Megjegyzések:	A flow cytometric, functional in vitro method, which detects the uptake of the fluorescent dye rhodamin 123 was developed and applied in malignant hematological diseases in order to measure the sensitivity or the resistance of the patients to cytostatic drugs. The essential point of the test is the determination of an index value (pump activity index, PAI), which is based on the observation, that the accumulation of the fluorescent dye R 123 (which is substrate of the pump protein) in increased after incubation of the cells with vincristine or verapamil. In this way the index value of resistant cells is more than 1.1, while in the cells of patients having no active resistance protein it remains about 1.0. The pump activity indexes were calculated in patients suffering from non-Hodgkin's lymphoma (n=18), acute lymphoblastic leukemia (n=6) and chronic myeloid leukemia (n=6) . It was found that the determination of the pump activity indexes applying both vincristine and verapamil showed high correlation with the clinical observation.

Megjegyzések:	Waldenström macroglobulinemia is a rare lymphoproliferative disease of B-cell origin.These tumorous B-cells producemonoclonal IgM type protein. Diagnosis is based on the detection of lymphoplasmacytic invasion of the bonemarrow and serum electrophoresis. Clinical symptoms such as anemia, hyperviscosity and neuropathy are the commomconsequences of bone marrow infi ltration and serum monoclonal IgM protein. Former use of alkylating agentsare replaced by purine analogues, rituximab and bortezomib. Additional clinical data have also accumulated regardingautologous and allogenous stem-cell transplantation. The authors present their own clinical experience and givea detailed review of current therapeutic approaches.
