Megjegyzések:To estimate whether a history of a previoussuccessful vaginal birth after cesarean delivery (VBAC)has an effect on a subsequent VBAC attempt.METHODS: A chart review of cases identi?ed from theInternational Classi?cation of Diseases, 9th Revision(ICD-9) codes and cases identi?ed in the logbooks on Laborand Delivery yielded 1,216 cases of attempted VBAC from1996 to 2000. Data gathered from these cases includedhistory of previous successful VBAC. Variables of interestincluded previous successful normal spontaneous vaginaldelivery, history of diabetes, labor induction, and recurrent indication for cesarean delivery.RESULTS: Of the 336 patients with a history of one or moreprevious successful VBAC attempts, 94.6% had a subsequentsuccessful VBAC, whereas 70.5% of the remaining 880 patients were successful (P < 0.001). For those patientswith one or more previous successful normal spontaneousvaginal deliveries, 87.8% had a successful VBAC, whereas75.6% were successful without this history (P=0.001). Thepresence of diabetes and a recurrent indication for cesarean delivery both decreased VBAC success and were independently associated with poor outcome (P < 0.001). Patients with a history of previous VBAC were 7 times more likely to have a subsequent VBAC success (odds ratio 7.40, 95% con?dence interval 4.51?12.16; P < 0.001). Those with ahistory of previous normal spontaneous vaginal deliverywere not more likely to have a successful VBAC when theother variables were controlled. Induction did not affectVBAC outcome.CONCLUSION: A history of a previous successful VBAC increases the likelihood for success with future attempts. Maternal diabetes and history of a recurrent indication for cesarean delivery are poor prognosticators for successful trial of labor.

Megjegyzések:To evaluate whether closure of the uterine incision with one or two layers changes uterine rupture or vaginal birth after cesarean section (VBAC) success rates. METHODS: Subjects with one previous cesarean section by documented transverse uterine incision that attempted VBAC were identified. Exclusion criteria included lack of documentation of the type of closure of the previous uterine incision, multiple gestation, more than one previous cesarean section, and previous scar other than low transverse. Uterine rupture and VBAC success rates were compared between those with single-layer and double-layer uterine closure. Time interval between deliveries, birth weight, body mass index (BMI), and history of previous VBAC were evaluated as possible confounders. RESULTS: Of 948 subjects identified, 913 had double-layer closure and 35 had single-layer closure. The uterine rupture rate was significantly higher in the single-layer closure group (8.6% vs. 1.3%, p = 0.015). This finding persisted when controlling for previous VBAC, induction, birth weight >4000 g, delivery interval >19 months, and BMI >29 (OR 8.01, 95% CI 1.96-32.79). There was no difference in VBAC success rate (74.3% vs. 77%, p = 0.685). CONCLUSION: Single-layer uterine closure may be more likely to result in uterine rupture.
