Megjegyzések:Acquired factor XIII (FXIII) deficiency due to autoantibody against FXIII is a very rare severe hemorrhagic diathesis. Antibodies directed against the A subunit of FXIII, which interfere with different functions of FXIII, have been described. Here, for the first time, we report an autoantibody against the B subunit of FXIII (FXIII-B) that caused life-threatening bleeding in a patient with systemic lupus erythematosus. FXIII activity, FXIII-A(2)B(2) complex, and individual FXIII subunits were undetectable in the plasma, whereas platelet FXIII activity and antigen were normal. Neither FXIII activation nor its activity was inhibited by the antibody, which bound to structural epitope(s) on both free and complexed FXIII-B. The autoantibody highly accelerated the elimination of FXIII from the circulation. FXIII supplementation combined with immunosuppressive therapy, plasmapheresis, immunoglobulin, and anti-CD20 treatment resulted in the patient's recovery. FXIII levels returned to around 20% at discharge and after gradual increase the levels stabilized above 50%.

Megjegyzések:Congenital factor XIII (FXIII) deficiency is a rare,autosomal-recessive bleeding diathesis that can be associatedwith impaired wound healing and recurrent miscarriages.Delayed umbilical cord bleeding is a typicalearly presentation. Intracranial haemorrhage developsfar more frequently than in any other inherited bleedingdisorder. Therefore, life-long prophylactic factor supplementationis mandatory if FXIII activity is below 1%.Subcutaneous, mucosal, intramuscular or joint bleedingand postoperative haemorrhage can also occur [1].Previously, plasma-derived sources of FXIII wereavailable for preventive and on-demand therapy. A noveloption for FXIII replacement is a recombinant FXIII(rFXIII) concentrate (catridecacog, NovoThirteen , NovoNordisk, Bagsvaerd, Denmark) that contains the A subunit(FXIII-A) from the tetramer of plasma FXIII-A2B2.Recombinant FXIII-A2 (rFXIII-A2) combines with endogenousB subunit (FXIII-B) to form stable FXIII-rA2B2reaching a half-life of 6?12 days [2,3]. Currently rFXIII islicensed for monthly prophylaxis in a dose of 35 IU kg 1in severe congenital FXIII-A subunit deficiency [www.ema.europa.eu, www.fda.gov]. The efficacy and safety ofrFXIII have not been tested in acute bleeding. However,pharmacokinetic profile of rFXIII [3] supports the hypothesisthat it is also suitable for on-demand therapy [1].This letter reports the effective use of rFXIII for thetreatment of a major bleeding in a patient with severecongenital FXIII-A deficiency.

Megjegyzések:Blood coagulation factor XIII (FXIII) is essential for maintaining hemostasis. The absence of FXIII results in severe bleeding diathesis, which without prophylaxis frequently leads to fatal bleeding. As the usual hemostasis screening tests remain normal, the diagnosis of FXIII deficiency needs specific tests. Here, we describe FXIII activity determination by the ammonia release assay, which is the first-line test in the diagnostic algorithm for FXIII deficiency. The method for another activity test, the undeservedly rarely used fibrin cross-linking assay, is also presented. Further tests used for the classification of FXIII deficiencies, measurement of FXIII activity in platelets, ELISAs for the measurement of complex plasma FXIII (FXIII-A2B2) antigen and for FXIII-A2 in plasma and platelets are also included. Detailed description of the methods for the detection and measurement of neutralizing auto- and alloantibodies is also provided.
