Megjegyzések:In developed, developing and low-income countries alike, type 2 diabetes mellitus (T2DM)is one of the most common chronic diseases, the severity of which is substantially a consequenceof multiple organ complications that occur due to long-term progression of the disease beforediagnosis and treatment. Despite enormous investment into the characterization of the disease, itslong-term management remains problematic, with those afflicted enduring significant degradationin quality-of-life. Current research efforts into the etiology and pathogenesis of T2DM, are focusedon defining aberrations in cellular physiology that result in development of insulin resistance andstrategies for increasing insulin sensitivity, along with downstream effects on T2DM pathogenesis.Ongoing use of plant-derived naturally occurring materials to delay the onset of the disease oralleviate symptoms is viewed by clinicians as particularly desirable due to well-established efficacyand minimal toxicity of such preparations, along with generally lower per-patient costs, in comparisonto many modern pharmaceuticals. A particularly attractive candidate in this respect, is fenugreek,a plant that has been used as a flavouring in human diet through recorded history. The presentstudy assessed the insulin-sensitizing effect of fenugreek seeds in a cohort of human volunteers, andtested a hypothesis that melanin-concentrating hormone (MCH) acts as a critical determinant ofthis effect. A test of the hypothesis was undertaken using a hyperinsulinemic euglycemic glucoseclamp approach to assess insulin sensitivity in response to oral administration of a fenugreek seedpreparation to healthy subjects. Outcomes of these evaluations demonstrated significant improvementin glucose tolerance, especially in patients with impaired glucose responses. Outcome data furthersuggested that fenugreek seed intake-mediated improvement in insulin sensitivity correlated withreduction in MCH levels.

Megjegyzések:Az organokinek expressziójának megváltozása növeli az elhízottak kardiometabolikus kockázatát. A különböző szövetekben expresszált organokinek egymással való kapcsolata nem teljesen tisztázott. Célkitűzés: Tanulmányunkban a szérum afaminszintjének összefüggéseit vizsgáltuk a plazminogénaktivátor- inhibitor-1 (PAI-1) és a retinolkötő fehérje 4 (RBP-4) szintjével, valamint a nagy denzitású lipoprotein (HDL) szubfrakciókkal elhízott 2-es típusú diabéteszes (T2DM) és elhízott nem diabéteszes (NDO) betegekben. Módszerek: Vizsgálatunkba 49 fő egészséges személyt (testtömegindex [BMI]: 24,7?9,1 kg/m2), 106 fő nem diabéteszes elhízottat (NDO; BMI: 42,6?8,1 kg/m2) és 62 fő 2-es típusú diabéteszes (T2DM; BMI: 43,1?9,1 kg/m2) beteget vontunk be. Mindhárom csoport korban és nemben illesztett volt. Az afamin, a PAI-1 és az RBP-4 meghatározása ELISA-val; a HDL-szubfrakciók elválasztása Lipoprint gélelektroforézissel történt. Eredmények: A szérumafamin- és a PAI-1-szint magasabb, míg az RBP-4-szint alacsonyabb volt az NDO és a T2DM csoportban az egészségesekhez képest (p<0,001; p<0,001 és p<0,001). Az afamin a PAI-1-szinttel pozitívan (r=0,21; p=0,002), míg az RBP-4-szinttel negatívan korrelált (r=?0,22; p=0,004). Az afamin a HDL1?5 szubfrakciókkal negatív, a HDL7?10 szubfrakciókkal szoros pozitív korrelációt mutatott. Az említett korrelációk mind az össz, mind az NDO+T2DM csoportban fennálltak. Az afaminszint független előrejelzője a BMI (?=0,214; p<0,001), éhomi glükóz (?=0,291; p<0,001), közepes HDL (?=?0,36; p<0,001) és a kis HDL szubfrakció (?=0,446; p<0,001) volt. Következtetés: Eredményeink alapján az afamin a kardiometabolikus folyamatok súlyosságának egyik fontos kockázati tényezője lehet. Az organokinmintázat összetettsége az elhízással összefüggő metabolikus eltérések változatos spektrumára hívja fel a figyelmet. Köszönetnyilvánítás: A prezentáció elkészítését az NKFIH ? Megjegyzések:Altered organokine expression contributes to increased cardiometabolic risk in obesity. Our aim was to evaluate the associations of serum afamin with glucose homeostasis, atherogenic dyslipidemia, and other adipokines in severe obesity to clarify the early metabolic alterations. 106 non diabetic obese (NDO) subjects and 62 obese patients with type 2 diabetes matched for age, gender, and body mass index (BMI) were enrolled in this study. We compared their data with 49 healthy, lean controls. Serum afamin and retinol-binding protein 4 (RBP4), as well as plasma plasminogen activator inhibitor-1 (PAI-1), were measured with ELISA, and lipoprotein subfractions were analyzed using Lipoprint gel electrophoresis. Afamin and PAI-1 found to be significantly higher in the NDO and T2M group (p < 0.001 and p < 0.001, respectively) than in the controls. In contrast, RBP4 was unexpectedly lower in the NDO and T2DM group compared to controls (p < 0.001). Afamin showed negative correlations with mean LDL size and RBP4, but positive correlations with anthropometric, glucose/lipid parameters, and PAI-1 in both the overall patients and the in NDO + T2DM groups. BMI, glucose, intermediate HDL, and small HDL were predictors of afamin. Afamin may serve as a biomarker for the severity of cardiometabolic disturbances in obesity. The complexity of organokine patterns in NDO subjects draws attention to the diverse spectrum of obesity-related comorbidities.

Megjegyzések:Background and Aims : The elevated level of plasminogen activator inhibitor- 1 (PAI-1) in obese subjects with metabolic syndrome and in patients with type 2 diabetes is well established. A common 4G/5G single guanine insertion/deletion polymorphism in the promoter region of the PAI-1 gene is of functional importance in regulating PAI-1 expression. We aimed to study the potential different correlations of carbohydrate and lipid parameters and plasma PAI-1 levels in the 4G and 5G carriers in obese and lean subjects. Methods: Ninety-three morbid obese and thirty-two lean non-diabetic participants were enrolled. PAI-1 4G/5G polymorphism was determined with allele-specific PCR. Plasma PAI-1 levels were measured by ELISA. Results: The genotype distribution of PAI-1 4G/5G polymorphism was not significantly differed in obese patients (4G/4G 27.9%; 4G/5G 45.2% and 5G/ 5G 26.9%) compared to controls (4G/4G 31.3%; 4G/5G 46.9% and 5G/5G 21.9%, p?0.8). Slightly higher PAI-1 levels were found in 4G carriers (4G/ 4G+4G/5G) in obese and control group (p?0.07 and p?0.02, respectively). In all subjects with 4G/5G genotype, plasma PAI-1 correlated negatively with high-density lipoprotein-cholesterol (HDL-C) (p?0.02) and apolipoprotein AI (apoAI) levels (p<0.001). In 5G/5G participants, PAI-1 also correlated negatively with HDL-C (p?0.025) and apoAI (p?0.007) concentrations, moreover positively with triglyceride (p?0.02), fasting glucose (p?0.002) and haemoglobin A1c (p?0.027). These correlations are lacking in 4G/4G participants. Conclusions: The observed correlations between PAI-1 levels and the components of metabolic syndrome suggest a closer link between PAI-1 and lipid and carbohydrate metabolism in subjects with 5G/5G genotype. To date, associations of circulating fetuin-A and RBP4 with lipoprotein subfractions and high-density lipoprotein (HDL)-linked proteins have not been completely investigated in morbid obese and lean non-diabetic subjects. Methods: One-hundred obese non-diabetic patients (body mass index, BMI: 42.5?8.1 kg/m2) and 32 gender and age-matched normal weight controls (BMI: 24.5?2.5 kg/m2) were enrolled to our study. Serum fetuin-A and RBP4 were measured by ELISA. Lipoprotein subfractions were distributed by Lipoprint gelelectrophoresis. Results: Serum fetuin-A and RBP4 were unexpectedly lower in obese patients (p<0.01 and p<0.01, respectively) compared to controls and positively correlated with each other (p<0.001). Fetuin-A had positive correlations with HDL-C, apolipoprotein AI (apoAI), very-low density lipoprotein (VLDL) subfraction and large HDL subfraction levels but did not show association with carbohydrate parameters in all subjects. RBP4 correlated positively with HDL-C, apoAI, VLDL subfraction, intermediate HDL subfraction and small HDL subfraction concentrations, as well as C-peptide levels in overall participants. Conclusions: Our data may indicate the potential role of fetuin-A and RBP4 in impaired lipoprotein metabolism in obesity.

Serum fetuin-A and RBP4 were measured by ELISA. Lipoprotein subfractions were distributed by Lipoprint gelelectrophoresis. Results: Serum fetuin-A and RBP4 were unexpectedly lower in obese patients (p < 0.01 and p < 0.01, respectively) compared to controls and correlated with each other (r = 0.37; p < 0.001). Fetuin-A had positive correlations with HDL-C (r = 0.22; p = 0.02), apolipoprotein AI (apoAI) (r = 0.33; p < 0.001), very-low density lipoprotein (VLDL) subfraction (r = 0.18; p = 0.05), and large HDL subfraction levels (r = 0.3; p = 0.001) but did not show correlation with carbohydrate parameters in all subjects. RBP4 correlated positively with HDL-C (r = 0.2; p = 0.025), apoAI (r = 0.23; p = 0.01), VLDL subfraction (r = 0.37; p < 0.001), intermediate HDL subfraction (r = 0.23; p = 0.01), and small HDL subfraction (r = 0.21; p = 0.02) concentrations, as well as C-peptide levels in overall participants. Backward stepwise multiple regression analysis showed that serum fetuin-A concentration is best predicted by RBP4 and large HDL subfraction. In model 2, VLDL subfraction was the independent predictor of serum RBP4 level. Conclusions: Our data may indicate a potential role of fetuin-A and RBP4 in impaired lipoprotein metabolism associated with obesity.

Megjegyzések:Introduction: Adult growth hormone deficiency (AGHD) is associated with a high prevalence of metabolic syndrome (MS), which contributes to the unfavorable cardiovascular risk profile in these patients. Insulin like growth factor-1 (IGF-1) is a widely used biomarker, however it does not always reflect the cardiometabolic risk and has a poor relationship with clinical efficacy endpoints. Consequently, there is an unmet need for biomarkers to monitor responses to GH-replacement. Afamin is a hormone-like glycoprotein, expressed in the liver. Higher afamin levels are strongly associated with MS and insulin resistance (IR). Although both MS and IR are very common in AGHD, afamin has not been investigated in these patients. Purpose: To investigate afamin as a potential biomarker in patients with AGHD. Materials and methods: Participants included 20 AGHD patients (11 GH-substituted and 9 GH-unsubstituted) and 37 healthy controls. Subjects underwent routine laboratory examinations, anthropometric measurements, body composition analysis using multi-frequency bioelectrical impedance analysis (InBody720) and measurement of serum afamin concentrations. In GH-substituted subjects, GH-substitution was withdrawn for 2 months. Measurements were carried out right before GH-withdrawal, at the end of the 2-month withdrawal period, and 1 month after reinstituting GH-replacement therapy (GHRT). Results: GH-unsubstituted patients demonstrated higher afamin levels compared to controls (p=0.03). Afamin positively correlated with skeletal muscle mass, bone mineral content, total body water, extracellular- and intracellular water content, insulin (all, p<0.01), HOMA-IR (p=0.01) and C-peptide (p=0.03) levels in AGHD but not in healthy controls. In GH-substituted patients 2-month of GH-withdrawal caused significant changes in body composition, including decreased fat-free mass, skeletal muscle mass, total body water, and intracellular water content (all, p<0.01); but these changes almost fully recovered 1 month after reinstituting GHRT. Unexpectedly, afamin levels decreased after GH-withdrawal (p=0.03) and increased with reinstitution (p<0.01). Changes of afamin levels during GH-withdrawal positively correlated with changes of HOMA-IR (r=0.80; p<0.01) and changes of insulin (r=0.71; p=0.02). Conclusion: Higher afamin levels in unsubstituted AGHD patients might indicate severe metabolic dysregulation. Significant changes accompanying GH-withdrawal and reinstitution, along with strong correlations with measures of IR, suggest that afamin could be a promising biomarker to monitor GHRT-associated changes of insulin sensitivity.

Megjegyzések:A növekedési hormon?inzulinszerű növekedési faktor-1 (GH?IGF-1-) tengely jelentős szerepet tölt be a cardiovas-cularis rendszer fiziológiás működésének fenntartásában. Az elmúlt évtizedek kutatási eredményei alapján a GH?IGF-1-tengely részt vesz a perifériás ellenállás szabályozásában, és hozzájárul a normális szívizomtömeg és balkamra-funkció megtartásához. A GH?IGF-1-tengely érfalprotektív funkciói az atherosclerosis ellenében hatnak. Ellentétbena gyermekkori GH-hiánnyal, amelynél a betegség egyértelmű biológiai következménye a gyermek növekedésbelivisszamaradása, a felnőttkori növekedésihormon-hiánynak (adult growth hormone deficiency ? AGHD) nincsenekspecifikus tünetei. Az AGHD klinikai megjelenését a cardiovascularis rizikótényezők halmozódása jellemzi, amelymiatt a klinikai kép leginkább a metabolicus szindrómához hasonlítható. Újabb kutatások AGHD esetében a tradici-onális rizikótényezők mellett krónikus kisfokú gyulladás, prothromboticus állapot és oxidatív stressz jelenlétére isrámutattak, amelyek szintén közrejátszhatnak a fokozott kardiometabolikus rizikó kialakulásában. A tartós GH-szubsztitúció AGHD-betegek esetében a lipidprofil jelentős javulását eredményezi, és kedvező hatással van a testösz-szetételre, a szívizomtömegre, az endothelfunkcióra, valamint az újonnan felismert, nem tradicionális rizikóténye -zőkre is. A betegséggel összefüggő fokozott mortalitás a mai felfogás szerint multikauzális, és nem vezethető visszacsupán a GH hiányára. A kedvezőtlen mortalitásért az etiológia, az alkalmazott kezelés és az egyéb hypophysishor -mon-hiányok kezelésének nehézsége is felelős lehet. Mindazonáltal hypopituitarismus esetében optimális, a GH-pót-lást is magában foglaló hormonszubsztitúcióval az átlagpopulációéhoz közelítő mortalitás érhető el.Orv Hetil. 2023; 164(41): 1616?1627. Megjegyzések:Adult growth hormone deficiency (AGHD) is considered a rare endocrine disorder involving patients with childhood-onset and adult-onset growth hormone deficiency (AoGHD) and characterized by adverse cardiometabolic risk profile. Besides traditional cardiovascular risk factors, endothelial dysfunction, low-grade inflammation, impaired adipokine profile, oxidative stress and hypovitaminosis D may also contribute to the development of premature atherosclerosis and higher cardiovascular risk in patients with AGHD. Growth hormone replacement has been proved to exert beneficial effects on several cardiovascular risk factors, but it is also apparent that hormone substitution in itself does not eliminate all cardiometabolic abnormalities associated with the disease. Novel biomarkers and diagnostic techniques discussed in this review may help to evaluate individual cardiovascular risk and identify patients with adverse cardiometabolic risk profile. In the absence of disease-specific guidelines detailing how to assess the cardiovascular status of these patients, we generally recommend close follow-up of the cardiovascular status as well as low threshold for a more detailed evaluation.
