Megjegyzések:Data mainly from one-off surveys clearly show that the health of Roma, the largest ethnic minority of Europe, is much worse than that of the general population. However, results from comprehensive exploratory studies are missing. The aim of our study was to create a complex database for comparative and association studies to better understand the background of the very unfavourable health of Roma, especially the high burden of cardiometabolic diseases. A three-pillar (questionnaire-based, physical and laboratory examinations) health survey was carried out on randomly selected samples of the Hungarian general (HG, n = 417) and Roma (HR, n = 415) populations, and a database consisting of more than half a million datapoints was created. Using selected data, the prevalence rates of metabolic syndrome (MetS) and of its components were determined, and to estimate the risk of insulin resistance (IR), surrogate measures (the homeostasis model assessment of insulin resistance index, quantitative insulin sensitivity check index, McAuley and TyG indices and the TG/HDL-C ratio) were calculated. Receiver operating characteristic curve analysis and Youden's method were used to define the optimal cut-off values of each IR index. The prevalence of MetS was very high in both study populations (HG: 39.8%, HR: 44.0%) with no statistically significant difference between the two groups in females or males. The prevalence of MetS showed a very marked increase in the HR 35?49 years age group. Among surrogate measures, the TyG index showed the greatest power for predicting IR/MetS at a cut-off value of 4.69 (77% sensitivity, 84% specificity) and indicated a 42.3% (HG) and 40.5% (HR) prevalence of IR. The prevalence of MetS and IR is almost equally very unfavourable in both groups; thus, the factors underlying the high premature mortality burden of Roma should be further clarified by investigating the full spectrum of risk factors available in the database, with a special focus on the access of Roma people to preventive and curative health services.

Megjegyzések:The increased supply and the demand-driven explosion of global health care costs threaten the sustainability of health care services, mandating optimisation of cost-efficiency via health policy. As of July 1 2006, preventive-curative health care has been reimbursed using a centralised, closed prospective method; thus Hungarian health care providers operate budgets allocated through macro, mezzo or micro-level allocation mechanisms. Lacking sufficient incentives for patient care on the lowest level of care, production efficiency is the only tool left to increase efficiency. Nonetheless, one of the bigger challenges of the twenty-first century is managing chronic diseases that mandate continuous complex care encompassing all levels of care and the optimisation of patient referral methods. However, current allocation mechanisms have not yet reflected this paradigm shift. The paper discusses how capitation financing, by providing strict budgetary constraints, a framework for local political goals and enhancing allocation efficiency, could be a proper tool for optimising both allocation and production efficiency. Moreover, we propose that given the theoretical and legal framework of gate-keeping and the organisation of patient care by primary care physicians in Hungary, vertical allocation and horizontal production efficiency may be established by strengthening a primary care system that is reimbursed through complete capitation financing.

Megjegyzések:Jelen tanulmányunkban két hasonló gazdaságfejlődési háttérrel rendelkező egészségügyi ellátórendszer összehasonlítását végezzük el. Mind Nagy-Britanniában, mind az Egyesült Államokban a korai iparosodásra jellemző, piac által vezérelt kapitalista rendszer alakult ki. Bár feltételezhető, hogy hasonlóan a többi gazdasági szektorhoz, az egészségügyben is a piaci koordináció képezi az egészségügyi ellátórendszer szervezésének alapját, de mivel az egészségügyben több ponton is sérülnek a piaci koordináció hatékonyságához szükséges alapfeltételek, a kezdetben pusztán piaci koordináción alapuló egészségügyi ellátórendszer szükségszerűen kudarcra ítéltetett. Ez Nagy-Britanniában a második világháború idején következett be, amikor megfogalmazódtak a jelen egészségügyi rendszer alapjait is képező Beveridge bizottság ajánlásai. Ez a javaslat tartalmazta, hogy az egészségügyi kiadásokat adókból kell fedezni, az egészségügyi hozzáférést minden állampolgárra ki kell terjeszteni, a fekvőbeteg-ellátás infrastruktúráját pedig állami tulajdonba kell helyezni. Így az NHS (National Health System ? 1948) rendszer fennállásának kezdetétől elmondható, hogy szinte teljesen a kormányzati szektor által finanszírozott. Ezzel szemben az Egyesült Államokban a piaci koordináció vezérelte egyéni egészségbiztosításból finanszírozott magánellátás csak az 1960-as évekre generált olyan mértékű társadalmi problémákat, hogy kormányzati szinten létrehozták a Medicaid és a Medicare rendszerét a szegények és az idős emberek egészségügyi ellátásának biztosítása céljából. Az elmúlt évek reformkísérletei eredményeként megfigyelhető a rendszerek konvergálása: az Egyesült Királyságban a piac felé történtek elmozdulások, az Egyesült Államokban pedig az állami beavatkozások súlya nőtt meg ? legutóbb ezt az Obama elnök által fémjelzett egészségügyi reformban figyelhettük meg. A két rendszer áttekintésének tükrében kitérünk a magyarországi egészségügyi ellátórendszer sajátosságaira is, valamint felvázoljuk az esetleg szükséges reformok körét.

Megjegyzések:Általánosan elfogadott, hogy az egészségügyi ellátórendszeren belül az alapellátás minősége van a legnagyobb hatással az egészségi állapotra. Hazánkban a krónikus megbetegedések okozta korai halálozás felveti az alapellátás szűrő-gondozó tevékenység javításának szükségességét. A mostani fejkvóta alapú finanszírozási rendszer kiterjesztése egy vegyes, fejkvóta alapú és tevékenységhez kapcsolt finanszírozási rendszerré megteremthetné egy lakosság közeli alapellátás - aminek a hatékony lakossági egészségfejlesztési és prevenciós programok is szerves részét képezik - alapjait. Az ellátás minőségét is figyelembe vevő finanszírozás bevezetését a prevenció (elsődleges, másodlagos és harmadlagos) szintjeihez rendelt indikátorokhoz kapcsolva javasoljuk megvalósítani.
It is generally accepted that - considering the health care system - the quality of primary care affords the greatest influence on the health status of the population. The high early mortality rates caused by chronic non-communicable diseases indicates that the preventive interventions are insufficient at the level of primary care and the current capitation-based financing fails to offer adequate incentives for improvement. By changing to a mixed capitation-based and fee-for service financing efficient health promotion and prevention could be advocated. Here we propose introducing a fee-for-service component in addition to the capitation-based financing in connection with indicators based on primary, secondary and tertiary preventive interventions performed by the general practitioners.

Megjegyzések:The wide life expectancy gap between the old and new member states of the EuropeanUnion is most strongly related to the high rate of premature mortality caused bycardiovascular diseases (CVDs). To learn more about the background of this gap,the relationship of socioeconomic status (SES) with CVD mortality, morbidity andthe utilization of antihypertensive drugs was studied in Hungary, a Central-EasternEuropean country with an extremely high relative risk of premature CVD mortality. Riskanalysis capabilities were used to estimate the relationships between SES, which wascharacterized by tertiles of a multidimensional composite indicator (the deprivation index)and CVD burden (mortality and morbidity) as well as the antihypertensive medicationsat the district level in Hungary. The excess risks caused by premature mortality fromCVDs showed a strong correlation with deprivation using the Rapid Inquiry Facility. Thedistribution of prevalence values related to these diseases was found to be similar, butin the areas of highest deprivation, where the prevalence of chronic ischaemic heartdiseases and cerebrovascular diseases was found to be higher than the national averageby 30 and 20%, the prevalence of hypertension exceeded the national average by only4%. A linear association between the relative frequency of prescriptions/redemptions anddeprivation for most antihypertensive drugs, except angiotensinogen receptor blockers,was shown. More intense screening for hypertension is proposed to improve the controlof CVDs in countries affected by high disease burden.

Megjegyzések:The risk of premature mortality caused by cardiovascular diseases (CVDs) is approximately three times higher in the Central Eastern European region than in high income European countries, which suggests a lack and/or ineffectiveness of preventive interventions against CVDs. The aim of the present study was to provide data on the relationship between premature CVD mortality, statin utilization as a preventive medication and socioeconomic deprivation at the district level in Hungary. As a conceptually new approach, the prescription of statins, the prescription redemption and the ratio between redemption and prescription rates were also investigated. The number of prescriptions for statins and the number of redeemed statin prescriptions were obtained from the National Health Insurance Fund Administration of Hungary for each primary healthcare practice for the entire year of 2012. The data were aggregated at the district level. To define the frequency of prescription and of redemption, the denominator was the number of the 40+-year-old population adjusted by the rates of 60+-year-old population of the district. The standardized mortality rates, frequency of statin prescriptions, redeemed statin prescriptions and ratios for compliance in relation to the national average were mapped using the ♭♭disease mapping'' option, and their association with deprivation (tertile of deprivation index as a district-based categorical covariate) was defined using the risk analysis capabilities within the Rapid Inquiry Facility. The risk analysis showed a significant positive association between deprivation and the relative risk of premature cardiovascular mortality, and a reverse J-shaped association between the relative frequency of statin prescriptions and deprivation. Districts with the highest deprivation showed a low relative frequency of statin prescriptions; however, significantly higher primary compliance (redemption) was observed in districts with the highest deprivation. Our data clearly indicate that insufficient statin utilization is strongly linked to the so-called physician-factor, i.e., a statin prescription. Consequently, statin treatment is poor and represents a significant barrier to reducing mortality, particularly among people living in highly deprived areas of the country.

Megjegyzések:Background: Primary nonadherence to prescribed medications occurs when patients do not fill or dispense prescriptions written by healthcare providers. Although it has become an important public health issue in recent years, little is known about its frequency, causes, and consequences. Moreover, the pattern of risk factors shows remarkable variability across countries according to the published results. Our study aimed to assess primary nonadherence to medications prescribed by general practitioners (GPs) and its associated factors among adults in Hungary for the period of 2012?2015. Methods: Data on all general medical practices (GMPs) of the country were obtained from the National Health Insurance Fund and the Central Statistical Office. The ratio of the number of dispensed medications to the number of prescriptions written by a GP for adults was used to determine the medication adherence, which was aggregated for GMPs. The effect of GMP characteristics (list size, GP vacancy, patients' education provided by a GMP, settlement type [urban or rural], and geographical location [by county] of the center) on adherence, standardized for patients' age, sex, and eligibility for an exemption certificate, were investigated through generalized linear regression modeling. Results: A total of 281,315,386 prescriptions were dispensed out of 438,614,000 written by a GP. Overall, 64.1% of prescriptions were filled. According to the generalized linear regression coefficients, there was a negative association between standardized adherence and urban settlement type (b = ?0.099, 95%CI = ?0.103 to ?0.094), higher level of education (b = ?0.440, 95%CI = ?0.468 to ?0.413), and vacancy of the general practices (b = ?0.193, 95%CI = ?0.204 to ?0.182). The larger GMP size proved to be a risk factor, and there was a significant geographical inequality for counties as well. Conclusions: More than one-third of the written prescriptions of GPs for adults in Hungary were not dispensed. This high level of nonadherence had great variability across GMPs, and can be explained by structural characteristics of GMPs, the socioeconomic status of patients provided, and the quality of cooperation between patients and GPs. Moreover, our findings suggest that the use of the dispensed-to-prescribed medication ratio in routine monitoring of primary health care could effectively support the necessary interventions.
