Megjegyzések:Oral squamous cell carcinoma (OSCC) accounting for about 90% of malignant oral lesionsis the 6th most common malignancy worldwide. Diagnostic delay may contribute to dismalsurvival rate therefore, there is a need for developing specific and sensitive biomarkers toimprove early detection. Hungarian population occupies the top places of statistics regardingOSCC incidence and mortality figures therefore, we aimed at finding potential salivaryprotein biomarkers suitable for the Hungarian population. In this study we investigated 14proteins which were previously reported as significantly elevated in saliva of patients withOSCC. In case of IL-1?, IL-1?, IL-6, IL-8, TNF-? and VEGF a Luminex-based multiplex kitwas utilized and the salivary concentrations were determined. In case of catalase, profilin-1,S100A9, CD59, galectin-3-bindig protein, CD44, thioredoxin and keratin-19, SRM-basedtargeted proteomic method was developed and the relative amount of the proteins wasdetermined in the saliva of patients with OSCC and controls. After several rounds of optimizationand using stable isotope-containing peptides, we developed an SRM-based methodfor rapid salivary protein detection. The validation of the selected potential biomarkers byELISA revealed salivary protein S100A9 and IL-6 as useful protein biomarkers for OSCCdetection improving the diagnostic accuracy for OSCC in the Hungarian population.A noninvasivediagnostic method to detect biomarkers useful for the early diagnosis of OSCC wasdeveloped. This can be an attractive strategy in screening saliva samples collected in anation-wide multi-centric study in order to decrease morbidity, mortality, to enhance survivalrate and to improve quality of life. The heterogeneity of protein biomarkers found in differentethnic groups presented in the literature highlights the importance of identification of population-tailored protein biomarkers.

Regrettably Hungary occupies top places of statistics regarding OSCC incidence, therefore there is a high need for developing sensitive and specific biomarkers to improve early detection. The saliva as a biological fluid for the detection of diagnostic and prognostic biomarkers has become increasingly important. Different research groups used saliva and potential salivary biomarkers specific for different countries were identified. In this study 14 proteins previously reported in the literature as significantly elevated in saliva of OSCC patients were examined. Collection of human saliva is a simple, non-invasive and cost-effective approach for screening large populations. A targeted proteomics method was developed and optimized for catalase, thioredoxin, S100A9, galectin-3 binding protein, cytokeratin 21, profilin, CD44 and CD59. Moreover the relative amounts of these proteins was determined in the saliva of OSCC patients and controls. In case of IL-1?, IL-1?, IL-6, IL-8, TNF-? and VEGF, the salivary concentrations were determined using Luminex-based multiplex immunobead assay.Based on our results, the levels of protein S100A9, thioredoxin, IL-6 and TNFa were significantly altered in OSCC patients compared to controls. The ROC analysis revealed the S100A9, thioredoxin and IL-6 as potential OSCC biomarkers applicable for the Hungarian population. In order to verify the results obtained by Luminex and SRM analyses, ELISA experiments were carried out on a multi centric study from the patients of the four University Medical Centers (Debrecen, Budapest, Pécs and Szeged) in Hungary. The salivary concentrations of IL-6, protein S100A9 and thioredoxin were determined. Based on our results the IL-6 seem to be useful biomarkers for OSCC detection in the Hungarian population.This research was supported by the Hungarian Scientific Research Fund OTKA K105034.
