Megjegyzések:Study Objective Facilitate to find the correct layer between the fibroid and the normal myometrium by analyzing videos of hysteroscopic fibroid resection. Design Images are cut off of recorded videos of TCRMs. These images were analyzed by full connected convolutional neural network. Images and sub-images were created. Different filters and procedures were applied by the fully convolutional neural network (FCNN) for identifying previously annotated structures. Design Classification Canadian Task Force II-2 Setting On the images of surgery, different structures were signed and annotated for the training phase. After the appropriate training of the deep neural network with 4688 images from that training set, 1600 formerly unseen images were used for testing. Patients Indication for surgery was heavy menstrual bleeding and hysteroscopic finding was submucous fibroid. Operative intervention was fibroid resection. Recorded videos of transcervical resection of myoma are used for the study. Measurements and Main Results Previously manually annotated images and the manually drawn bitmasks were used for training the applied fully convolutional neural network and then this pre-trained network was used for automatic segmentation of normal myometrium in an unseen video frame. The segmentation pixel-wise accuracy achieved the 86.19% considering the Hausdorff metric. Conclusion Using deep learning technique in analyzing process of endoscopic video frame could help in real-time identification of structures while performing endoscopic surgery.

Megjegyzések:Objective: to measure the accuracy of the assessment of tubal patency using selective pertubation with office hysteroscopy compared to laparoscopy in infertile women.Method: Selective pertubation with office hysteroscopy was also performed in 35 infertile patients prior to their scheduled laparoscopy and chromohydrotubation as part of infertility evaluation. We compared the findings of the two methods. Results: Hysteroscopic tubal assessment had a 82.9% accuracy with the laparoscopic dye method taken as reference, with a positive predictive value of 87.5%, and a negative predictive value of 76.7%. No complication or failure occurred.Conclusion: Selective pertubation with office hysteroscopy is a reliable method for the assessment of tubal patency. As a minimal invasive office procedure it can be offered as a first line method for the measurement of the uterine cavity and the tubes in infertile women.

Megjegyzések:Objective To assess the diagnostic accuracy of hysteroscopy compared to laparoscopic chromopertubation for the detection of fallopian tubal obstruction. Data Sources An electronic search from inception to March 31, 2020 was performed in Medline, Scopus, EMBASE and the Cochrane Controlled Trials Register. Methods of Study Selection This diagnostic accuracy meta-analysis was conducted following PRISMA and SEDATE recommendations. A combination of the following MeSH terms and keywords was included in search strategy: "hysteroscopy[MeSH]", "tubal obstruction" "tubal patency", "tubal dysfunction", "tubal blockage". Quality assessment of included studies was conducted using the QUADAS-2 tool. Publication bias was evaluated by means of Deek funnel plot asymmetry test. The following outcomes were analyzed: diagnostic odds ratio (DOR), area under hierarchical summary receiver operating characteristic (HSROC) and area under receiver operating characteristic curve (AUROC), sensitivity, specificity, positive and negative likelihood ratio (PLR and NLR). Tabulation, Integration, and Results Six studies comparing hysteroscopy with laparoscopic chromopertubation were included in the meta-analysis. After pooling of all the studies, DOR was 43. Evaluated AUROC was 0.93, correlating with high diagnostic accuracy for the index test. Sensitivity and specificity were 88% and 85% respectively. Also, PLR and NLR were 5.88 and 0.16 respectively. Conclusion Hysteroscopic techniques are highly accurate and sensitive for detecting fallopian tubal obstruction. Interventional assessment enhances diagnostic accuracy relative to observational assessment as does the office setting relative to the operating room. Additional studies, along with refinement of the techniques, could facilitate the widespread use of hysteroscopic techniques for the detection of fallopian tube occlusion
