Megjegyzések:Background. Cerebral thromboembolism is a rare but feared complication of transcatheter ablation in patients with atrial fibrillation (AF). Here, we aimed to test which pre-procedural anticoagulation strategy results in less intracardiac activation of hemostasis during ablation. Patients and methods. In this observational study, 54 paroxysmal / persistent AF patients undergoing cryoballoon ablation were grouped according to their periprocedural anticoagulation strategy: no anticoagulation (oral anticoagulation (OAC) free; n = 24), uninterrupted vitamin K antagonists (VKA) ( n = 11), uninterrupted dabigatran ( n = 17). Blood was drawn from the left atrium before and immediately after the ablation procedure. Cryoablations were performed according to standard protocols, during which heparin was administered. Heparin-insensitive markers of hemostasis and endothelial damage were tested from intracardiac samples: D-dimer, quantitative fibrin monomer (FM), plasmin-antiplasmin complex (PAP), von Willebrand factor (VWF) antigen, chromogenic factor VIII (FVIII) activity. Results. D-dimer increased significantly in all groups post-ablation, with lowest levels in the dabigatran group (median [interquartile range]: 0.27 [0.36] vs. 1.09 [1.30] and 0.74 [0.26] mg / L in OAC free and uninterrupted VKA groups, respectively, p < 0.001 ). PAP levels were parallel to this observation. Post-ablation FM levels were elevated in OAC free ( 26.34 [30.04] mg / L ) and VKA groups (10.12 [16.01] mg / L), but remained below cut-o in all patients on dabigatran ( 3.98 [2.0] mg / L; p < 0.001). VWF antigen and FVIII activity increased similarly post-ablation in all groups, suggesting comparable procedure-related endothelial damage. Conclusion. Dabigatran provides greater inhibition against intracardiac activation of hemostasis as compared to VKAs during cryoballoon catheter ablation.

Megjegyzések:AbstractTreatment of atrial fibrillation (AF) with catheter ablation has proven to be a safe and effective treatment modality which is offered to an increasing number of patients in many centers. Pulmonary vein isolation (PVI) is an established cornerstone of AF ablation strategies. Athough the isolation of the pulmonary veins (PVs) with irrigated focal radiofrequency (RF) catheters using a point-by-point method is considered as the gold standard, it can be challenging to create contiguous lesions, time consuming, and require advanced three dimensional (3D) mapping and navigational systems. The phased RF ablation system was designed to address many of these challenges associated with conventional focal RF ablation. In this review, we describe the main features of phased RF ablation and summarize the data available on clinical outcome with this technology.

Megjegyzések:Background: Clinically silent cerebral ischemia (SCI) detected by diffusion-weighted MRI hasbeen reported in 5-40% of patients undergoing pulmonary vein isolation (PVI). Although initial reportssuggested a high rate of SCI with phased radiofrequency (RF) ablation on use of the pulmonary veinablation catheter (PVAC), the incidence was subsequently markedly reduced in consequence ofprocedural modifications in recent studies.We analyzed cerebral microembolization as assessed with transcranial Doppler during phasedRF ablation and with two other single-shot AF ablation technologies: the cryoballoon (CB) and thenMARQTM multipolar irrigated RF ablation system.Methods and results: A total of 89 patients (mean age: 57, SD: 12 years; 62 male) withparoxysmal or persistent AF, underwent PVI. Phased RF was used according to the initial protocol in 7patients (PVAC Group I), with procedural modifications and a newer (14.4) version of the RF generatorin 37 patients (PVAC Group II) and with the most recent (version 15.0) generator in 18 patients (PVACGroup III). Ablation was performed with the CB in 13 and with the nMARQ system in 14 patients.

Megjegyzések:AbstractBackground: High incidences of silent cerebral ischemia (SCI) have been revealed bydiffusion-weighted magnetic resonance imaging (DW MRI) after pulmonary vein isolation(PVI) for atrial fibrillation. A high number of mostly gaseous micro-embolic signals (MESs)was detected by transcranial Doppler (TCD) during PVI. In this investigation the possiblerelationship between MESs detected intraoperatively by TCD and new SCI on DW MRI postablationis reported.Methods: 27 consecutive atrial fibrillation patients (6 female, age median: 64 interquartilerange: 13.23) undergoing PVI with the pulmonary vein ablation catheter (PVAC), pre- andpost-ablation DW MRI and intraoperative MES detection by TCD were included in the study.Procedures were performed on a therapeutic international normalized ratio (2?3) and with atarget activated clotting time ? 350 s in all patients. DW MRI scans performed pre- and postablationrevealed new SCI in 6 out of 27 (22%) patients.Results: The median (interquartile range) MES count recorded during the whole procedurewas 1642 (912) in patients with and 1019 (529) in those without SCI (p = 0.129). The numberof MESs recorded during PV angiography was significantly higher in patients as compared tothose without a new lesion on the post-ablation DW MRI: 257 (249) vs. 110 (71), respectively(p = 0.0009). On multivariate logistic regression, the total MES count was predictive of SCIin patients older than 68 years.Conclusions: Micro-embolus generation detected by TCD during PV angiographysignificantly correlates with new SCI on DW MRI post-ablation.

Megjegyzések:Background: Pulmonary vein isolation with phased radiofrequency current and use of a pulmonary vein ablation catheter (PVAC) has recently been associated with a high incidence of clinically silent brain infarcts on diffusion-weighted MRI, and a high microembolic signal (MES) count detected by transcranial Doppler. Objective: to investigate the potential correlation between different biophysical parameters of energy delivery (ED) and MES generation during PVAC ablation.Methods: MES counts during consecutive PVAC ablations were recorded for each ED and time-stamped for correlation with temperature, power and impedance data from the GENius 14.4 generator. Additionally, catheter-tissue contact was characterized by the template deviation score, calculated by comparing the temperature curve with an ideal template representing good contact, and by the respiratory contact failure score, to quantify temperature variations indicative of intermittent contact due to respiration.Results: A total of 834 EDs during 48 PVAC ablations were analyzed. A significant incease in MES count was associated with a lower average temperature, a temperature integral over 62 ?C, a higher average power, the total energy delivered, higher respiration and template deviation scores (p<0.0001), and simultaneous ED to the most proximal and distal poles of the PVAC (p<0.0001).Conclusions: MES generation during ablation is related to different indicators of poor electrode-tissue contact, the total power delivered and the interaction between the most distal and the most proximal electrodes.
