Megjegyzések:Fluorokinolones (i.e. Peflacine, PEF) are used in the dental and medical therapy. It was demonstrated that chronic treatment on the rats resulted in disturbance in the secretory function of the salivary glands accompanied by the morphological sign of atrophy in the secretory units. The mechanism of this impairment is unknown. Because the peripheral neuropathy was previously described as the toxic side effect of the chronic PEF treatment we proposed that the morphological and funcional disorder of salivary glands developed on the basis of a neuronal disorder. Earlier studies described, that the mast cells could release nerve growth factor (NGF), which is important in the surviving of neurons. The lack of NGF results degenerative processes in the peripheral neurons which can change the expression of neuropeptides (i.e. serotonin, SER).The aim of this study was to determine the number of mast cells and the qualitative and quantitative changes of SER immunoreactive (IR) nerve terminals in the salivary glands after PEF treatment. Adult rats were treated with PEF for 3 and 7 days. For the visualization of mast cells we have used Toluidine blue staining. Immunohisthochemical methods were used to detect the SER containing fibers on the salivary glands. After the chronic treatment we could detect the incresed number of mast cells which supports the protective role of the NGF. The number of SER IR fibers decreased compared to the control. The changes in the number of IR fibers support our previous results, that local denervation of Salivary gland can cause the atrophy of the acini.

Megjegyzések:Pefloxacin is a second-generation fluoroquinolone antibiotic. Besides its advantageous characteristics, side effects including the hypofunction of salivary glands, decreased saliva production, and peripheral neuropathy were observed during the administration of pefloxacin. The aim of this study was to investigate the changes in the number of serotonergic immunoreactive fibers and mast cells after pefloxacin treatment in the parotid and sublingual glands of rats to detect the possible neurotoxic effect of pefloxacin. The adult female rats were treated with intraperitoneal (i.p.) injection of pefloxacin for three or seven days (at a concentration of 20?mg/100g body weight) and the serotonergic innervation pattern along with the change in mast cell number were evaluated by using histochemistry and immunohistochemistry in the parotid and sublingual glands. We found that a three-day treatment significantly increased the number of immunoreactive serotonergic nerve fibers, but after a seven-day treatment the number of serotonin positive nerve fibers decreased almost to values of the control group. The alteration of mast cell number was parallel with the changes of the serotonin positive fibers during the treatment. These results suggest that pefloxacin treatment can modify the finely controlled communication between the immune- and the peripheral nervous systems, resulting neurogenic inflammatory process. The background of this process is the altered serotonergic innervation and the increased number of activated mast cells releasing different mediators for example histamine, which can finally lead to reduced number of serotonin positive nerve fibers after a seven-day treatment of pefloxacin leading to atrophy and hypofunction of the salivary glands.

Megjegyzések:A fluorokinolonok a kinolonok fluorozott származékai és elsősorban légúti és húgyúti infekciók fellépésekor alkalmazzák. Osteomyelitisek kezelésekor több héten át tartó adagolásuk is lehetséges. Krónikus kezelés hatására a nyálmirigy acinusok atrófiája figyelhető meg, amelynek hátterében perifériás idegrendszeri hatások feltételezhetőek. Korábbi vizsgálatainkban kimutattuk a substance P (SP) és calcitonin gén rokon peptid (CGRP) tartalmú idegvégződések számának csökkenését patkány nyálmirigyekben. Irodalmi adatok eredménye alapján ismert, hogy a nyálmirigyekben előforduló hízósejtek idegnövekedési faktort (nerve growth factor, NGF) termelnek, amely a neuronok túléléséhez szükséges. Az NGF hiánya a perifériás idegekben degenerációs folyamatot indíthat el, amelynek következtében megváltozhat a neuropeptidek (pl. serotonin, Ser) termelése. Jelen munka célja a fluorokinolonok csoportjába tartozó pefloxacinnal történő kezelés következtében fellépő hízósejtszám változás, valamint a Ser immunoreaktív (IR) rostok kvalitatív és kvantitatív változásának vizsgálata.A kísérletek során felnőtt nőstény Wistar patkányokat 3 vagy 7 napon keresztül kezeltük intraperitoneálisan adagolt pefloxacinnal. A kontroll csoport fiziológiás sóoldatot kapott. A túlélési idő után az állatokat elaltattuk, majd transzkardiálisan perfundáltuk fiziológiás sóoldattal és paraformaldehid 4%-os oldatával. A fixálást követően eltávolítottuk a parotis és sublingualis nyálmirigyeket. A blokkokból 10 ?m-es paraffinos metszeteket készítettünk. A hízósejteket toluidinkékkel megfestett szövettani metszeteken vizsgáltuk. A serotoninerg rostokat pedig immunhisztomkémiai módszerekkel megfestett szövettani metszeteken számoltuk. Statisztikai értékelésre a Mann-Whitney próbát használtuk.Kísérleteink eredményeként azt találtuk, hogy a 3 napos kezelés után a hízósejtek és a serotoninerg rostok száma is sziginifikánsan emelkedett a kontroll csoporthoz képest, míg a 7 napos kezelés eredménye mindkét esetben a kontroll csoporthoz hasonló volt. A hízósejtek 15-20 ?m átmérőjűek, granuláltak és a mirigyek kötőszöveti rétegében, míg a serotoninerg rostok az acinusok körül találhatóak. A változások hátterében neuroinflammatorikus folyamatokat feltételezünk, amely a hízósejtekből felszabaduló mediátoroknak tulajdonítható. Az idegrostok számának kezdeti emelkedése az idegnövekedési faktor hatásának tulajdonítható, míg további adagolás esetén a inflammatorikus mediátorok által az idegrostok károsodása léphet fel, amely megkérdőjelezi a fluorokinolonok hosszú távú alkalmazásának javallatát.

Megjegyzések:Introduction: Fluoroquinolone antibiotics (FQAs) are the fluorinated derivatives of quinolones. They can be used in the treatment of oral surgery infections and periodontal diseases. The FQAs have some adverse effects, the patients can suffer from dry mouth and peripheral neuropathic pain. In the background, the reported neurotoxicity of FQAs might be presumable. In our earlier experiments we detected changes in the serotoninergic innervation pattern in the major salivary glands after FQA treatment. In the present work we studied the changes in the minor salivary and lacrimal glands.Methods: Female rats (n=8) were treated with an intraperitoneal injection of pefloxacine, a FQA, while the control animals (n=8) were treated intraperitoneally with physiological saline.Results: After three- or seven-day treatment, the serotoninergic fibers were visualized with fluorescent immunohistochemical method and mast cells with toluidine blue staining. We calculated the number of fibers that expressed serotonin compared with control values at three- and 7-days after treatment. After three-day treatment, the number of serotoninergic fibers was significantly increased. After seven-day treatment, the number of serotonin positive nerve fibers was decreased to levels near those of the control group. In addition, we observed alteration of mast cell number, which paralleled the changes in serotonin expressing fibers during the treatment periods.Conclusions: These findings suggest that pefloxacine can modify the finely controlled communication between immune and nervous systems. The inflammatory mediators liberated from activated mast cells can result the alteration of serotonine immunoreactive fibers, which might be responsible for the decreased saliva flow and neuropathic pain.
