Megjegyzések:Bevezetés: Az iskolai egészségfejlesztés elgondolását 1996-ban hirdette meg az Egészségügyi Világszervezet. Az azóta összegyűjtött tapasztalatok alapján egyértelmű, hogy leghatékonyabbak az egész iskolát bevonó, minél áthatóbb és minél hosszabb időtartamú olyan programok, melyek a táplálkozásra, a testmozgásra és a lelki egészség fejlesztésére hatnak. A fentiek figyelembevételével került sor az Új Magyarország Fejlesztési Terv keretében teljeskörű iskolai egészségfejlesztési program megvalósítását segítő pályázat kiírására. Ennek egyik nyertese, a debreceni Tóth Árpád Gimnázium a 2010/2011-es tanévben valósította meg programját (a diákok mellett a tanárok és a szülők bevonásával), melynek célkitűzései között szerepelt az egészséges táplálkozás és a mindennapos testnevelés lehetőség szerinti alkalmazása, a diákok személyiségfejlesztése személyközpontú pedagógiai módszerekkel és az egészséggel kapcsolatos ismeretek oktatása. Módszertan: A program diákokat célzó elemeit ismételt keresztmetszeti kérdőíves vizsgálat keretében értékeltük, amelyből egy vizsgálat beavatkozás (intervenció) előtt, három vizsgálat intervenció után történt: a program kezdete előtt, 2011 áprilisában az iskola minden tanulója egy online kérdőívet töltött ki tanóra keretében (továbbiakban bázisfelmérés), majd ugyanezen kérdőív kitöltésére a három következő tanév kezdetén is meghívást kapott az iskola minden tanulója. Jelen közlemény csak a követés utolsó évében (2013 szeptemberében) felvett kérdőívek (zárófelmérés) adatait vizsgálta a bázisfelméréshez képest, annak érdekében, hogy azon évfolyam tanulóit értékeljük, akik mindkét felmérésben részt vettek: a bázisfelmérésben 9. évfolyamosokként, a zárófelmérésben 12. évfolyamosokként. A szekuláris irányvonalak vizsgálatához a zárófelmérésben résztvevő 12. évfolyamosokat az ugyanezen tanévben 9., 10. és 11. évfolyamos tanulókhoz hasonlítottuk. A kérdőív kérdései az Iskoláskorú gyermekek egészségmagatartása (Health Behaviour of School-aged Children, HBSC) című követéses vizsgálat 2010. évi magyar változatából származtak. Demográfiai adatok mellett a tanulók egészségi és lelkiállapotára, valamint az iskolához és a tanárokhoz fűződő viszonyára vonatkozó kérdések szerepeltek. A statisztikai elemzést STATA 16.0 programmal végeztük, folytonos változók esetén kétmintás t-próbát, kategorikus változók esetén ?2 próbát alkalmaztunk. Eredmények: A bázisfelmérésben 265 fő 9. évfolyamos tanuló (54,1% lány) vett részt; három tanévvel később, a zárófelmérés idején 175 fő 12. évfolyamos tanuló (70,3% lány) töltötte ki a kérdőívet. Utóbbiakat a szekuláris irányvonalak feltérképezésére a zárófelmérés idején 9-11. évfolyamos tanulókhoz (N=504, 56,0% lány) hasonlítottuk. A zárófelmérés idején 12. évfolyamos fiúkat a bázisfelmérés idején 9. évfolyamos tanulókhoz hasonlítva szignifikánsan csökkent a fogyasztásra ritkán ajánlott ételek fogyasztása (fiúk: d=20,0%, p<0,001; d=1,6%, p=0,003). A bázisfelmérésben részt vett tanulók szignifikánsan könnyebbnek érezték az iskolai feladatokat a zárófelmérés idején (fiúk: 15,7, szórás (SD) 3,3 pont vs. 14,4, SD 3,7 pont, p=0,028; lányok: 16,4, SD 2,6 pont vs. 13,7, SD 2,7 pont, p<0,001). Következtetések: Szignifikánsan csökkent a fogyasztásra ritkán ajánlott ételek fogyasztása, valamint könnyebbnek értékelték az iskolai feladataikat a 3 éves utánkövetés alapján a tanulói kortárs csoportok azon tagjai, akik részt vettek a teljeskörű iskolai egészségfejlesztési programban. Ugyanakkor az egy tanévben megvalósított program nem tudta ellensúlyozni a csökkenő testmozgás és a növekvő kábítószerfogyasztás szekuláris irányvonalát. A hosszútávú értékelés korlátja, hogy csak egy középiskola vett részt a vizsgálatban. Hasonló kutatási programok esetén ajánlott azok kiterjesztése több iskolára, hogy a változásokat el lehessen különíteni aszerint, hogy azok a program hatásai, az iskolára jellemző változások, vagy szekuláris irányvonalak. Ilyen típusú kutatási programokhoz és azok értékeléséhez nem csupán az iskolák, de a döntéshozó és támogató szervezetek támogatására is szükség van. Background: The concept of health promoting schools of the World Health Organization was approved in 1996. Intensive and long-term programmes that involve the whole school and focus on nutrition, physical activity and mental health proved to be the most effective. A call for proposals observing these principles was launched under the New Hungary Development Plan to support the implementation of a comprehensive school health promotion programme. One of the winners, a high school in Debrecen implemented its programme in the 2010/2011 school year (with the involvement of students, teachers and parents) with the objectives of promoting healthy eating and daily physical education, developing students' personalities through personcentred teaching methods, and improving mental health-related education. Methodology: Elements of the student-focused program were evaluated using repeated crosssectional questionnaire surveys. The first (or baseline) survey was carried out in April 2011, before the implementation of the program, after which three other surveys took place at the beginning of the next three academic years for the purpose of evaluation. All students in the school were invited to complete the online questionnaire in class at each survey. In this paper, we examined data from the last follow-up survey completed in September 2013 and compared its data to that of the baseline survey in order to assess the students who participated in both surveys. Those who participated in the baseline survey as 9th graders were 12th graders in the final survey. To identify secular trends, that is, changes having occurred independent of the programme, 12th grade pupils in the final survey were compared to pupils in 9, 10 and 11th grades in the same survey year. The questionnaire used in all surveys used items from the Hungarian version of the Health Behaviour in School-aged Children (HBSC) questionnaire of 2010 such as items on demographics, subjective health, self-esteem, health behaviour, and attitudes towards the school and teachers. Statistical analysis was performed using STATA 16.0 software, using a two-sample t-test for continuous variables and ?2 test for categorical variables. Results: 265 students (54.1% female) in 9th grade in the baseline survey were compared to 175 students (70.3% female) in 12th grade in the final survey. The 12th grade students were compared to 9th-10th-11th grade students (N=504, 56.0% girls) at the time of the final survey to map secular trends. There was a significant decrease in the intake of foods rarely recommended for consumption among boys in 12th grade at the time of the final survey compared to students in 9th grade at the time of the baseline survey (boys: d=20.0%, p<0.001; d=1.6%, p=0.003). Students in the baseline survey felt significantly easier their school tasks at the time of the final survey (boys: 15.7, standard deviation (SD) 3.3 points vs. 14.4, SD 3.7 points, p=0.028; girls: 16.4, SD 2.6 points vs. 13.7, SD 2.7 points, p<0.001). Conclusions: There was a significant reduction in consumption of unhealthy foods, and better attitude towards school tasks in that group of students who participated in a holistic school health program and were still in the school at the 3-year follow-up. However, the programme delivered in one school year could not counterbalance the secular trends of declining physical activity and increasing use of psychoactive substances. Since only one secondary school participated in this study, there is a limitation regarding conclusions on long-term causal effects. Research on the long-term outcomes of school health programs require the inclusion of several schools so that changes could be differentiated by probable cause such as the effects of the program, specific changes within schools, or secular trends. Such programs and their research require the support of not only schools, but also of decision-makers and financing organizations.

Megjegyzések:Background: This paper describes the outcomes of an integrated health promotion programme implemented in a Hungarian high school offering health education in the curriculum, daily optional physical education, teacher training in applying a person-centered approach in teaching, and parental involvement in school activities. Methods: The evaluation used mixed methods of which results of the before-6-months-after quantitative survey among pupils is described. The health status and behaviour of students were assessed by applying the Hungarian version of the HBSC questionnaire. Results: Significant improvement was found in the self-rated health of girls (6.6% increase in being of excellent health, p = 0.04), and the consumption of sweets and sugary soft drinks decreased significantly for both genders (boys: ?10.2%, p = 0.01; girls: ?6.06%, p = 0.04). However, the proportion of physically inactive girls significantly increased (girls: 11.2%, p = 0.01), and substance use did not change significantly. Discussion and conclusions: The intervention had significant positive impacts on subjective health and dietary habits and could counteract the secular trend of increasing tobacco, alcohol, and drug consumption by age among adolescents, but this unfortunately does not include physical inactivity. Offsetting the most widespread health risk behavior, physical inactivity, may require mandatory daily physical education in schools. ? 2023 by the authors.

Megjegyzések:Objectives: Physical activity (PA) has a positive effect on life satisfaction (LS) among adolescents, but the moderating effect of gender and level of PA intensity have been equivocal. Our aim was to examine the pattern of physical activity by grade in high school students, and the role of gender and grade on the association between physical activity and life satisfaction. Methods: Four repeated cross-sectional online questionnaire surveys between 2011 and 2013 were carried out among all students in one Hungarian high school (N = 3,450). Health status and behavior was assessed by the Hungarian online version of the health behavior of school-aged children (HBSC) questionnaire. Regression with robust variance estimator was used to identify determinants of life satisfaction. Results: Good self-reported health as opposed to bad increased life satisfaction by 0.30 standard deviation; having very well or well-off family as opposed to not well-off increased LS by 0.16 standard deviation; and being inactive compared to being vigorously active decreased LS by 0.1 standard deviation. Conclusions: Physical inactivity has a negative effect on life satisfaction in boys and girls regardless of grade but compounded by low perceived family wealth.

Megjegyzések:Our aim was to investigate the impact of the school psychosocial environment, including students' general attitude towards the school, perception of support from teachers and classmates as well as individual psychosocial factors including self-esteem and loneliness on life satisfaction (LS). Four repeated cross-sectional online questionnaire surveys were carried out between 2011 and 2014, inviting all students in one Hungarian high school. Health status and behaviour were assessed by the Hungarian version of the HBSC questionnaire. Results from the surveys were pooled for analysis (N = 3310 students). Heteroskedastic regression estimating robust variance was used to identify potential determinants of LS. Family wealth perceived to be well-off, self-esteem, and being perceived as a good student were identified to be the most important significant positive contributing factors of LS. Perceived good relations with classmates and teachers and an overall positive attitude to school had smaller but still significant positive effects on LS. Self-esteem was a significant moderator for the effect of perceived difficulty of schoolwork, relation with classmates, and gender. This paper shows that self-esteem is not only an independent factor but also a modifier of some school-related variables on LS. The complex interplay among school-related and individual potential determinants of LS should be taken into account in future research by controlling for their interactions.
