Megjegyzések:Background/Aims: To examine the clinical and protein expression characteristics of tumor tissues for prediction of prognosis in colorectal cancer (CRC). Methodology: We retrospectively analyzed the clinicopathological data of patients with stage T3N0 CRC, operated between 1997-2003 and the surgical materials for the relation between disease prognosis and p53, p21, p16, β-catenin, E-cadherin, EGFR, hMLH1, hMSH2 and TS protein expressions. Results: A significantly shorter 3-year disease free survival was observed in patients under the age of 50. The worst 5-year overall survival (OS) observed for patients over 70. Tumor localization and number of processed lymph nodes significantly affected prognosis. The EGFR, hMSH2 and TS expressions and the 5-fluorouracyl treatment were not found to be of prognostic value; p53 and p21 positivity had significantly worse survival. When catenin membrane expression disappeared on tumor cells, the 5-year OS rate decreased and time to metastasis shortened significantly. Membrane catenin expression, processed lymph nodes number and age were detected as independent prognostic markers. Conclusions: These results suggest that the evaluation of a clinicopathological profile, based on age, tumor localization, number of examined lymph nodes, p53, p21 and E-cadherin catenin expression appears to be useful in identifying high risk patients.

Megjegyzések:Blood coagulation factor XIII (FXIII) plays a key role in the protectionof fibrin clot against fibrinolysis, in the cross-linking of fibrinand its mechanical strength. The role of the FXIII-A subunit Val34-Leu polymorphism with fatal primary intracerebral hemorrhages(PICH) has not been studied. We evaluated retrospectively theprevalence of this polymorphism in stroke patients with fatal PICHand population control matched for age and gender. The prevalenceof this polymorphism was determined for patients with fatalPICH (n = 98, female/male: 41/57) and controls. DNA was obtainedfrom peripheral white blood cells in case of controls and from paraffin-embedded tissue sections in case of fatal PICH. The odds forincreasing the risk of PICH against the control group were 5.429,3.286, and 7.661 for total, female, and male patients, respectively.The Leu34Leu homozygous variant of the FXIII Val34Leu polymorphismsignificantly increased the risk of fatal PICH stroke in men.

Megjegyzések:Platelets are increasingly considered a bridge between mental and immunological disorders. However, data relating to platelet parameters in patients with autoimmune disorders are limited. The aim of the present study was to investigate, for the first time, the association of platelet parameters with the symptoms of affective disorders in patients with autoimmune conditions. In this cross-sectional study, we measured the complete blood count (CBC), the Generalized Anxiety Disorder Scale for anxiety (GAD-7), and the Beck Depression Inventory for depression (BDI) in 121 patients with autoimmune disorders. Mean platelet volume (MPV) was positively correlated with both anxiety and depression. Platelet distribution width (PDW) was negatively correlated with anxiety and depression. Before adjustment for covariates, logistic regression analysis revealed a significant association of MPV with depression and anxiety. After adjustment for covariates, only depression was associated with MPV. The area under the ROC curve of MPV for GAD-7 determined anxiety and BDI determined depression was 0.63. Our study showed that among the CBC hematological parameters, the MPV might be a useful biomarker of depression and anxiety in patients with autoimmune disorders. Further investigations of platelet parameters in controlled prospective studies are warranted to confirm our preliminary results.

Megjegyzések:The purpose of this study was to evaluate the association between platelet (PLT) count and mean platelet volume (MPV) and venous thromboembolism (VTE). Thus, this study reviewed and performed a quantitative synthesis on data from the literature. This meta-analysis was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. 18 studies were included in this paper. A random-effect meta-analysis was conducted for the assessment of heterogeneity using thrombosis place, type of analyzer, type of anticoagulant and incubation time of samples as covariates. A mixed-effect meta-regression was performed based on the subgroup for the whole samples using thrombosis place and method of measurement as moderators for MPV and PLT, respectively. The cumulative estimates and 95% confidence interval (95%CI) of specificity, sensitivity, area under the receiver operator characteristic curve (AUC) and diagnostic odds ratio (DOR) for MPV were calculated using a random effect model. The quality assessments were evaluated according to the quality assessment and diagnostic accuracy tool-2 (QUADAS-2). The primary outcome was the occurrence of VTE. Secondary outcomes included PLT and MPV. Patient with deep vein thrombosis is likely to have a higher value of MPV than control group (P < 0.001). The presence of pulmonary embolism (PE) had no significant effect on the standardized mean difference of MPV between patients and controls. Patients are likely to have less PLT than the control group regarding all studies. However, subgroup analysis demonstrated that this effect was significant for patients with PE (P < 0.05). The summary receiver operating characteristic (SROC) curve indicated that AUC was 0.745 (95% CI: 0.672?0.834). The DOR for MPV was 4.76 (95%CI: 2.3?9.85), with diagnostic accuracy of 0.66.

Megjegyzések:Patients with primary Raynaud's phenomenon may have a genetically determined risk for clotting factors that predispose them to aberrant microvascular thrombosis. We investigated the prevalence of factor V substitution of G to A at position 1691 (FVLeiden), prothrombin G20210A, and methyltetrahydrofolate reductase C677T mutations in these patients. Two hundred (158 women, 42 men, mean age of 42.4 +/- 13.7 years) consecutive patients with primary Raynaud's phenomenon and 200 age-sex-matched healthy controls of Hungarian origin were included in a case-control study. The prevalence of methyltetrahydrofolate reductase C677T homozygous among patients was significantly lower than in the control group (odds ratio 0.4, 95% confidence interval 0.2-0.9, P < 0.05). The prevalence of other thrombosis-associated alleles did not differ between patients with primary Raynaud's phenomenon and control subjects. FVLeiden, prothrombin G20210A, and polymorphism, prothrombin G20210A mutations have no apparent effect on the etiology of primary Raynaud's phenomenon.

Megjegyzések:The prothrombotic state resulting from the elevation of certain clotting factors and suppression of fibrinolysis con- tribute to the risk of atherothrombotic diseases. Peripheral artery disease (PAD) has been associated with elevated plasma fibrinogen concentration, increased plasminogen activator inhibitor (PAI-1) level and decreased tissue plas- minogen activator (tPA) activity. 1-3 Coagulation factor XIII (FXIII) circulates in association with fibrinogen and plays a central role in fibrin stabilization and fibrinolysis. 4 We have recently shown that in women with coronary steno- sis and history of myocardial infarction adjusted plasma FXIII levels are elevated as compared to clinical controls, and in women, but not in men, elevated FXIII level repre- sents a 2.5-3.0-fold increased risk of myocardial infarc- tion. 5 In this study we investigated FXIII levels in patients with PAD and determined the risk of PAD conferred to patients by elevated FXIII level.
