After 1 year, the femoral neck BMD increased significantly in the tight control compared to the routine management group. INTRODUCTION: We intended to ascertain whether tight control (i.e., follow-up visits and bone turnover markers/BTM/and parathyroid hormone/PTH/monitoring at 3-month intervals) strategy achieves a statistically greater increase in bone mineral density over the observation period than standard follow-up care (i.e., bone densitometry at 1-year intervals, without BTM monitoring). METHODS: We studied involutional osteoporotic patients newly enrolled into chronic care. One hundred and eleven patients underwent tight control, while another 113 received routine treatment (with follow-up visits scheduled at >?1-year intervals). We compared the changes in bone mineral density reflected by the results of bone mineral density (BMD) measurements of the lumbar spine and of the left femoral neck. Statistical analyses were performed with version 22 of the SPSS software package. RESULTS: In the group of patients under tight control, baseline and follow-up median BMD values were 0.842/0.881 g/cm2 at the L1-4 vertebrae and 0.745/0.749 g/cm2 at the femoral neck. In the group under routine care, the corresponding values were 0.903/0.915 g/cm2 and 0.742/0.72 g/cm2, respectively. The relative changes of the bone mineral density of the femoral neck was significantly (p?=?0.041) higher in patients under tight control than in those receiving routine care; however, BMD changes in the lumbar spine were not statistically different. CONCLUSION: Our findings suggest that adopting tight control as a new therapeutic strategy might be justified in the osteoporosis management. In fact, a greater improvement of BMD can be achieved by treatment according to these principles.

Megjegyzések:Limited data are available on the vitamin D3 status and bone mineral density (BMD) of patients with psoriasis orwith psoriatic arthritis. Our study intended to explore possible correlations between vitamin D status and BMD, aswell as among these parameters and the features of the underlying disorder. Seventy-two patients with psoriasis/or psoriatic arthritis (female : male ratio, 40:32; mean age, 58.5 ? 11.6 years; mean duration of follow up,142.7 ? 147.7 months) participated in the study. We evaluated the characteristic clinical features of the underlyingdisease, performed bone densitometry of the lumbar spine and the hip region, measured the serum vitamin 25(OH)D3 levels of the patients, and undertook the statistical analysis of the relationships between the clinical andthe laboratory parameters. The proportion of patients with a low BMD value did not exceed that seen in the generalpopulation. We found an inverse correlation between the serum level of vitamin 25(OH)D3 and body massindex, as well as between the former and the severity of skin involvement. Furthermore, the activity of psoriaticarthritis was significantly higher in patients with inadequate vitamin D3 status. In patients with psoriatic arthritis,BMD significantly exceeded the values measured in patients suffering from psoriatic skin lesions only. Our findingssuggest the importance of evaluating the vitamin D3 status and screening for comorbid conditions in patientswith psoriasis or psoriatic arthritis. This appears justified, in particular, due to the possible role of hypovitaminosisD3 in provoking the development of skin lesions and joint symptoms.

Megjegyzések:BEVEZETÉS: Kevés adat áll rendelkezésre a psoriasisban, illetve arthritis psoriaticában szenvedő betegek D3-vitamin-ellátottságával, illetve csontdenzitásával kapcsolatban. A D3-vitamin-ellátottság és az alapbetegség klinikai jellemzői közötti kapcsolat egyelőre szintén tisztázatlan. Célunk a psoriasisban/arthritis psoriaticában szenvedő betegeink körében a D-vitamin-státusz, a csontdenzitás (BMD), valamint ezen paraméterek és az alapbetegség összefüggéseinek vizsgálata volt. BETEGEK ÉS MÓDSZEREK: 72 psoriasisban/arthritis psoriaticában szenvedő beteg (nő/férfi arány 40/32, átlagéletkoruk 58,5 +- 11,6 év, átlagos követési idő 142,7 +- 147,7 hónap) vett részt a vizsgálatban. Felmértük az alapbetegségre jellemző klinikai paramétereket, elvégeztük az ágyéki gerinc és a csípőtájék csontdenzitometriás vizsgálatát, vizsgáltuk a betegek szérum-25(OH)-D3-vitamin-szintjét, majd a klinikai, valamint laboratóriumi paraméterek közötti összefüggéseket statisztikailag elemeztük. EREDMÉNYEK: Azt találtuk, hogy a psoriasisos/arthritis psoriaticás betegek körében igen magas volt az inadekvát D3-vitamin-ellátottság prevalanciája, de az alacsony BMD nem volt gyakoribb, mint az átlagpopulációban. A szérum 25(OH)-D3-vitamin-szintje fordítottan korrelált a bőrbetegség súlyosságával és a BMI-vel, az arthritis psoriatica akti-vitása pedig szignifikánsan magasabb volt elégtelen D3-vitamin-ellátottság esetén. Az arthritis psoriaticában szenvedők csontdenzitását szignifikánsan magasabbnak mértük, mint a csak bőrtünettel rendelkező betegekét. KÖVETKEZTETÉS: Eredményeink amellett szólnak, hogy a D3-vitamin-ellátottság és a fennálló komorbid állapotok szűrése fontos lehet psoriasisban, illetve arthritis psoriaticában szenvedő betegekben, különösen amiatt, hogy felmerül a D3-hypovitaminosis provokáló szerepe a bőr és ízületi tünetek kialakulásában.
