Megjegyzések:Abstract Acute-on-chronic liver failure (ACLF) has received different operating definitions according to the areas. Consortia in Western countries have developed definitions that apply to patients with cirrhosis while Consortia in Asia (the Asian Pacific Association for the Study of the Liver (APASL) ACLF Research Consortium [AARC] and Chinese Group on the Study of Severe Hepatitis B [COSSH] consider that ACLF can develop in patients with chronic liver diseases with or without cirrhosis. Investigators of the COSSH and Western Consortia consider that ACLF can be precipitated by acute insults that are intrahepatic (e.g., alcoholic hepatitis) or extrahepatic (bacterial infection, gastrointestinal hemorrhage), and that extrahepatic organ system failures can be used to define ACLF. In contrast, investigators of the AARC consider that ACLF is only defined by an acute onset of liver failure in response to an acute hepatic insult. Of note, although ACLF has received different operating definitions, every definition recognizes that ACLF is a distinct clinical entity. This article updates the knowledge about the distinctive features of ACLF according to the definitions used to characterize this syndrome. In addition, it discusses future directions for research in identifying the hallmarks of ACLF.
