Megjegyzések:Paroxysmal nocturnal hemoglobinuria (PNH) is an acquired disorder of the pluripotent stem cell resulting from the somatic mutation of the X-linked PIG-A gene, involved in the synthesis of the glycosylphosphatidyl-inositol anchor of membrane proteins such as CD55, CD59 and CD14. In the past decade, flow cytometry has become a valuable diagnostic tool in the detection of deficient expression of the GPI-anchored proteins. We report the diagnosis of PNH in four patients confirmed by flow cytometry. Red blood cells, granulocytes and monocytes were classified as PNH types I, II and III according to the mean fluorescence intensities (MFI) of membrane proteins. MFI rate is a numerical data reflecting the severity of decreased antigen expression, and it is obtained by dividing the MFI of the type II or type III cells by the MFI of the respective cells obtained for a normal sample. We found that the investigation of granulocytes and monocytes was more informative than red blood cells when percent negativity was evaluated. In addition, the lowest MFI rate (mean 0.011) was obtained for CD14 on monocytes while CD59 and CD55 gave higher values on all three investigated cell types (0.021-0.34). Thus, CD14 on monocytes seems to be the most reliable marker for establishing the PNH clone size and the severity of antigen deficiency.

Megjegyzések:BACKGROUND: A new flow cytometric (FC) BCR-ABL immunobead assay has been developed recently. Here we present the laboratory evaluation of the commercially available kit. METHODS: Mononuclear cells were isolated, lysed and processed according to the instructions of the manufacturer. Anti-BCR antibodies adsorbed to capture beads bind the BCR-ABL fusion proteins of the lysed cells, a phycoerythrin (PE)-conjugated anti-ABL antibody is the detector reagent and mean fluorescence intensity (MFI) signals were recorded by flow cytometry. Detection of t(9;22)(q34;q11) translocation was carried out with a quantitative PCR assay. RESULTS: MFI results of 20 normal peripheral blood samples were 88±8 (mean±SD), CV 9%. K562 cells were used as positive control. Within-batch imprecision was excellent (3.7% in the normal and 10% in the pathological range). Cut-off was chosen at MFI 112, where both sensitivity and specificity were 100%. Altogether 17 chronic myeloid leukemia (CML) and 16 acute leukemia samples were analyzed. All PCR positive samples (n=14) were positive with the FC method and negative results were also concordant (n=15). Frozen cell lysates can be stored up to 4 weeks without significant decrease of MFI signal. CONCLUSIONS: The FC BCR-ABL assay is a fast, reproducible and reliable method that may be incorporated into standard flow cytometric protocols to help clinical decision-making.

Megjegyzések:Background: In acute myeloid leukemias, there is anincreased chance to develop thrombotic disorders. Wehypothesized that in addition to leukemic promyelocytes,monocytic leukemia cells may also have a higher procoagulantactivity.Methods: Fibrin formation was assessed by a one-stageclotting assay using a magnetic coagulometer. The thrombingeneration test (TGT) of magnetically isolated normalhuman monocytes, intact leukemic cells and their isolatedmicroparticles was performed by a fluorimetric assay.Phosphatidylserine (PS) expression of leukemic cells andmicroparticle number determinations were carried out byflow cytometry.Results: All cell lines displayed a significant procoagulantpotential compared to isolated normal human monocytes.In the TGT test, the mean of lagtime and the time to peakparameters were significantly shorter in leukemic cells(3.9?4.7 and 9.9?10.3 min) compared to monocytes (14.9and 26.5 min). The mean of peak thrombin in variousmonocytic leukemia cell lines was 112.1?132.9 nM vs.75.1 nM in monocytes; however, no significant differencewas observed in the ETP parameter. Factor VII-deficientplasma abolished all procoagulant activity, whereas factorXII-deficient plasma did not affect the speed of fibrinformation and thrombin generation but modulated theamount of thrombin. Factor XI-deficient plasma affectedthe time to peak values in one leukemic cell line and alsoattenuated peak thrombin. Leukemia cell-derived microparticlesfrom all three cell lines exerted a procoagulanteffect by significantly shortening the lagtime in TGT; therewas a nonsignificant difference in case of ETP parameter.Conclusions: All investigated monocytic leukemia celllines exhibited significant thrombin generation. This phenomenonwas achieved by the procoagulants on the surfaceof leukemic cells as well as by their microparticles.

Megjegyzések:Multidrug resistance (MDR) is an unwanted phenomenon, that may cause therapy failure in several neoplasms includinghematological malignancies. The purpose of any type of laboratory MDR assay is to reliably identify such patients and to provideuseful data to clinicians with a relatively short turnaround time. MDR can be multicausal and several previous data identified agroup of transmembrane proteins - the ATP-binding casette (ABC) proteins - that may be involved in MDR in varioushematological malignancies. The prototype of these proteins is the P-glycoprotein (Pgp, MDR1, ABCB1) that is a seven-membranespanning transmembrane protein capable of extruding several cytotoxic drugs that are of key importance in the treatmentof hematological disorders. Similarly other ABC proteins ? Multidrug resistance associated protein 1 (ABCC1) and breastcancer resistance protein (ABCG2) are both capable of pumping out cytotoxic drugs. Here, we present flow cytometric methodsto identify MDR proteins by antigen and activity assays. The advantage of flow technology is the short turnaround time and itsrelative easiness compared to nucleic acid based technologies. However, for the activity assays, it should be noted, that thesefunctional tests require live cells, thus adequate results can only be provided if the specimen transport can be completed within6 hours of sample collection. Identification of MDR proteins provides prognostic information and may modulate therapy, thussignifies a clinically useful information in the evaluation of patients with leukemias.

Megjegyzések:Activated platelets are key components in many arterialdisorders. P-selectin is an activation-dependentplatelet receptor, which is also identified in endothelialcells. Together with E- and L-selectin it constitutesthe selectin family. These transmembrane proteinshave continued to attract great interest as they supportrapid and reversible cell adhesion in flow systemsand thus play an essential role in multicellularinteractions during thrombosis and inflammation.Similarly to other lectins, selectins bind to differentglycoconjugates with varying affinities. Proteinligands, equipped with the appropriate carbohydrateand sulfate moieties for P-selectin binding, have beenidentified in normal peripheral blood leukocytes andseveral non-hematopoietic organs, as well as on cancercells. For diagnostic purposes, P-selectin can readilybe detected on the platelet surface by flowcytometry and by ELISA as a soluble ligand in theplasma. Along with other markers, these data can beused in the assessment of platelet activation status.Such results bear clinical significance since P-selectinhas been implicated in the pathogenesis of widespreaddisorders including coronary artery disease,stroke, diabetes and malignancy.

Megjegyzések:Myelodysplastic syndromes are clonal hematopoietic stem cell disorders. Their exact etiology is unknown. Myelodysplasticsyndromes cause progressive bone marrow failure resulting in pancytopenia and refractory, transfusion-dependent anemia.One can observe typical morphological alterations in the erythroid, myeloid and/or megakaryocytic cell lineage. Blast countsmay also be increased. The pathologic cells are genetically unstable, and a myelodysplastic syndrome might transform into acutemyeloid leukemia. The overall survival of these diseases range between few months to around ten years. Correct diagnosis andaccurate prognostic classification is essential. In the past decades several scoring systems were established beginning with theFrench-American-British classification to the most recent Revised International Prognostic Scoring System. In all of theseclassifications bone marrow morphology is still the most important factor, though nowadays the genetic aberrations and flowcytometry findings are also included. The diagnosis and prognostic classification of myelodysplastic syndromes remain a greatchallenge for hematologists.
