Megjegyzések:The prevalence of obesity has been increasing worldwide. Chemerin is a recently discovered adipokine secreted by the enlarged adipose tissue with diverse biological effects that are not well detailed yet. This study aimed to elucidate the potential role of chemerin in oxidative stress and inflammation that are characteristics for excess weight and may eventually lead to insulin resistance and atherosclerotic complications. We also analysed the associations between chemerin and classical adipokines, namely leptin and adiponectin. Therefore, we investigated non-diabetic obese patients without manifest cardiovascular disease and compared their data to healthy lean individuals. Chemerin correlated positively with markers of oxidative stress and inflammation, while it showed a negative correlation with the measure of antioxidant status, characterized by the HDL-linked paraoxonase-1 enzyme. Chemerin also correlated positively with leptin and negatively with adiponectin respectively. In our study population, oxidized low-density lipoprotein and high-sensitivity C-reactive protein were found to be the strongest predictors of chemerin level. We conclude that chemerin may contribute to chronic inflammation and increased oxidative stress in obese individuals, even in the absence of manifest insulin resistance.

Megjegyzések:Background: Afamin is a liver-produced bioactive protein and features α-and γ-tocopherol binding sites. Afamin levels are elevated in metabolic syndrome and obesity and correlate well with components of metabolic syndrome. Afamin concentrations, correlations between afamin and vitamin E, afamin and lipoprotein subfractions in non-diabetic, obese patients have not been fully examined. Methods: Fifty non-diabetic, morbidly obese patients and thirty-two healthy, normal-weight individuals were involved in our study. The afamin concentrations were measured by ELISA. Lipoprotein subfractions were determined with gel electrophoresis. Gas chromatography-mass spectrometry was used to measure α-and γ tocopherol levels. Results: Afamin concentrations were significantly higher in the obese patients compared to the healthy control (70.4 ± 12.8 vs. 47.6 ± 8.5 ?g/mL, p < 0.001). Positive correlations were found between afamin and fasting glucose, HbA1c, hsCRP, triglyceride, and oxidized LDL level, as well as the amount and ratio of small HDL subfractions. Negative correlations were observed between afamin and mean LDL size, as well as the amount and ratio of large HDL subfractions. After multiple regression analysis, HbA1c levels and small HDL turned out to be independent predictors of afamin. Conclusions: Afamin may be involved in the development of obesity-related oxidative stress via the development of insulin resistance and not by affecting alpha- and gamma-tocopherol levels.

Megjegyzések:Betatrophin, also known as angiopoietin-like protein 8 (ANGPTL8), mainly plays a role in lipid metabolism. To date, associations between betatrophin and lipoprotein subfractions are poorly investigated. For this study, 50 obese patients with type 2 diabetes (T2D) and 70 nondiabetic obese (NDO) subjects matched in gender, age, and body mass index (BMI) as well as 49 gender- and age-matched healthy, normal-weight controls were enrolled. Serum betatrophin levels were measured with ELISA, and lipoprotein subfractions were analyzed using Lipoprint gel electrophoresis. Betatrophin concentrations were found to be significantly higher in the T2D and NDO groups compared to the controls in all subjects and in females, but not in males. We found significant positive correlations between triglyceride, very low density lipoprotein (VLDL), large LDL (low density lipoprotein), small LDL, high density lipoprotein (HDL) -6-10 subfractions, and betatrophin, while negative correlations were detected between betatrophin and IDL, mean LDL size, and HDL-1-5. Proportion of small HDL was the best predictor of betatrophin in all subjects. Small LDL and large HDL subfractions were found to be the best predictors in females, while in males, VLDL was found to be the best predictor of betatrophin. Our results underline the significance of serum betatrophin measurement in the cardiovascular risk assessment of obese patients with and without T2D, but gender differences might be taken into consideration.

Megjegyzések:Az organokinek expressziójának megváltozása növeli az elhízottak kardiometabolikus kockázatát. A különböző szövetekben expresszált organokinek egymással való kapcsolata nem teljesen tisztázott. Célkitűzés: Tanulmányunkban a szérum afaminszintjének összefüggéseit vizsgáltuk a plazminogénaktivátor- inhibitor-1 (PAI-1) és a retinolkötő fehérje 4 (RBP-4) szintjével, valamint a nagy denzitású lipoprotein (HDL) szubfrakciókkal elhízott 2-es típusú diabéteszes (T2DM) és elhízott nem diabéteszes (NDO) betegekben. Módszerek: Vizsgálatunkba 49 fő egészséges személyt (testtömegindex [BMI]: 24,7?9,1 kg/m2), 106 fő nem diabéteszes elhízottat (NDO; BMI: 42,6?8,1 kg/m2) és 62 fő 2-es típusú diabéteszes (T2DM; BMI: 43,1?9,1 kg/m2) beteget vontunk be. Mindhárom csoport korban és nemben illesztett volt. Az afamin, a PAI-1 és az RBP-4 meghatározása ELISA-val; a HDL-szubfrakciók elválasztása Lipoprint gélelektroforézissel történt. Eredmények: A szérumafamin- és a PAI-1-szint magasabb, míg az RBP-4-szint alacsonyabb volt az NDO és a T2DM csoportban az egészségesekhez képest (p<0,001; p<0,001 és p<0,001). Az afamin a PAI-1-szinttel pozitívan (r=0,21; p=0,002), míg az RBP-4-szinttel negatívan korrelált (r=?0,22; p=0,004). Az afamin a HDL1?5 szubfrakciókkal negatív, a HDL7?10 szubfrakciókkal szoros pozitív korrelációt mutatott. Az említett korrelációk mind az össz, mind az NDO+T2DM csoportban fennálltak. Az afaminszint független előrejelzője a BMI (?=0,214; p<0,001), éhomi glükóz (?=0,291; p<0,001), közepes HDL (?=?0,36; p<0,001) és a kis HDL szubfrakció (?=0,446; p<0,001) volt. Következtetés: Eredményeink alapján az afamin a kardiometabolikus folyamatok súlyosságának egyik fontos kockázati tényezője lehet. Az organokinmintázat összetettsége az elhízással összefüggő metabolikus eltérések változatos spektrumára hívja fel a figyelmet. Köszönetnyilvánítás: A prezentáció elkészítését az NKFIH ? Megjegyzések:Altered organokine expression contributes to increased cardiometabolic risk in obesity. Our aim was to evaluate the associations of serum afamin with glucose homeostasis, atherogenic dyslipidemia, and other adipokines in severe obesity to clarify the early metabolic alterations. 106 non diabetic obese (NDO) subjects and 62 obese patients with type 2 diabetes matched for age, gender, and body mass index (BMI) were enrolled in this study. We compared their data with 49 healthy, lean controls. Serum afamin and retinol-binding protein 4 (RBP4), as well as plasma plasminogen activator inhibitor-1 (PAI-1), were measured with ELISA, and lipoprotein subfractions were analyzed using Lipoprint gel electrophoresis. Afamin and PAI-1 found to be significantly higher in the NDO and T2M group (p < 0.001 and p < 0.001, respectively) than in the controls. In contrast, RBP4 was unexpectedly lower in the NDO and T2DM group compared to controls (p < 0.001). Afamin showed negative correlations with mean LDL size and RBP4, but positive correlations with anthropometric, glucose/lipid parameters, and PAI-1 in both the overall patients and the in NDO + T2DM groups. BMI, glucose, intermediate HDL, and small HDL were predictors of afamin. Afamin may serve as a biomarker for the severity of cardiometabolic disturbances in obesity. The complexity of organokine patterns in NDO subjects draws attention to the diverse spectrum of obesity-related comorbidities.

Megjegyzések:Obesity is characterized by an excessive accumulation of fat leading to a plethora of medical complications, including coronary artery disease, hypertension, type 2 diabetes mellitus or impaired glucose tolerance and dyslipidemia. Formerly, several physiological roles of organokines, including adipokines, hepatokines, myokines and gut hormones have been described in obesity, especially in the regulation of glucose and lipid metabolism, insulin sensitivity, oxidative stress, and low-grade inflammation. The canonical effect of these biologically active peptides and proteins may serve as an intermediate regulatory level that connects the central nervous system and the endocrine, autocrine, and paracrine actions of organs responsible for metabolic and inflammatory processes. Better understanding of the function of this delicately tuned network may provide an explanation for the wide range of obesity phenotypes with remarkable inter-individual differences regarding comorbidities and therapeutic responses. The aim of this review is to demonstrate the role of organokines in the lipid and glucose metabolism focusing on the obese non-diabetic subgroup. We also discuss the latest findings about sarcopenic obesity, which has recently become one of the most relevant metabolic disturbances in the aging population.

Megjegyzések:Background and Aims : The elevated level of plasminogen activator inhibitor- 1 (PAI-1) in obese subjects with metabolic syndrome and in patients with type 2 diabetes is well established. A common 4G/5G single guanine insertion/deletion polymorphism in the promoter region of the PAI-1 gene is of functional importance in regulating PAI-1 expression. We aimed to study the potential different correlations of carbohydrate and lipid parameters and plasma PAI-1 levels in the 4G and 5G carriers in obese and lean subjects. Methods: Ninety-three morbid obese and thirty-two lean non-diabetic participants were enrolled. PAI-1 4G/5G polymorphism was determined with allele-specific PCR. Plasma PAI-1 levels were measured by ELISA. Results: The genotype distribution of PAI-1 4G/5G polymorphism was not significantly differed in obese patients (4G/4G 27.9%; 4G/5G 45.2% and 5G/ 5G 26.9%) compared to controls (4G/4G 31.3%; 4G/5G 46.9% and 5G/5G 21.9%, p?0.8). Slightly higher PAI-1 levels were found in 4G carriers (4G/ 4G+4G/5G) in obese and control group (p?0.07 and p?0.02, respectively). In all subjects with 4G/5G genotype, plasma PAI-1 correlated negatively with high-density lipoprotein-cholesterol (HDL-C) (p?0.02) and apolipoprotein AI (apoAI) levels (p<0.001). In 5G/5G participants, PAI-1 also correlated negatively with HDL-C (p?0.025) and apoAI (p?0.007) concentrations, moreover positively with triglyceride (p?0.02), fasting glucose (p?0.002) and haemoglobin A1c (p?0.027). These correlations are lacking in 4G/4G participants. Conclusions: The observed correlations between PAI-1 levels and the components of metabolic syndrome suggest a closer link between PAI-1 and lipid and carbohydrate metabolism in subjects with 5G/5G genotype. Megjegyzések:Background and Aims: Fetuin-A and retinol-binding protein 4 (RBP4) are secreted as both hepatokine and adipokine, and are involved in insulin resistance, obesity related dyslipidemia and atherosclerosis. To date, associations of circulating fetuin-A and RBP4 with lipoprotein subfractions and high-density lipoprotein (HDL)-linked proteins have not been completely investigated in morbid obese and lean non-diabetic subjects. Methods: One-hundred obese non-diabetic patients (body mass index, BMI: 42.5?8.1 kg/m2) and 32 gender and age-matched normal weight controls (BMI: 24.5?2.5 kg/m2) were enrolled to our study. Serum fetuin-A and RBP4 were measured by ELISA. Lipoprotein subfractions were distributed by Lipoprint gelelectrophoresis. Results: Serum fetuin-A and RBP4 were unexpectedly lower in obese patients (p<0.01 and p<0.01, respectively) compared to controls and positively correlated with each other (p<0.001). Fetuin-A had positive correlations with HDL-C, apolipoprotein AI (apoAI), very-low density lipoprotein (VLDL) subfraction and large HDL subfraction levels but did not show association with carbohydrate parameters in all subjects. RBP4 correlated positively with HDL-C, apoAI, VLDL subfraction, intermediate HDL subfraction and small HDL subfraction concentrations, as well as C-peptide levels in overall participants. Conclusions: Our data may indicate the potential role of fetuin-A and RBP4 in impaired lipoprotein metabolism in obesity.

Serum fetuin-A and RBP4 were measured by ELISA. Lipoprotein subfractions were distributed by Lipoprint gelelectrophoresis. Results: Serum fetuin-A and RBP4 were unexpectedly lower in obese patients (p < 0.01 and p < 0.01, respectively) compared to controls and correlated with each other (r = 0.37; p < 0.001). Fetuin-A had positive correlations with HDL-C (r = 0.22; p = 0.02), apolipoprotein AI (apoAI) (r = 0.33; p < 0.001), very-low density lipoprotein (VLDL) subfraction (r = 0.18; p = 0.05), and large HDL subfraction levels (r = 0.3; p = 0.001) but did not show correlation with carbohydrate parameters in all subjects. RBP4 correlated positively with HDL-C (r = 0.2; p = 0.025), apoAI (r = 0.23; p = 0.01), VLDL subfraction (r = 0.37; p < 0.001), intermediate HDL subfraction (r = 0.23; p = 0.01), and small HDL subfraction (r = 0.21; p = 0.02) concentrations, as well as C-peptide levels in overall participants. Backward stepwise multiple regression analysis showed that serum fetuin-A concentration is best predicted by RBP4 and large HDL subfraction. In model 2, VLDL subfraction was the independent predictor of serum RBP4 level. Conclusions: Our data may indicate a potential role of fetuin-A and RBP4 in impaired lipoprotein metabolism associated with obesity.
