Megjegyzések:RESUME : L'histoire du schwannome du nerf acoustique (SV)montre un déplacement du ?standard de traitement? de lamicrochirurgie vers la radiothérapie. Pour ce type de tumeursbénignes, le traitement a pour but d'éviter les complicationsfonctionnelles et de préserver la capacité auditive. Aprℓes microchirurgiepour une lésion de moins de 25 mm, on observera quele nerf facial est indemne dans approximativement 60% descas et que la fonctionnalité du VIII est préservée dans 15% descas pour des SV de plus de 15 mm et, dans 50% des cas, pourdes SV de moins de 15 mm. Les différents progrℓes techniqueset informatiques dans le domaine de la radiothérapie permettentaujourd'hui d'obtenir un taux de contr©ole tumoral variantde 92 ℓa 98% avec une morbidité quasi inexistante au niveaudu nerf facial (<2%). La capacité auditive est maintenue dansapproximativement 70% des cas. Forts de ces chiffres, on peutestimer que la prise en charge de ce type de tumeurs bénignesdoit se faire par radiothérapie.ENGLISH: I beg your pardon ? I can be treated by another modality thansurgery for my vestibular schwannomaSUMMARY : The history of the treatment of vestibularschwannoma (VS) clearly shows a shift from microsurgerytowards radiotherapy as "standard therapy". The microsurgicalapproach obviously yields excellent tumour control, but is hamperedby a high morbidity rate, especially at the level of cranialnerves. The literature reports a normal facial nerve function inonly 60% of patients surgically treated for VS with a maximumdiameter of 25 mm. Hearing preservation is only around 15%for tumours >15 mm and around 50% for tumours less than 15mm. Progress in techniques and computer sciences in the fieldof radiotherapy provides the opportunity to reach high levels oftumour control (92 ? 98%), keeping cranial nerve morbidity ona very low level (<2%). Hearing capacity and serviceable hearingcan be maintained in about 70% of cases. Based on theseresults, one can assume that radiation therapy should be usedupfront for those benign tumours.
