Megjegyzések:Objectives: To determine the additive diagnostic value of HE4 tumor marker to in the IOTA's three step strategy for managing adnexal masses focusing on non-experienced examiners.Methods: Imaging properties of ovarian masses were evaluated preoperatively using the IOTA terms and definitions. Two groups of patients were made:1: premenopausal (PEM); 2: postmenopausal (POM). In both groups adnexal masses were characterised by the ♭easy descriptors' and in cases where these were not applicable the IOTA Simple rules had been used. US results then were matched with the preoperative CA125 and HE4 tumor marker levels and histological findings. Results: Overall285patientswithovarianmasseswererecruitedin the analysis (average: 42.0 yrs; range: 12?82 yrs). In the PEM group we had 199 cases (69.82%; range: 12?53 yrs; average: 33.12 yrs),in the POM group 86 cases were involved (30.18%; range: 48?82 yrs; average: 63.09 yrs). In the PEM group 104 patients had the diagnosis based on easy descriptors (52.26%) while 76 patient of the rest fulfilled the requirements for the simple rules. In the POM group 48 patients (55.81%) were diagnosed by the easy descriptors and 36 (41.86%) of the rest could also be scanned according to the M- and B-rules and only 2 (2.33%) patients called for pattern recognition. If CA125 determination was changed to HE4 evaluation in case of easy descriptors the total diagnostic accuracy increased in the PEM group (PPV: 1.00; NPV: 0.98; sensitivity: 33.33%; specificity 100%), similar increase in POM group was not detected. 23 malignant cases were found in the PEM group, 2 of them were diagnosed by easy descriptors while 16 of them were diagnosed by the simple rules. In the POM group 47 malignancies were detected, 32 of them were diagnosed by the easy descriptors additional 12 by M-rules. Conclusions: IOTA easy descriptors and simple rules can be used effectively for non-experienced examiners in more than 90% of the cases. This can be increased by HE4 evaluation especially in premenopausal patients.

Megjegyzések:Objectives: To determine the additive diagnostic value of HE4 tumor marker to the IOTA simple ultrasonographic (US) morpho- logic scoring system in the management of ovarian masses. Methods: Imaging properties of ovarian masses were evaluated preoperatively using the IOTA M and B rules. According to the US findings and the IOTA simple rules three groups were made: 1. B group: only B rules were applicable, 2. M group: only M rules were applicable, 3. E group: both M and B rules were applicable, expert's opinion needed. US results were matched with the preoperative CA125 and HE4 levels and the histological findings.Results: patients with ovarian masses were involved in the analysis (average: 42.10yrs, 13-83yrs). Among the 107 cases (50.95%, average: 38.39yrs, 13-78yrs) of the B group two turned to be malignant (1.87%). HE4 was elevated in 8, CA125 in 66, both of them in 3 cases. 74 patients fulfilled the requirements for M-rules (35.24%, average: 49.43yrs, 15-83yrs,), 42 proved to be malignant (56.76%). HE4 was increased in 34, CA125 in 55, both of them in 31 cases. E group was formed by 29 cases (13.81%, range: 16-81yrs, average: 37.10yrs) of which 5 showed malignancy (17.24%). Where the IOTA simple rules were applied US alone had a a sensitivity of 48.84% and a specificity of 76.64% which increased to 93.40% adding HE4 evaluation to the diagnostic algorithm. PPV also increased from 0.56 to 0.79 in those cases where HE4 was used together with US. Pattern recognition showed a NPV of 0.92 and a PPV of 0.26 which was raised to 1.0 as HE4 was used additionally. Specicity of combining pattern recognition and HE4 evaluation proved to be 100% in this study.Conclusions: IOTA simple rules can be used effectively to discrim- inate between benign and malignant ovarian masses in more than 85% of the cases. Masses with complex morphology often need pattern recognition which can call for other methods increasing both the predictive values and specificity of the method. HE4 can be used effectively to increase the specificity of the diagnostic ability of US.

Megjegyzések:Background: Human epididymis protein 4 (HE4) has recently become an available tumour biomarker for detecting ovariancancer along with the standard cancer antigen 125 (CA125). However, it is unknown if the levels of HE4 and CA125 may bealtered in subjects who have impaired renal function with no ovarian disorders.Methods: In 113 female patients at different stages of chronic kidney disease (CKD) with no ovarian and lung cancer and 68subjects with normal renal and ovarian function, HE4 and CA125 concentrations were analysed by using chemiluminescentmicroparticle immunoassay (Architectw, Abbott) and electrochemiluminescent immunoassay (Modular E170w, Roche),respectively. Renal function was evaluated by measuring serum creatinine and urea concentrations (Cobas Integra-800w,Roche). Estimated glomerular filtration rate (eGFR in mL/min/1.73 m2) was calculated by the 4v-MDRD formula.Results: Significantly increased HE4 concentrations (P, 0.0001) were found in individuals with differently decreased eGFRvalues (,90 mL/min/1.73 m2) compared with clinical controls. CA125 serum concentration was higher than normal insubjects with CKD3 (eGFR ? 30?59 mL/min/1.73 m2), but significant elevation (P ? 0.006) in CA125 concentrations wasseen only in those who had severe renal failure (CKD4-5; eGFR , 30 mL/min/1.73 m2). These tendencies were independent ofage in our study cohort, and seemed to be more evident among women in premenopausal status.Conclusions: HE4 concentrations may be elevated in CKD patients with no ovarian and lung cancer. Thus, HE4 results shouldbe interpreted cautiously in women with renal disorders.
