoL were evaluated.Results: The study demonstrated the excellent reliability and internal consistency of OHIP-49-H by a high andnarrow range of Cronbach's alpha value (0.81-0.93). A median OHIP-49-H score of 52; IQR = 25-83 demonstrated apoor OHRQoL on first admission. Decreasing median total OHIP-49-H scores 1 month (24; IQR = 9-51; p < 0.001)and 6?12 months (20; IQR = 7-37; p = 0,055) after therapy indicated an improvement of OHRQoL. Patients' ageand CPI value assessed before treatment proved to be significant factors of OHRQoL.Conclusions: Here we presented representative data about self-assessed OHRQoL of patients requiring prosthetictreatment from Hungary using OHIP-49-H questionnaire. The results demonstrated that the restoration of oralhealth was associated with an improvement in patients' OHRQoL. According to the demographical and T0 phaseclinical status, the treatment was more effective in the respect of OHIP-49-H score improvement among females(than among males), among younger (than among more aged), and among patients with more serious CPIassessed at T0. The type of prosthetic interventions did not exert a significant effect on total OHIP-49-H score,suggesting that the improvement in OHRQoL is independent from the type of denture applied.

Despite the genetic and environmental risk factors, OSCC is also associated with microbial infections and/or dysbiosis. The secreted saliva serves as the chemical barrier of the oral cavity and, since OSCC can alter the protein composition of saliva, our aim was to analyze the effect of OSCC on the salivary chemical barrier proteins. Publicly available datasets regarding the analysis of salivary proteins from patients with OSCC and controls were collected and examined in order to identify differentially expressed chemical barrier proteins. Network analysis and gene ontology (GO) classification of the differentially expressed chemical barrier proteins were performed as well. One hundred and twenty-seven proteins showing different expression pattern between the OSCC and control groups were found. Protein?protein interaction networks of up- and down-regulated proteins were constructed and analyzed. The main hub proteins (IL-6, IL-1B, IL-8, TNF, APOA1, APOA2, APOB, APOC3, APOE, and HP) were identified and the enriched GO terms were examined. Our study highlighted the importance of the chemical barrier of saliva in the development of OSCC.

Megjegyzések:Salivary IL-6 mRNA was previously identified as a promising biomarker of oral squamous cell carcinoma (OSCC). We performed a multi-center investigation covering all geographic areas of Hungary. Saliva from 95 patients with OSCC and 80 controls, all Caucasian, were collected together with demographic and clinicopathological data. Salivary IL-6 mRNA was quantified by real-time quantitative PCR. Salivary IL-6 protein concentration was measured by enzyme-linked immune-sorbent assay. IL-6 protein expression in tumor samples was investigated by immunohistochemistry. Normalized salivary IL-6 mRNA expression values were significantly higher (p < 0.001) in patients with OSCC (mean ? SE: 3.301 ? 0.885) vs. controls (mean ? SE: 0.037 ? 0.012). Differences remained significant regardless of tumor stage and grade. AUC of the ROC curve was 0.9379 (p < 0.001; 95% confidence interval: 0.8973?0.9795; sensitivity: 0.945; specificity: 0.819). Salivary IL-6 protein levels were significantly higher (p < 0.001) in patients (mean ? SE: 70.98 ? 14.06 pg/mL), than in controls (mean ? SE: 12.45 ? 3.29). Specificity and sensitivity of IL-6 protein were less favorable than that of IL-6 mRNA. Salivary IL-6 mRNA expression was significantly associated with age and dental status. IL-6 manifestation was detected in tumor cells and tumor-infiltrating leukocytes, suggesting the presence of a paracrine loop of stimulation. Salivary IL-6 mRNA is one of the best performing and clinically relevant biomarkers of OSCC.
