Megjegyzések:Ultraviolet (UV) light is absorbed by nucleic acids, proteins or other endogenous chromophores, such as por-phyrins, ?avins and melanin, triggering biological processes in skin cells. Both UV-induced mutations in mel-anocytes and changes in the immune microenvironment are understood to play a role in the development ofcutaneous melanoma. The degree of UV-induced stress and the protection against this stress are in?uenced byboth intracellular and intercellular molecular interactions. The present review summarizes the known majormolecular biological changes induced by UV light in the skin that play a role in melanoma initiation and pro-motion. Nevertheless, cutaneous melanoma is not a homogenous disease, and the interaction of variable en-vironmental exposure and di?erent genetic susceptibility and other host factors lead to the formation of mel-anomas with di?erent biological behavior and clinical characteristics. This review highlights the challenges inthe understanding of how UV radiation contributes to the formation of cutaneous melanoma, and reviews thenew results of photobiology and their link to tumor genetics and tumor immunology with potential implicationson melanoma prevention and therapeutic strategies. The information presented here is expected to add clarity toongoing research e?orts in this ?eld to aid the development of novel strategies to prevent and treat melanoma.

Megjegyzések:Actinic keratosis is the precancerous lesion of squamous cell carcinoma. Their multiple appearance on one area reflects field cancerization. Therefore active treatment of the field is necessary to avoid the development of incidental squamous cell carcinoma. Conventional photodynamic therapy (PDT) is a widely used therapeutic method for actinic keratosis. Er:YAG (erbium:yttrium-aluminium-garnet) ablative fractional laser (AFL) - assisted PDT is used to increase the therapeutic efficacy of conventional PDT. The aim of this study was to assess the differences between conventional PDT and Er:YAG - assisted PDT both in efficacy and rejuvenation effect. To investigate the therapeutic efficiency, sixteen patients with actinic keratosis and chronic sun-damaged skin were irradiated with wIRA lamp on the half side of the skin involved in actinic keratosis e after 20% 5-aminolevulinic acid was applied as photosensitizer. The other side of the skin was pre-treated with Er:YAG-AFL before 5-ALA. 24/ 48 hours and 3 months after the treatment biopsy samples were taken to measure the degree of the keratinocyte dysplasia compared to the control (untreated) samples. P53 mutant keratinocytes were detected with immunohistochemical staining. The number and grade of actinic keratosis and the objective signs of photoaging was also assessed. In our hands, both conventional PDT and Er:YAG-AFL - assisted PDT led to great reduction in the number of actinic keratosis, and this decrease was significantly higher in the case of Er:YAG-AFL PDT. Both improved global photoaging, but we found no significant differences between the two methods. According to the immunohistochemical staining, both decreased the number of p53 mutant keratinocytes in the epidermis. We found no significant differences in the therapeutic efficiency between Er:YAG-AFL PDT and conventional PDT regarding clinical and immunohistochemical evaluation.

Megjegyzések:Introduction: Ultraviolet light induced DNA damage, combined with immunosuppression and inflammation are involved in the pathogenesis of actinic keratosis. Photodynamic therapy not only destroys dysplastic cells via tissue destruction and vascular shutdown, but also induces an acute local inflammatory response and activates both the innate and adaptive immune system. In our current work we aimed to compare immunohistochemistry features of inflammatory infiltrate of actinic keratoses after 5-aminolevulinic acid photodynamic therapy with or without Er:YAG laser resurfacing. Methods: Eleven patients with multiple actinic keratosis on the scalp, face, hands or forearms were treated by conventional and Er:YAG laser assisted 5-aminolevulinic acid PDT in split-site manner. Biopsies of AKs were taken before, 48 h and 3 months after the treatment. CD3, CD4, CD8, CD1a, Ki67 and p53 expressions were analyzed by immunohistochemical methods. Results: The number of p53 and Ki67 positive cells decreased significantly 3 months after treatment, but the abnormal cells were not eliminated totally. The number of CD1a+ Langerhans cells significantly decreased 48 h after both treatments, while CD8+ T cell count was significantly lower 3 months after Er:YAG laser assisted photodynamic therapy. However, the number of CD3+ and CD4+ T cells were not changed significantly 48 h and 3 months later. Conclusions: One session of 5-aminolevulinic acid photodynamic therapy even with Er:YAG laser pretreatment could not terminate actinic damage totally. Photodynamic therapy induced immunological changes. However further investigations are needed to answer how the composition of actinic keratosis' immune infiltrate influence the effect of photodynamic therapy.

Megjegyzések:The precise role of total body 18 F-fluorodeoxyglucose-positron emission tomography/computed tomography (PET/CT) in the clinical management of patients with cutaneous malignant melanoma (CMM) is not well established.OBJECTIVE:The purpose of this study was to investigate the diagnostic accuracy of PET/CT in early- and late-stage patients with high-risk CMM.METHODS:We retrospectively analysed various imaging, histopathological and clinical data from 97 patients also examined by PET/CT during a 5-year period (2007-2011). Three groups were assessed: stage I/II, resected stage III and unresectable stage III/stage IV.RESULTS:The median follow-up time of living patients was 43.48 ? 19.67 (15-142) months. We observed a high diagnostic accuracy in all stages (91.3%, 92.5% and 96.2% respectively). PET/CT appeared to be reliable diagnostic tool even for the detection of small lymph node metastases. PET/CT was informative in 14 of 19 cases wherein another imaging examination provided inconclusive results regarding lesion dignity. However, PET/CT was less suitable for properly evaluating the dignity of a lung lesion. A true positive scan was twice as likely in clinically negative patients with resected stage III disease than in patients with stage I/II disease (35.9% and 14.5%, P = 0.007).CONCLUSIONS:These results confirm that PET/CT is an important diagnostic tool in the management of patients with high-risk CMM, but it cannot replace the standard of care examinations. More accurate clinicopathological and timing criteria must be defined to best utilize the advantages of this imaging method.
