Megjegyzések:The release of different cytokines and mediators in tears of patients with keratoconus (KC) wearing contact lenses (CLs) may contribute to the pathology of KC. METHODS: Cohort study was established in patients with KC wearing rigid gas permeable (RGP) CL (group I), patients with ametropia wearing silicone hydrogel (Si-Hi) CL (group II) and ametropic patients wearing RGP CL (group III). RESULTS: Our findings indicate that before CL wear, the release of epidermal growth factor (EGF) and tissue-type plasminogen activator (t-PA) was attenuated, whereas matrix metalloproteinase (MMP)-9, interleukin (IL)-6, chemokine (C-C motif) ligand 5 (CCL5), IL-13, and plasminogen activator inhibitor (PAI)-1 were enhanced in KC compared with ametropes. An increasing linear trend over time was found for MMP-9, EGF, and CXCL8 in KC and MMP-9, MMP-13, IL-6, and CXCL8 in group III. Significant differences were observed in the linear trend over time between groups I and III for MMP-13 and tissue inhibitor of metalloproteinases (TIMP)-1; between groups I and II for MMP-9 and CXCL8; and between groups III and II for MMP-9, CXCL8, and MMP-13. In KC, the release of MMP-9 at week 6 and nerve growth factor (NGF) at 10 min was higher, but NGF at week 2 was lower than that in group II. The release of MMP-13 and NGF at week 2 and 6 were lower in the KC group as compared with group III, and similarly, with IL-6 and CXCL8 at week 2 and PAI at all time points. CONCLUSIONS: Contact lens wear can influence the levels and dynamics of various mediators in the tears of patients with KC that might have an impact on the progression of the disease.

Megjegyzések:Purpose We measured vascular endothelial growth factor(VEGF) levels in tear fluid and serum in patients with retinalvein occlusion (RVO).Patients and methods Eight patients with RVO due to secondarymacular oedema were examined. VEGF levels were measuredby enzyme-linked immunosorbent assay. All patientshad a full ophthalmic examination (visual acuity, slit lampbiomicroscopy, perimetry, and fluorescein angiography). Centralretinal thickness (CRT) was examined using optical coherencetomography (OCT). Tear and serum samples werecollected and examinations were performed at diagnosis and1 and 4 weeks later.Results VEGF levels in the tears of RVO eyes were significantlyhigher than in fellow eyes at diagnosis and after both 1and 4 weeks (paired t test, p1=0.01, p2=0.02, p3=0.006). Wefound a weak but significant positive correlation betweenVEGF levels in tear fluid and serum of patients with RVO(r=0.21), while this correlation tended to be stronger betweenthe fellow eyes and serum levels (r=0.33).Conclusion To the best of our knowledge, we are the first toreport an increased level of VEGF in the tear fluid of patientswith RVO. Alterations of VEGF levels in tears may be usefulfor determining stages of RVO. This non-invasive and objectivemethod may also be helpful for estimating the severity ofmacular oedema and efficacy of treatment.

Megjegyzések:Célkitűzés: Vizsgálni újonnan illesztett kontaktlencse (KL) viselésének hatását a könnyben lévő mediátorokra keratoconusos (KC) betegeken. Betegek és módszerek: Hat héten át könnymintát gyűjtöttünk 10 KC-os és 19 ametrop szemből KL-viselés (RGP/Si-Hi) esetén. Az IL-6, IL-8, IL-13, RANTES, MMP-9, MMP-13, TIMP-1, NGF és EGF koncentrációját áramlási citométerrel mértük meg. A mediátorok release-ét hasonlítottuk össze, mely a koncentráció és a könnyminták térfogatának szorzata időegység alatt. Eredmények: KL-viselés előtt az NGF alacsonyabb (p=0,049); a RANTES, IL-13, MMP 9, TIMP-1, IL-8 és IL-6 magasabb volt a KC esetén az ametropokhoz képest. KL-viselés KC-nál növelte az MMP-9 (3 x), csökkentette a TIMP-1, RANTES és IL-13 release-ét (2-22x). A 6. hét végére az MMP-9 és az IL-8 különbözött (p=0,01; p=0,04) a két csoportot összehasonlítva. A teljes időtartamra vonatkozó trend eltérő: MMP-9 (p=0,02; növekedés KC-ban, csökkenés ametropiában) és RANTES (p=0,03; csökkenés KC-ban, emelkedés ametropiában) esetén. Következtetés: A könnyben lévő mediátorok KL-viselés során befolyásolhatják a KC etiopatológiai tényezőit és hozzájárulhatnak a betegség progressziójához. Tudomásunk szerint ez az első prospektív összehasonlító vizsgálata ezen mediátoroknak keratoconusban.

Megjegyzések:Retinal Vein Occlusion (RVO) is a retinal disorder resulting in a progressive, severe loss of visual acuity. Its pathogenesis has not yet been clarified. The aim of our study was to investigate cardiovascular and thrombophilic risk factors, and their interaction. 145 RVO patients and 100 control subjects were examined. Protein C, protein S, antithrombin activity, antigen levels, prothrombin G20210A gene polymorphism, factor VIII level, factor V (Leiden) mutation, plasminogen activity, lipoprotein(a), von Willebrand factor, homocysteine and fibrinogen levels and anticardiolipin antibodies were assessed. Possible relevant pathologies (e.g. diabetes mellitus, hypertension and hyperlipidemia) were also registered. Multiple logistic regression analysis revealed that hypertension was a predictive thrombophilic factor in female patients with decreased antithrombin (OR=5.69, 95% CI=2.00-16.14, p=0.0011). Male gender may be a risk factor in patients with normal blood pressure and non-elevated lipoprotein(a) level as well (OR=4.274, 95% CI=1.748-10.448, p=0.0015).Based on our results it appears that thrombophilia is likely to contribute to the development of RVO, especially if associated with cardiovascular risk factors, like hypertension. In case of justified thrombophilia mainly with hypertension, anticoagulant and/or antiplatelet therapy may be advantageous to prevent fellow eye involvement.

Megjegyzések:Purpose To compare the concentrations of 11 tear mediators in order to reveal the biochemical difference between pellucid marginal degeneration (PMD) and keratoconus (KC).MethodsWe have designed a cross-sectional study in which patients with corneal ectasia based on slit-lamp biomicroscopy and Pentacam HR (keratometry values (K1, K2, Kmax), astigmatism, minimal radius of curvature (Rmin), corneal thickness (Apex and Min), indices (surfacevariation, vertical asymmetry, keratoconus, central keratoconus, height asymmetry and decentration)) were enrolled. Eyes of keratoconic patients were similar to the PMD patients in age and severity (K2, Kmax and Rmin). Non-stimulated tear samples were collected from nine eyes of seven PMD patients, 55 eyes of 55 KC patients and 24 eyes of 24 healthy controls. The mediators' (interleukin -6, -10, chemokine ligand 5, -8, -10, matrix metalloproteinase (MMP) -9, -13, tissue inhibitor of metalloproteinases (TIMP)-1, tissue plasminogen activator, plasminogen activator inhibitor, nerve growth factor) concentrations were measured using Cytometric Bead Array.ResultsMMP-9 was the only mediator which presented relevant variances between the two patient groups (p = 0.005). The ratios of MMP-9 and TIMP-1 were 2.45, 0.40 and 0.23 in PMD, KC and the controls, respectively.ConclusionAs far as we are aware, this is the first study that aims to reveal the biochemical differences between PMD and KC. Further studies of biomarkers to investigate the precise role of these mediators need to be defined, and it is important to confirm the observed changes in a larger study to gain further insights into the molecular alterations in PMD.

Megjegyzések:Background: The aim of this study was to present ophthalmological findings regarding Alport syndrome and report refractometry data and to present possible early signs of the syndrome. Material/Methods: Seven patients suffering from Alport syndrome were referred to the Department of Ophthalmology at the University of Debrecen between January 1st, 2014, and December 31st, 2015. All patients underwent slit lamp evaluation and dilated fundus biomicroscopy, with special attention paid to lenticonus and retinal changes. IOL Master, Pentacam HR, and ultrasound biomicroscopy were performed to assess keratometry, corneal thickness, anterior chamber depth, lens size, and axial length data. Results: One patient out of seven had ocular symptoms. Posterior polymorphous corneal dystrophy (PPMD) and dotand-fleck retinopathy were seen. Meanwhile, although keratoconus was not proven, remarkable astigmatism with high myopia was detected. The other six patients were found to have a significantly smaller lens diameter (an average of 7.82?0.66 mm, p=0.035) compared to normal controls (an average of 8.65?0.46 mm). Lenses also tended to be thicker in Alport patients (3.48?0.19 mm) compared to controls (3.4?0.2 mm), although the difference was not significant (p=0.394). The power of the lens also showed a significant difference (p=0.026), with Alport patients having lower lens power. Conclusions: Alport syndrome patients without classical ophthalmological findings have smaller crystalline lens diameter and lower lens power. These signs may support the diagnosis of Alport syndrome. Ophthalmologists should not only seek for the known classic signs, but also the parameters of the crystalline lens, especially if genetic testing is not available.

Megjegyzések:The objective of this study was to demonstrate the effect of smoking history on soft tissue expansion in specific orbital compartments in patients with Graves' ophthalmopathy. The volumes of the rectus muscles, intra and extraorbital connective, and soft tissues were measured in 110 orbits of 35 patients and 20 control subjects. Data sets from current smokers, ex-smokers, and non-smokers were compared. The total number of cigarettes smoked was calculated, and it was used as an estimate for the severity of smoking (cumulative smoking). The volume measurements were performed on T1-weighted contiguous transversal magnetic resonance images of the orbits. Connective tissue volumes were influenced by smoking history, while muscle volumes were not affected. Ex-smokers had larger amount of extraorbital connective tissue than current smokers (p = 0.012), and this volume showed a good correlation with the number of cigarettes smoked (r = 0.539, p < 0.05). In current smokers, the amount of intraorbital connective tissue correlated well with the cumulative smoking (r = 0.635, p < 0.001). We conclude that connective tissue volumes in certain orbital compartments correlate well with cumulative smoking. Extraocular muscle volumes are not influenced by smoking in patients with Graves' ophthalmopathy.

Megjegyzések:Az endokrin orbitopathia (EOP) autoimmun megbetegedés, amelyben a könnyfilm összetétele megváltozik. A tanulmány célkitűzése a könny citokin- és kemokintartalmának vizsgálata volt. Huszonhét EOP-ban szenvedő beteg 54 szeméből (21 nő, 6 férfi, kor 43,4 ± 15,2 év), 9 szemtünet nélküli Graves-Basedowkóros (GB) beteg 18 szeméből (8 nő, 1 férfi, kor 46,8 ± 11,7 év) és kontrollcsoportként 12 egészséges egyén 24 szeméből (7 nő, 5 férfi, kor 38,6 ± 13,8 év) történt könnymintavétel. Részletes szemészeti vizsgálatot követően az EOP aktivitásának megítélése Clinical Activity Score (CAS) felvétellel történt. A könnyminták tumornekrózis faktor-(TNF-) alfa-, interleukin- (IL-) 1beta-, IL-6-, IL-18-, IL-17A-, IL-13-, valamint RANTES- (CCL5, illetve regulated and normal T cell expressed and secreted) tartalmának meghatározására multiplex bead array technikát használtak. A koncentrációkból és a mintavételi időből számítva határozták meg a markerek könnybe való szekréciós rátáját (release). Az IL-1beta, IL-6, IL-13, IL-17A, IL-18, TNF-alfa és RANTES release-értéke szignifikánsan magasabbnak bizonyult az EOP csoportban a kontroll csoporthoz viszonyítva. Az EOP és a GB, illetve a GB és a kontroll csoport között nem igazolódott szignifikáns eltérés a citokin release vonatkozásában. Az EOP csoportban pozitív korreláció volt az IL-6 és a CAS (r = 0,27, p <0,05) között. Endokrin orbitopathiában a könnyfilm citokin- és kemokin-összetétele megváltozik. A citokinek megjelenése a könnyben feltételezhetően az autoimmun folyamat következménye. Az emelkedett IL-6-release az EOP immunológiai aktivitására utaló jel lehet.

Megjegyzések:Secretion of cytokines and display of cytokine receptors have been reported in the orbital connective tissue in Graves' orbitopathy (GO). Lacrimal glands are putative autoimmune targets, and changes in tear film and ocular surface have also been described. Our aim was to characterize the cytokine profile of tears in Graves' patients with (GO) and without orbitopathy (GD).Methods: Tear samples were collected from 54 eyes of GO patients (age 43.4 ?15.2 years), 18 eyes of GD patients (age 46.8?11.7 years), and 24 control eyes (age 38.6?13.8 years). Patients underwent ophthalmological examination including Clinical Activity Score (CAS). The level of interleukin (IL)-1beta, IL-6, IL-13, IL-17A, IL-18, Tumor Necrosis Factor-alfa and RANTES (Regulated upon Activation, Normal T-cell Expressed, and Secreted) as well as Plasminogen activator inhibitor-1 (PAI-1) were measured by multiplex bead array and release values were calculated. Results: The release of IL-1beta, IL-6, IL-13, IL-17A, IL-18, TNF-alfa and RANTES were significantly higher in GO patients compared to controls (p<0.05). There was a 2.5-fold increase of IL-6 release. No significant differences were found in cytokine release between the GO and GD groups. In the GO group, significant positive correlation was found between CAS and the release of IL-6 and PAI-1 into tears (r=0.27, p<0.05 and r=0.24, p<0.05, respectively). PAI-1 release was significantly higher in GO than in GD patients, and was increased in both the GD and GO groups compared to controls. Conclusions: Impaired cytokine balance has been observed in tears of GO patients. Secretion of IL-6 into tears might be a useful indicator of disease activity in GO.
