Megjegyzések:The wide life expectancy gap between the old and new member states of the EuropeanUnion is most strongly related to the high rate of premature mortality caused bycardiovascular diseases (CVDs). To learn more about the background of this gap,the relationship of socioeconomic status (SES) with CVD mortality, morbidity andthe utilization of antihypertensive drugs was studied in Hungary, a Central-EasternEuropean country with an extremely high relative risk of premature CVD mortality. Riskanalysis capabilities were used to estimate the relationships between SES, which wascharacterized by tertiles of a multidimensional composite indicator (the deprivation index)and CVD burden (mortality and morbidity) as well as the antihypertensive medicationsat the district level in Hungary. The excess risks caused by premature mortality fromCVDs showed a strong correlation with deprivation using the Rapid Inquiry Facility. Thedistribution of prevalence values related to these diseases was found to be similar, butin the areas of highest deprivation, where the prevalence of chronic ischaemic heartdiseases and cerebrovascular diseases was found to be higher than the national averageby 30 and 20%, the prevalence of hypertension exceeded the national average by only4%. A linear association between the relative frequency of prescriptions/redemptions anddeprivation for most antihypertensive drugs, except angiotensinogen receptor blockers,was shown. More intense screening for hypertension is proposed to improve the controlof CVDs in countries affected by high disease burden.

Megjegyzések:A serious adverse effect of cancer therapies is cardiovascular toxicity, which significantly limits the widespread use of antineoplastic agents. The promising new field of cardio-oncology offers the identification of potent anti-cancer therapeutics that effectively inhibit cancer cell proliferation without causing cardiotoxicity. Future introduction of recently identified cardio-safe compounds into clinical practice (including ERK dimerization inhibitors or BAX allosteric inhibitors) is expected to help oncologists avoid unwanted cardiological complications associated with therapeutic interventions.

Megjegyzések:Ventricular fibrillation (VF) and sudden cardiac death (SCD) are predominantly caused by channelopathies and cardiomyopathies in youngsters and coronary heart disease in the elderly. Temporary factors e.g. electrolyte imbalance, drug interactions and substance abuses may play an additive role in arrhythmogenesis. Ectopic automaticity, triggered activity, and re-entry mechanisms are known as important electrophysiological substrates for VF determining the antiarrhythmic therapies at the same time. Emergency need for electrical cardioversion is supported by the fact that every minute without defibrillation decreases survival rates by approximately 7-10%. Thus, early defibrillation is an essential part of antiarrhythmic emergency management. Drug therapy has its relevance rather in the prevention of sudden cardiac death, where early recognition and treatment of the underlying disease has significant importance. Cardioprotective and antiarrhythmic effects of beta blockers in patients predisposed to sudden cardiac death were highlighted in numerous studies, hence nowadays these drugs are considered to be the cornerstones of the prevention and treatment of life-threatening ventricular arrhythmias. Nevertheless, other medical therapies have not been proven to be useful in the prevention of VF. Although amiodarone has shown positive results occasionally, this was not demonstrated to be consistent. Furthermore, the potential proarrhythmic effects of drugs may also limit their applicability. Based on these unfavourable observations we highlight the importance of arrhythmia prevention, where echocardiography, electrocardiography and laboratory testing play a significant role even in the emergency setting. In the following we provide a summary on the latest developments in cardiopulmonary resuscitation, and the evaluation and preventive treatment possibilities of patients with increased susceptibility to VF and SCD.
