Megjegyzések:Vitamin D is an important nutrient involved in many fundamental health outcomes. However, its influence on female reproductive function remains ambiguous. Cholecalciferol seems to have a role in the reproductive processes and in the patients affected by polycystic ovary syndrome, endometriosis, uterine myomas and premature ovarian failure. Moreover, it may play an important role in the assisted reproductive techniques, given that it is capable of influencing oocyte quality, but also an adequate preparation of the endometrium for embryonic implantation. Although promising, available evidence is based on a limited number of in vitro, in vivo and clinical studies. We provide an overview of the association between vitamin D and female infertility reporting the most recent data published in the literature. Nevertheless, properly randomised clinical trials are mandatory to achieve more conclusive results about the promising role of vitamin D in the management of female assisted reproduction.

Megjegyzések:During the last decades, the number of couples with reproductive issues has substantially increased. Many different factors are implicated in reproductive failure, including uterine factors. Endometrial pathologies, such as endometrial polyps, hyperplasia, endometritis, and Mullerian anomalies, can also hinder embryo implantation. Hysteroscopy remains the gold standard for the evaluation and treatment of intrauterine pathology. Over the last few years, advances in hysteroscopic instrumentations and surgical techniques have significantly evolved, the refinement in technology, miniaturization of instruments, and improved image quality have rendered hysteroscopy a more patient and user-friendly procedure that has enhanced its use in reproductive medicine. Nowadays, hysteroscopy is essential in the evaluation and treatment of women with infertility. This article underscores the major technological breakthroughs achieved over the last few years with emphasis on the role of artificial intelligence, augmented reality, and 3D hysteroscopy, which can set new benchmarks in hysteroscopy applied to reproductive medicine.

Megjegyzések:Gestational diabetes mellitus (GDM) is a very common condition among pregnant women worldwide with important metabolic implications on the mother and the o spring. Vitamin D status has been suggested to potentially play a role in GDM risk with no documented consequences for the offspring. The purpose of this article was to review currently available evidence on the relationship between vitamin D and GDM. Current evidence shows an association between vitamin D blood levels and risk of GDM, despite heterogeneity of results across studies limit the conclusions. Moreover, data from randomized controlled trials is scarce and resulting in null findings. Among the limitations to be noted, improving the standardization of dosages, the characteristics of individuals in the sample, and the appropriate outcome measurement could provide a more effective approach in understanding the relationship between vitamin D and GDM. In conclusions, despite observational studies may suggest that adequate vitamin D levels may decrease the risk of GDM compared to deficiency status, evidence from clinical trials is inadequate to draft any definitive conclusion regarding its supplementation. Future better designed randomized clinical trials taking into account a more integrated approach could provide clearer and definitive data on the outcomes of such a multifactorial condition.

Megjegyzések:Objective: To evaluate the risk of endometrial carcinoma (EC) and atypical endometrial hyperplasia (AEH) in asymptomatic postmenopausal women in relation to the endometrial thickness (ET) measured by transvaginal ultrasonography (TVS) stratified by threshold categories used for performing subsequent endometrial sampling and histologic evaluation. Data sources: MEDLINE, Scopus, ClinicalTrials.gov, Scielo, EMBASE, the Cochrane Library at the CENTRAL Register of Controlled Trials, LILACS, conference proceedings and international controlled trials registries were searched without temporal or geographical or language restrictions. Study eligibility criteria: Studies were selected if they had a cross-over design evaluating the risk for AEH and EC in postmenopausal asymptomatic women and calculated the diagnostic accuracy of TVS thresholds (at least 3.0 mm) confirmed by histopathological diagnosis. Study appraisal and synthesis methods: We conducted a systematic review and diagnostic test accuracy meta-analysis according to PRISMA-DTA and SeDATE guidelines. ET thresholds were grouped as follows: from 3.0 mm to 5.9 mm; between 6.0 and 9.9 mm; between 10.0 and 13.9 mm; and equal or greater than 14.0 mm. Quality assessment was performed using QUADAS-2 tool. Publication bias was quantified by Deek funnel plot test. Co-primary outcomes were risk for AEH or EC according to ET and diagnostic accuracy of each threshold group. Results: A total of 18 studies provided the data of 10,334 women who were all included in the final analysis. Overall, at an ET threshold of at least 3.0 mm, the risk for AEH or EC was increased three-fold relative to women below the cut-off (relative risk (RR) 3.77, 95% confidence interval (CI) 2.26 to 6.32, I2=74%). Similar degrees of risk were reported for thresholds between 3.0 and 5.9 mm (RR 5.08, 95% CI 2.26 to 11.41, I2=0%), 6.0 and 9.9 mm (RR 4.34, 95% CI 1.68 to 11.23, I2=0%), 10.0 and 13.9 mm (RR 4.11, 95% CI 1.55 to 10.87, I2=86%) and over 14.0 mm (RR 2.53, 95% CI 1.04 to 6.16, I2=78%) with no significant difference among subgroups (p=0.885). Regarding diagnostic accuracy, the pooled sensitivity decreased from thresholds below 5.9 mm (0.81, 95% CI 0.49 to 0.85) to above 14.0 mm (0.28 95% CI 0.18 to 0.40) Meanwhile, specificity increased from 0.70 (95% CI 0.61 to 0.78) for ET between 3.0 mm and 5.9 mm to 0.86 (95% CI 0.71 to 0.94) when the ET is 14.0 mm or greater. For 3.0-5.9 mm and 10.0-13.9 mm thresholds, the highest diagnostic odds ratio of 10 (95% CI 3 to 41) and 11 (95% CI 2 to 49), with an area under curve of 0.81 (95% CI 0.77 to 0.84) and 0.82 (95% CI 0.79 to 0.86) respectively were retrieved. The summary point analysis revealed that, compared to the other subgroups, the 3.0-5.9 mm cut off point was placed higher in the summary receiver operator curve space, indicating increased EC or AEH diagnosis using these cut-offs. Conclusions: Both low and high ET thresholds in postmenopausal asymptomatic women seem equally effective in detecting EC and AEH. However, although using a 3.0 to 5.9 mm cut off results in lower specificity, the offsetting improvement in sensitivity may justify using this cut off for further endometrial evaluation in patients with suspected endometrial malignancy.

Megjegyzések:Purpose The aim of this review is to provide an overview of the literature about the perception and management of anxiety and pain in women undergoing an office hysteroscopic procedure. Methods We performed a systematic literature search in Embase, PubMed/MEDLINE, Cochrane Library and Web of Science for original studies written in English (registered in PROSPERO 2019-CRD42019132341), using the terms ♭hysteroscopy' AND ♭pain' AND ♭anxiety' published up to January 2019. Only original articles (randomized, observational and retrospective studies) about management of anxiety and pain related to the hysteroscopic procedure were considered eligible. Results Our literature search produced 84 records. After exclusions, 11 studies including 2222 patients showed the following results: (a) pain experienced during hysteroscopy is negatively affected by preprocedural anxiety; (b) pharmacological interventions seem to be help in reducing pain during hysteroscopy; (c) waiting time before the procedure is a significant factor affecting patients' anxiety; (d) music during the procedure may be helpful in reducing anxiety. Conclusions The utilization of office hysteroscopy is hampered by varying levels of anxiety and pain perceived by women who are candidates for the procedure. For these reasons, it is essential to identify effective pharmacological and non-pharmacological strategies to alleviate these factors. We recommend further studies especially focusing on non-pharmacological interventions to facilitate the dissemination of good clinical practices among hysteroscopists.

Megjegyzések:Purpose To investigate the effectiveness and risks of different surgical therapies for isthmocele in symptomatic women with abnormal uterine bleeding, infertility, or for the prevention of obstetric complications, considering safety and surgical complications. Methods PubMed/MEDLINE, Scopus, Embase, Science Direct, and Cochrane Library were systematically searched (n? CRD4201912035) for original articles on the surgical treatment of isthmocele published between 1950 and 2018. Data synthesis was completed using MedCalc 16.4.3. The body of evidence was assessed using the GRADE methodology. Results We retrieved 33 publications: 28 focused on a single surgical technique, and five comparing different techniques. Meta-analysis showed an improvement of symptoms in 85.00% (75.05?92.76%) of women after hysteroscopic correction, 92.77% (85.53?97.64%) after laparoscopic/robotic correction, and 82.52% (67.53?93.57%) after vaginal correction. Hysteroscopic surgery was associated with the lowest risk of complications (0.76%, 0.20?1.66%). Conclusions We found adequate evidence supporting the use of surgery for the treatment of symptomatic isthmocele, as it was found to improve the bleeding symptoms in more than 80% of patients. Differently, we found a lack of evidence regarding the role of surgery with the purpose of improving fertility or reducing the risk of obstetric complications in women with asymptomatic isthmocele. The hysteroscopic correction of isthmocele may be the safest and most effective strategy in those patients with adequate residual myometrial thickness overlying the isthmocele. Laparoscopic and vaginal surgeries may be the preferred options for patients with a thinner residual myometrium over the defect (< 2.5 mm) and when hysteroscopic treatment is inconclusive.

Megjegyzések:Objective: The aim of this systematic review is to summarize the current evidence regarding the effectiveness of hysterectomy and hysteroscopic endometrial techniques in improving quality of life, sexual function and psychological wellbeing of women affected from abnormal uterine bleeding (AUB). Methods: We performed a systematic literature search in PubMed/MEDLINE and Embase for original studies written in English (registered in PROSPERO - ?), using the terms "endometrial ablation", "endometrial destruction", "endometrial resection", "hysterectomy", "menorrhagia", "dysfunctional uterine bleeding", "quality of life", "sexuality" published up to April 2019. Results: Our literature search produced 159 records. After exclusions, 10 studies were included showing the following results: a) both types of treatments significantly improve quality of life and psychological wellbeing; b) hysterectomy is associated to higher rates of satisfaction; c) hysterectomy is not associated with a significant deterioration in sexual function. Conclusions: Hysterectomy is currently more advantageous in terms of improving AUB and satisfaction rates than endometrial destruction techniques. Furthermore, there are some evidence of a greater improvement in general health for women undergoing hysterectomy. However, high-quality prospective randomized controlled trials should be implemented to investigate the effectiveness of hysterectomy and endometrial ablation/resection in the improvement of quality of life outcomes in larger patient cohorts.
