Megjegyzések:Objective: To evaluate the diagnostic accuracy of a bygone method, hystero-salpingo-scintig- raphy (HSSG), for tubal patency assessment of infertile women. Material and methods: Prospective cohort study involving women in the infertility workup at the University of Debrecen, Hungary. Seventy infertile patients were scheduled to either basic dynamic HSSG, post-purge dynamic HSSG, or post-purge dynamic HSSG followed by SPECT/CT for reducing tracer contamination. The primary endpoint was the evaluation of the diagnostic accuracy of HSSG for the three methods. Results: During the basic dynamic group, the examination yielded a sensitivity of 87.5%, with a specificity of 71.7%, while positive and negative predictive values were 31.8%, and 97.4% respectively. Using post purge dynamic HSSG, it resulted in a sensitivity of 87.5%, a specificity of 88.7%, a positive predictive value of 53.8%, and a negative predictive value of 97.9%. Adding SPECT/CT to post-purge dynamic HSSG increased diagnostic accuracy with 100% sensitivity and 88.7% specificity, while positive and negative predictive values were 57.1% and 100%, respectively. Conclusion: HSSG is a non-invasive and well-tolerated technique for tubal patency. It could be used initially to predict tubal patency in case of infertility. Its diagnostic accuracy is higher when it is carried out by adding SPECT/CT to the post-purge dynamic method.

Megjegyzések:Objective: Our purpose was to investigate the predictive value of visible air bubble sign for real tubal patency during hysteroscopic procedures. Methods: In this cross-sectional study, clinical data of 61 infertile women who underwent diagnostic hysteroscopy and laparoscopy have been analysed. Results: Mean age of patients was 33.45 3.25 years. Bubble sign for the detection of patency demonstrated 73 % (95 % CI: 57?86 %) sensitivity, 70 % (95 % CI: 46?88 %) specificity, a positive predictive value of 83 % (95 % CI: 67?94 %), and a negative predictive value of 56 % (95 % CI: 35?76 %). Conclusions: A visible tubal "flow" of air bubbles during hysteroscopy was not accurate in the prediction of tubal patency.

Megjegyzések:Aim Our purpose is to investigate if transcervical resection of endometrial polyps improves the fertility in ovulatory infertile women, and whether polyp histology, intrauterine location, and the technique of polypectomy have any influence on the pregnancy rates. Methods In this retrospective study, clinical data of 87 ovulatory infertile women who underwent hysteroscopy and pol- ypectomy, and their 12-month follow-up have been analyzed. Subgroups according to the method of polyp removal (resec- toscope or curettage), the polyp localization (utero-tubal, anterior, posterior, lateral, multiple) and the histological result were interpreted. Results Meanageofpatientswas33.99?4.24years.TherewerenodifferencesintheBMIandbasalFSHlevelsbetweenthe subgroups. Pregnancy was recorded in 30 (34.5%) within the next 12 months without any difference between the subgroups of polypectomy method applied. Posterior wall polyp resection increased the pregnancy chance (OR 5.02), but no other dif- ferences were observed in 1-year pregnancy rates to other localizations. Removal of polyps which had normal endometrial histology had lower pregnancy rates as compared to that of polyps with hyperplasia or endometrial polyp histology results (OR 0.25). Conclusions Polypectomy improved the conception rate in the subsequent year regardless of the intrauterine localization and the method of its surgical removal. Therefore, we can conclude that polypectomy should be considered in infertile women.

Megjegyzések:Célkitűzés: A laparoszkópos műtétek során a taktilis ingerek hiánya miatt, sajátos technika és szemlélet szükséges. Az endoszkópos műtétek során a szervfelismerés támogatására kifejlesztésre került egy félautomata, mély tanuló, konvolúciós neurális hálózat alapú eszköz, amely nagy pontossággal képes megkülönböztetni a különböző típusú szövettani struktúrákat. Anyag és módszer: A kifejlesztett döntéstámogató alkalmazás eszköztárában a felismerendő anatómiai részeket manuálisan kell megjelölni a kimerevített képkockán. Ezután az azonosítandó szerveket a kijelölt rész elemzésével megoldja egy alkalmasan betanított konvolúciós neurális hálózat segítségével, és a kapott címkéket a képkocka megfelelő szerveihez rögzíti. Eredmények: Adatainkat a Debreceni Egyetem Szülészeti és Nőgyógyászati Klinikáján végzett endoszkópos nőgyógyászati műtétek során rögzített felvételek feldolgozásából nyertük. Ebből (27 videóról felvett), 2000 kép készült hogy betanítsuk a neurális hálót, míg a fennmaradó részt tesztelésre használtuk. Jelenlegi beállításunkkal 94,2% -os pontosságot értünk el az ureter és artéria uterina megkülönböztetésében. Következtetések: A konvolúciós neurális hálózat segítségével létrehoztunk egy olyan döntéstámogató rendszert, melynek alkalmazása mind a tanulásban ? oktatásban, mind pedig az endoszkópos beavatkozások során nagy segítséget nyújthat az operatőrnek, ezzel csökkentve a műtéti időt és a szövődmények kialakulásának esélyét.

Megjegyzések:Background: Vaginal fibromas are very rare. They are clinically firm, benign, mesenchymal, monoclonal noninfiltrating growths. These tumors originate from smooth-muscle cells, and have diverse and nonspecific clinical features. The tumors can be asymptomatic but, depending on the size and location of occurrences, can cause various symptoms, including abdominal pain, vaginal bleeding, dyspareunia, or dysuria. The tumors can be intramural or pedunculated and solid as well as cystic. Case: A 42-year-old woman, para 1, developed a vaginal myoma. This benign tumor was a solid mass in her fornix. Surgical excision of the tumor was accomplished easily and histopathologic examination established the diagnosis of a fibroid tumor of the vagina. Results: The patient had an uneventful postoperative recovery. Her hospital stay was 24 hours. Clinical examination at a follow-up consultation, after 6 weeks, showed that her vagina was normal. A postoperative examination revealed complete vaginal healing. Conclusions: Histopathologic confirmation is the "gold standard" of diagnosis of fibroid tumors, as it is the only way to exclude malignancy with certainty. Removal of such tumors by the vaginal route, wherever possible, appears to be the optimum solution.

Megjegyzések:Background Infertility is a common problem of couples with reproductive age. Tubal abnormality is an important cause of infertility, accounting for 25%?35% of female infertility. Tubal patency testing is important during infertility work-up before any assisted reproductive technique is performed. The relevance of diagnostic hysteroscopy, especially when performed in an outpatient "office" setting, in the evaluation process of infertility has increased in the last few years. During diagnostic hysteroscopy any morphological change of the uterine cavity or endometrium can be detected, not to mention the possibility for the tubal patency test. Our aim was to investigate the results of women who underwent selective pertubation with office hysteroscopy. Methods A total of 212 women were included in this retrospective study, who chose diagnostic office hysteroscopy during their infertility work-up. In each case before and after the procedure 3D ultrasound was used for evaluation of the uterine cavity. The selective pertubation with office hysteroscopy was performed in an outpatient setting, without general anaesthesia. Results Primary infertility (in 75 %), secondary infertility (in 22,17 %), repeated abortions (in 1,89 %), positive ultrasound results (in 0,94 % of the cases) made the indication for office hysteroscopy. The mean age of the patients was 33.1 ? 4.77. During the evaluation of uterine cavity 13 cases required polypectomy as well and 1 patient needed uterine myomectomy. Endometrial visualisation of 39 cases indicated biopsy as well during the evaluation with office hysteroscopy, this way 8 chronic endometritis were detected. Regarding the selective pertubation test, we detected positivity (tubal abnormality) in 61,32 % of the study, among them we explored unilateral occlusion in 43,08 % of the cases and bilateral occlusion in 56,92 % of the patients. No complications occurred during the 212 diagnostic office hysteroscopies. Conclusions Our results suggest this novel method, selective pertubation with office hysteroscopy, which is an effective, accurate, minimally invasive method to investigate tubal patency, in outpatient circumstances in order to choose proper assisted reproductive techniques. This way the complications of general anaesthesia or radiation (HSG) can be avoided. Moreover, with the examination of the uterine cavity, other factors, such as polyps, micropolyps, fibroids, chronic endometritis can be detected and treated so fertility can be improve as well.

Megjegyzések:Aim To minimize the experienced pain during office hysteroscopy, a number of technical approaches and maneuvers are used. The aim of the study was to assess the effect of suprapubic pressure (SuPuP) applied during hysteroscopy. The impact of this maneuver on experienced pain (assessed by visual analog scale [VAS]) and duration of passage through the cervical canal was measured and compared to a reference group with no SuPuP. Methods Hysteroscopy was performed in study subjects as part of their infertility work?up. Patients were randomized into two groups as part of a controlled trial. In group 1, SuPuP was applied while inserting the scope; in group 2, SuPuP was not applied. Pain experienced during the procedure was measured on a 0?10 VAS. Duration of passage through the cervical canal was measured by analyzing the video of the procedure. The effect of SuPuP was estimated by testing for between?groups differences in these outcomes. Results The number of patients included the study was 60. Mean (SD) VAS score was 3.40 (1.276) in group 1 compared to 3.33 (0.802) in group 2 (P?=?0.809). Mean (SD) time of passage through the cervical canal in group 1 was 30.5 (18.37) seconds compared to 43.0 (24.51) seconds in group 2 (P?=?0.029). Conclusion Applying suprapubic pressure could facilitate the procedure by significantly shortening the duration of the passage through the cervical canal, but significantly not reduces the pain experienced during hysteroscopy. Further studies are to be undertaken to investigate the advantages of suprapubic pressure during office hysteroscopy.

In an experienced hand provides less postoperative morbidity, complications and the time periods of hospital stay and returning to normal activity, so it has a lot of advantages compared to the laparotomy.Opposing to open-surgery, the recognition of organs is rather difficult in laparoscopy because of the lack of the tactile information. Knowing detailed anatomy, having experience letting us to recognize structures in the abdominal cavity. Because of the anatomical variations, tissue identification is not always sure relying on the visual information.Study design, size, duration COur dataset has been collected retrospectively during 35 different gynecological endoscopic operations at the Department of Obstetrics and Gynecology of the University of Debrecen. The videos have been recorded by a high definition 1-MOS endoscopic camera at 30 frames/sec rate and resolution of 1920?1080 pixels.Participants/materials, setting, methods Data of patients scheduled for gynecological endoscopic operations are analyzed. The medical expert or an assistant should manually mark the region of interest. Then, the maximum number of sub-images of size 224?224 pixels are cut off along the axis from the video frame. Finally, the classification problem is solved automatically using a convolutional neural network with the resulted labels are pinned on the corresponding organs in the video frame.Main results and the role of chance We have presented an approach to develop an application, which helps medical experts with performing endoscopic surgeries. Our effort primarily addressed the drawback of losing tactile information during key-hole surgery in the recognition of different organs. To address this problem, we have developed a semi-automatic tool, which requires a manual annotation regarding the axis of the interested organs first. Then, several sub-images covering the selected organs are extracted and classified by a fine-tuned GoogLeNet convolutional neural network.The classification performance of the fine-tuned GoogLeNet model on our test dataset considering the top-1 error rate is 0.193 at sub-image level. That is, 403 out of the 500 test images have been classified correctly. However, notice that these sub-images are only small, non-overlapping segments of the interested organs. That is, it is reasonable to fuse these label information for recognizing the corresponding organ. To do so, we have applied the simple majority-voting rule on the 4-5 labels supplied by the sub-images for the same organ. In this way, our proposed approach has reached 94.2% final accuracy regarding this binary classification task.Limitations, reasons for cautionOur collected dataset is relatively small with containing insufficient number of images to train a complex neural network, so we should extend the size of our dataset.Wider implications of the findings Using the software made by the results of the study, accuracy of the tissue/organ recognition could be increased during training laparoscopic technique, or for the experts as well.Study funding/competing interest(s) This work was supported in part by the projects GINOP-2.1.1-15-2015-00376 and VKSZ 14-1-2015-0072, SCOPIA: Development of diagnostic tools based on endoscope technology supported by the European Union, co-financed by the European Social Fund.
